Physical Demands
Ability to walk/stand for long periods of time.
Use correct body mechanism to position patients
Assist patient's movement to BSC, chair, and ambulating.
ESSENTIAL FUNCTIONS
Ability to perform complete and accurate vital signs.
Consistently rounds on patients to anticipate needs.
Passes meal trays in a timely manner, providing wet cloth for patients use. Ensures that all patients who are able, are sitting up and are comfortable for the meal. Picks up meal trays in a timely manner.
Feeds patients who are unable to feed themselves.
Provides ice water at least once every shift to bedside for patients who are able to have ice water. Removes additional fluids from patients on fluid restrictions.
Offers/Provides bath, linen change to patients who require assistance or complete care, and documents on activity record. Notifies licensed personnel if patient refuses bath and/or linen change, and documents on activity record.
Obtain intake and output as directed by physician or plan of care.
Update patient board in room daily.
Assists in admission process by obtaining vital signs, providing personal care items and room orientation.
Utilization of critical thinking skills in prioritization of care.
Ability to identify changes in patient condition and notifies licensed personnel of any change in patient condition.
Provide safe environment.
Documents all patient and family teaching as requested/necessary.
Demonstrates correct use and troubleshooting of equipment.
Participates in the development, prioritization, and evaluation of individualized plan of care.
Assist in modification of plan based on patient's response. Provides information directly to the physician and documents appropriately.
Ability to take directions from licensed and administrative professionals. Attempts to resolve conflict independently, if unable, seeks the assistance of department leader.
Provides health care at the best value without compromising patient care. Utilizes the Pyxis appropriately, obtains items under specific patient's name and returns unused items.
Responsible and accountable for all actions.
Assist with chart completion and copying of all paperwork.
Coordinates with nursing supervisor and/or materials management for supply items that cannot be obtained from the Pyxis. Ensures chart is documented and changes are added to the patient charge sheet.
Provides assistance to the physician during examination of patient.
Transports patient to procedures as needed.
Regularly attends monthly staff meetings.

WHAT WE'RE LOOKING FOR:

SUBJECT: - CERTIFIED NURSING ASSISTANT (CNA)
REPORTS TO: Supervisor
PRIMARY FUNCTION: Certified Nursing Assistant (CNA) will provide quality care and support to the individual, will assist in carrying out implementations of outcomes defined by the Support Plan, and will (by reports and verbal communication) keep their Supervisor, Residential Services Coordinator and Associate Director informed of all problems as they relate to the Program. CNAs will have input into plans for the individuals for whom they have responsibility.
RESPONSIBILITIES
1. Safety
The first concern is the safety of each individual. CNAs must always see that individuals are receiving appropriate supervision. CNAs are to follow all safety and security plans and procedures as they relate to the setting ie: residential, work, recreational. CNAs must schedule their breaks to ensure that individuals always have adequate supervision. It is never permissible for two staff to be on break at the same time.
2. Programming
A maximum degree of constructive interaction with an individual is to be maintained. The CNA is to follow the program as designed in both carrying out general routine and implementing the Person Centered Support Plan/Individual Support Plan for each individual including record keeping as designated in all locations where staff are assigned. Communication and cooperation with other staff and with supervisor are essential for overall program coordination. It may be necessary for CNAs to work hours or in programs other than their designated assignment. CNAs are expected to collect data and enter this documentation in the electronic record or on paper data sheets, as applicable.
Teamwork is essential. The Supervisor must be informed of all outings and activities outside of the home. The Supervisor must also be informed of any visitor to the home.
3. Dignity and Respect
In providing support, the CNA encourages individuals to make choices and decisions about areas that impact their lives. Activities in which individuals participate are to be of their choice and CNAs are there to support these choices. CNAs will treat individuals with dignity and respect and with sensitivity to an individual's feelings, wants and needs. Outings and activities are planned with input from the individual and person's interests in mind.
4. Individual Support
The CNA will give individuals opportunities to participate in their daily routine. However, the CNAs have the responsibility for all support necessary for individual functioning if the individual cannot complete the tasks thoroughly. Examples of such functions include, but are not limited to, assistance with bathing, grooming, arranging hair appointments, preparation of meals, and laundering clothes. CNAs will provide support and assistance to individuals in any employment setting in which they are placed. CNAs will assist individuals in maintaining appearance and dress appropriate for the occasion. Support is provided to the minimum degree necessary in order to promote the growth of independence.
5. Facility Maintenance
CNAs have the responsibility for the care and maintenance of the physical facility. They are expected to perform routine maintenance and cleaning functions. Examples of such functions include, but are not limited to, sweeping, dusting, cleaning of bathroom fixtures, and bed making. CNAs are expected to take pride in the appearance of the facility and include individuals in house maintenance to promote training and independence. CNAs are expected to keep the exterior grounds of the facility free of trash, clutter, and cigarette butts. Any maintenance concerns will be reported to management.
6. Transportation
A CNA will be responsible for transporting individuals to and from work, community activities, etc. as is necessary.
7. Other
A CNA will perform other duties as assigned by their supervisor or other management.
