PHYSICIAN (FAMILY PRACTICE)
Jericho Road Community Health Center is actively seeking a Physician (Family Practice) . This position is a full-time, within the Medical Care Department, working across all medical sites.
Responsibilities:
Maintain electronic medical records efficiently and maximize resources.
Provide high quality primary care for adult, pediatric, and obstetric patients of Jericho Road Community Health Center.
Offer comprehensive, patient- and family-centered care.
Ensure consistent, cost-effective care.
Provide inpatient adult, pediatric, and obstetric care as part of a call pool with other family medicine and OB/GYN physicians.
Provide patient care in a wholistic manner including attention to physical, mental, emotional, and spiritual health.
Collaborate with interdisciplinary clinical and program teams at Jericho Road Community Health Center.
Create a learning plan that includes familiarity with new research and changes the standard of practice.
Participate in quality improvement activities with a data-driven approach.
Willingness to work with family medicine and pediatrics residents.
Attending required staff and provider meetings except clinical activities prevents such attendance.
Qualifications:
Doctor of Medicine or Doctor of Osteopathy, licensed to practice in New York State prior to start date of employment
Completion of three-year or four-year Family Medicine Residency
Board-eligible from American Board of Family Medicine. Board-certified within one year of employment.
Passion for high quality primary health care and community development among a culturally and linguistically diverse urban population

ACUTE CARE MEDICAL PROVIDER
Jericho Road Community Health Center is actively seeking a Acute Care Medical Provider . This position is a full-time, within the Medical Care Department, working across all medical sites.
Responsibilities:
Improve patient outcomes and deliver quality care within practice areas as assigned.
Provide high quality acute medical care for adult, pediatric, and basic gynecology patients of Jericho Road Community Health Center
Follow up with patients regarding lab and imaging results
Ensure all clinical care is delivered in a manner that meets or exceeds goals and expectations for clinical outcomes, quality assurance standards, state/local regulations, and patient satisfaction.
Participate in an office call pool for patient phone calls overnight. Work one Saturday morning clinic per month.

PSYCHIATRIST
Jericho Road Community Health Center is actively seeking a Psychiatrist. This position is full-time, within the Medical Care Department, working across our Barton, Broadway, & Doat locations.
Responsibilities:
Conduct thorough psychiatric evaluations, including medical history, mental status examinations, and psychosocial assessments to diagnose mental health conditions
Develop and implement individualized treatment plans based on patient needs, which may include pharmacotherapy, psychotherapy, and behavioral interventions
Prescribe and manage psychotropic medications, including monitoring their effectiveness, potential side effects, and interactions with other medications
Provide follow-up care, assess treatment progress, and modify treatment plans as necessary
Perform crisis intervention and provide emergency psychiatric care when required
Collaborate with patients and their families to educate them about mental health conditions, treatment options, and medication adherence
Work with diverse patient populations, including those with co-occurring substance use disorders, trauma, and chronic medical conditions
Collaborate with primary care providers, behavioral health team, and other healthcare professionals to deliver integrated, holistic care to patients.

Provide hands-on patient care for all visits in schedule, including examinations during hygiene visits
Examine, review and consult patients' medical records for evaluation, diagnostic and treatment decisions
Check teeth, gums and other parts of the mouth, along with X-rays and tests, to diagnose dental problems
Advise patients regarding the best course of treatment and provide a customized exam and plan for every patient
Prepare patients for treatment, including sedation or anesthetic application

Work with JRCHC providers by scheduling and confirm patient diagnostic appointments, surgeries, and other specialty appointments that may not be a direct service of JRCHC
Provide customer and personal services, including customer needs assessment, meeting quality standards for services, and evaluation of customer satisfaction
Practice high level of ethical behavior while interacting with patients and appropriate staff
Practice cultural sensitivity while working with JRCHC patients and staff
Assist with front office receptionist responsibilities as needed
Assist with answering incoming calls from call center as needed

Receive and route messages or documents to appropriate staff

Answer transfer patient phone calls as needed for referral questions
Non-Clinical responsibilities: OB triage box, SNC scheduling, Deaf Access scheduling
Mail out appointments reminders to patients home or push it via patient portal
Closing of the referral loop, this is a mandatory QI initiative that we have to report on, still not implemented but will this year. Need to get records from specialist office for any referrals that provider sent to PN
Qualifications:
High School Graduate or GED Equivalent
Demonstrated ability and familiarity with tools, technology, and systems found within health care environments (such as Basic Computer Skills, Electronic Medical Records, Experience with Medent EMR a plus).
At least 1 year of experience in working in a medical office
Ability to demonstrate and model professionalism, candor and care of patients, visitors, and organizational partners

Conduct patient screening procedures, such as assessment of oral health conditions, review of the health history, oral cancer screening, head and neck inspection, dental charting and taking blood pressure
Removing calculus and plaque (hard and soft deposits) from all surfaces of the teeth
Applying preventive materials to the teeth (e.g., sealants and fluorides)
Sterilize dental instruments and tools
Instruct patients of all ages on proper dental hygiene and care
Take X-rays of patients' teeth and develop them for dentist's use
Provide chair-side assistance during dental procedures
Regularly and consistently meet quality and patient care standards established by management.
Assist in patient recruitment and growth of clinic
Performing documentation and establishing and maintaining patients' history

* Assigns diagnostic codes from provider documentation, entering essential information into practice management system
* Follow the official coding guidelines including Encoder Pro, CPT Assistant, CMS Documentation Guidelines, Official Guidelines for Coding and Reporting and other similar authoritative resources.
* Regularly and consistently meet quality and productivity standards established by management.
* Review provider documentation and abstract diagnosis codes, procedure codes and supply codes.

Responsibilities:
Assigns diagnostic codes from provider documentation, entering essential information into practice management system
Follow the official coding guidelines including Encoder Pro, CPT Assistant, CMS Documentation Guidelines, Official Guidelines for Coding and Reporting and other similar authoritative resources.
Regularly and consistently meet quality and productivity standards established by management.
Review provider documentation and abstract diagnosis codes, procedure codes and supply codes.
Complete administrative tasks, such as data reporting, in a timely manner.
Communicate with management regarding coding workload, turnaround time expectations and deliverables.
Email providers as needed
Participate in department meetings, coding training, organizational mandatory training, and compliance training.
