DEPARTMENT: ADRC ACCOUNTABILITY: ADRC Director STATUS: Exempt, Full-time - Hybrid schedule Under direction of ADRC Director, performs needs assessments, counseling and referrals, preliminary care planning and short-term tracking for persons referred by a Nursing Home Social Worker that have stated their interest in resources related to home and community-based services.
