Job DescriptionHospice RN: Registered Nurse Northeast Ohio Hospice Full Time - Days (Territory of Western Cuyahoga and Lorain Counties) Our Hospice RN - Northeast Ohio Hospice oversees primary caseload of Hospice patients and provides skilled professional nursing care to them, their families and caregivers. Provides the initial assessment and comprehensive nursing assessment of patient needs and collaborates to develop the Hospice Interdisciplinary Team (IDT) plan of care. Implements the plan of care and reevaluates and revises the plan of care relative to the patient's nursing needs to provide appropriate, comprehensive and responsive goals and interventions. Works closely with other members of the IDT to ensure all patient and family care needs are being met. Communicates the patient's progress with the patient/caregiver and other disciplines as well as directs, supervises and instructs direct care staff members and non-professional hospice aide staff to deliver quality care and coordinates delivery of care services. Requirements for a Hospice RN: Registered Nurse: * Graduate of an accredited registered nursing program. * Bachelor's degree in Nursing preferred. * Certified Hospice and Palliative Nurse (CHPN) certification preferred. * Minimum of 2 to 4 years as a RN with experience in a hospice or home health agency or other related health program. * Experience with pain and symptom management interventions preferred. * Ability to work within an interdisciplinary setting. * Understanding of Hospice philosophy, principles of death/dying. * Must have a current Registered Nurse license in good standing in the State of Ohio in accordance with the Ohio Board of Nursing in accordance with the Ohio Revised Code 4723 * Valid driver's license and reliable transportation that is insured in accordance with Ohio state requirements. $81k-99k yearly est.7d agoSTNA - Hospice Aide
