Formulate initial diagnostic impression and appropriate recommendations for level of care placement. 5. Provide case management and counseling services to assigned clients. 6. Utilize intervention skills as required to place the client in the appropriate level of treatment services. 7. Maintain knowledge of community, school and recovery resources in order to make appropriate interagency referrals as required. 8. Provide networking, linkage and follow-up with referral sources. 9. Maintain communications with all staff (e.g. assessments, admissions). 10. Meet with all individuals being admitted to inpatient treatment and provide warm hand-off to admissions clinician whenever possible. 11. Have Client Accounts Representative meet with the client if needed to discuss co-pay and get down payment when appropriate 12. Confirm that client has medications and appropriate medical records needed for admission. 13. Conduct pre-certification call for insurance if needed. 14. Notify appropriate staff of client status during admissions process. 15. Ensure that client health records are maintained consistent rules and regulations. 16. Assume on-call responsibilities as scheduled or when necessary. 17. Serve as a member of the treatment team and participate in all team meetings and activities.
