* An individual that possess a bachelor's degree in a human services field (including but not limited to: sociology, special education, rehabilitation counseling or psychology).
* A minimum of one year of supervised experience in mental health or human services is preferred.

Responsibilities:
Provides case management services in the community to individuals with intellectual and developmental disabilities and their families
Facilitates linking, advocating, coordinating, and monitoring services; to assist consumers in gaining access to health services,
financial assistance, employment, education, social services, and natural supports
Assesses needs, develops, and monitors Person Centered Plans and documents treatment
Qualifications:
Bachelor in Social Work or Master's Degree in an appropriate mental health discipline
QIDP, one years experience providing treatment to individuals with Intellectual and Developmental Disabilities
Must have reliable means of transportation for home visits and maintain Safe Driving Permit
