The Inpatient Case Manager is responsible for the assessment, treatment planning, intervention, monitoring, evaluation and documentation on identified patients during an acute care episode. Assesses and develops a care plan in collaboration with the admitting, attending and consultant physicians, and other healthcare practitioners. The goal of the Inpatient Case Manager is to effectively manage the patients during their hospitalization to assure the right level of care is provided for the patient's current severity of illness, utilizing the appropriate tests and diagnostics, in the appropriate facilities in order to provide the highest level of care, in the most efficient manner, while utilizing only the resources necessary for that episode of care. The Inpatient Case Manager is also responsible for the safe discharge of patients with the necessary resources to insure that care is provided without interruption along the continuum of care, that is, whether in a skilled nursing facility, assisted living facility or in the home setting. Such discharge will be planned and coordinated in collaboration with the Vital Care Case Manager for patients enrolled in the Vital Care program. The Inpatient Case Manager will document the discharge plan, together with the current medication list, and scheduled follow-up appointments in the authorization notes and will communicate the same to the Vital Care Case Manager, the patient, and family members concerned. A follow-up post-discharge call will be made by the Inpatient Case Manager or the department's designee within 24 hours of discharge to determine any problems, issues, or risks encountered in the transition.

Comply with and implement the Medical Q.A (Quality Assurance) and Clinical Risk Management Policies, including:
Evaluate the patients for any high-risk conditions (e.g., liver failure, pregnancy, overdose, bipolar, schizophrenia, etc.), which may endanger the patient (suicidal) or other parties (homicidal).
Administer the BAM-R, C-SSRS, GAD-7, and PHQ-9 screening tools to determine the severity of the patient's mental/emotional status.
Evaluate the risk factors concerning each such case, considering the appropriateness of, and professionalism in treating such individuals in an OTP in comparison to treatment in a residential facility or other treatment modalities.
Review and ask about other practitioners' involvement in the treatment of these high-risk conditions. Recommend immediate referral to such a specialist in case of need. Request the patient sign a waiver for release of information which will allow communication with the other practitioner in case of need (e.g., deterioration of the patient's condition)
Flag high-risk patients to the Medical Directors, Physician Extenders, and Regional Clinical Leadership (RCL) and highlight the conditions concerned.
Refer such patients to the Medical Directors, Physician Extenders or RCLs (Regional Clinical Leadership) based on the urgency, utilizing the informal Referral Policy or the scheduled Treatment Team/Case Conference session.
Review physician notes, following the Medical Directors or Physician Extenders' session with the patient, verifying that they noted and addressed the high-risk factor, or otherwise bring it immediately to the attention of the supervising counselor, as well as the clinic manager, until the matter's proper and professional handling by the physician or physician extender.
Advise, in writing, other staff members (including the clinic manager, supervising counselor, dispensing nurse and front office) of high-risk patients and their condition. Maintain and circulate minutes from treatment team/case conference sessions to the other staff members.
Provide professional counseling and referral services by:
Addressing patients' clinical and special needs by maintaining referrals to specialized (“second tier”) services rendered by physicians, physician extenders and counselors within Aegis, as well as referrals to Keys to Recovery (or other twelve-step) support groups, and other community resources (e.g., agencies and practitioners outside of Aegis), acting as a liaison to these agencies, as necessary.
Providing individual and group counseling as clinically necessary, to address the problem issues identified in the Initial Assessment and Treatment Plans. The counseling service will include protocols for the treatment of Relapse Prevention, Peer Pressure, Anger Management, Domestic Violence, Parenting, Family Preservation, Vocational Rehabilitation, etc.
Utilizing advanced counseling techniques such as Cognitive Behavioral Therapy, Motivational Interviewing, etc.
Scheduling regular counseling sessions with patients; issue patient appointment cards and record scheduled or rescheduled sessions in EHR scheduler.
Documenting counseling entries, including signature and date, as required by applicable federal and state regulations, CARF guidelines and Pinnacle policies.
Engaging in Community Relations and Outreach efforts.
Attending all recommended training including Physician Training of Counselors, RCL training, and case conferences with the program physicians and physician assistants.
Collecting research data when requested.
Observing the collection of patients' urine samples when required

Key Responsibilities:
Provides orientation to new patients to the programs' rules, policies, procedures, and patients' rights.
Conduct Intakes, Discharge Planning, Initial Assessments, and Initial Treatment Plans professionally and within time guidelines as required by applicable federal and state regulation, CARF guidelines and Pinnacle Policies.
Involve patients in the assessment, treatment planning, and intervention aspects of treatment, which provides the patient with a sense of entitlement to their own recovery.
Administer the biopsychosocial assessment, which evaluates all areas of patients' lives, including their addictions, medical and mental health history, socioeconomics, legal, etc. at intake and on an ongoing basis.
Recognize and utilize patients' Strengths, Needs, Ability, and Preferences (SNAP), to develop and implement individualized Treatment Plans that effectively help patients to meet their short & long-term goals. • Determine the clinical necessity of counseling services based on the clinical assessment and evaluation of the patients.
Monitor closely, and document any change of circumstances with the patients, especially as it relates to matters that may affect their recovery, identifying and addressing relapse indicators to promote relapse prevention interventions in an early stage.
Comply with and implement the Medical Q.A (Quality Assurance) and Clinical Risk Management Policies, including:
Evaluate the patients for any high-risk conditions (e.g., liver failure, pregnancy, overdose, bipolar, schizophrenia, etc.), which may endanger the patient (suicidal) or other parties (homicidal).
Administer the BAM-R, C-SSRS, GAD-7, and PHQ-9 screening tools to determine the severity of the patient's mental/emotional status.
Evaluate the risk factors concerning each such case, considering the appropriateness of, and professionalism in treating such individuals in an OTP in comparison to treatment in a residential facility or other treatment modalities.
Review and ask about other practitioners' involvement in the treatment of these high-risk conditions. Recommend immediate referral to such a specialist in case of need. Request the patient sign a waiver for release of information which will allow communication with the other practitioner in case of need (e.g., deterioration of the patient's condition)
Flag high-risk patients to the Medical Directors, Physician Extenders, and Regional Clinical Leadership (RCL) and highlight the conditions concerned.
Refer such patients to the Medical Directors, Physician Extenders or RCLs (Regional Clinical Leadership) based on the urgency, utilizing the informal Referral Policy or the scheduled Treatment Team/Case Conference session.
Review physician notes, following the Medical Directors or Physician Extenders' session with the patient, verifying that they noted and addressed the high-risk factor, or otherwise bring it immediately to the attention of the supervising counselor, as well as the clinic manager, until the matter's proper and professional handling by the physician or physician extender.
Advise, in writing, other staff members (including the clinic manager, supervising counselor, dispensing nurse and front office) of high-risk patients and their condition. Maintain and circulate minutes from treatment team/case conference sessions to the other staff members.
Provide professional counseling and referral services by:
Addressing patients' clinical and special needs by maintaining referrals to specialized (“second tier”) services rendered by physicians, physician extenders and counselors within Aegis, as well as referrals to Keys to Recovery (or other twelve-step) support groups, and other community resources (e.g., agencies and practitioners outside of Aegis), acting as a liaison to these agencies, as necessary.
Providing individual and group counseling as clinically necessary, to address the problem issues identified in the Initial Assessment and Treatment Plans. The counseling service will include protocols for the treatment of Relapse Prevention, Peer Pressure, Anger Management, Domestic Violence, Parenting, Family Preservation, Vocational Rehabilitation, etc.
Utilizing advanced counseling techniques such as Cognitive Behavioral Therapy, Motivational Interviewing, etc.
Scheduling regular counseling sessions with patients; issue patient appointment cards and record scheduled or rescheduled sessions in EHR scheduler.
Documenting counseling entries, including signature and date, as required by applicable federal and state regulations, CARF guidelines and Pinnacle policies.
Engaging in Community Relations and Outreach efforts.
Attending all recommended training including Physician Training of Counselors, RCL training, and case conferences with the program physicians and physician assistants.
Collecting research data when requested.
Observing the collection of patients' urine samples when required
Attend team meetings and complete all training courses timely as required.

Qualifications:
Counselor I: $20/hour to $21/hour
Possess an associate degree in relevant field and has the registration/credentials necessary to provide counseling services to our patient population.
Kern County requires a bachelor's degree or High school diploma/GED with 75% certification education.

Parent

Physical needs: Ensure campers have everything they need to stay healthy at camp. Think about visuals and other supports that campers might need in order to get their physical needs met.

Communicate with counselors, specialists, and A/UHs throughout the day regarding accommodations and modifications needed for campers with special needs, and to provide those services whenever possible.

Celebrate the campers for who they are! Your job is to help kids succeed and cheer them on along the way!

Limit Setter
Support campers in being on time and prepared for each activity to the extent that is possible for them - if a camper needs more transition time, brainstorm ways to make it work for them to participate as much as they can.
Track patterns of camper behavior and work with campers and counselors to figure out the root cause of the behavior. Review camper file notes and work with counselors and therapists to appropriately respond to the behavior based on the circumstances.
Brainstorm camper challenges at IC meetings and develop strategies to best support individual campers.
Use different tones and energy for different activities such as a sports block versus a meditation block.
Manage camper behavior, enforce rules and hand out appropriate consequences as needed.
Day in the Life of an Inclusion Counselor
7:30AM - Meet at assigned camper bunk and help kids with higher needs prepare for the day and transition to breakfast
8:00AM - Breakfast (followed by song session): check in on all campers you're working with and touch base with counselors and unit heads about the day ahead
9:30AM - Activities Begin: You'll be assigned to one of the bunks in your unit. Support the campers who need the most help to succeed in the activities. Implement accommodations and modifications where needed.
12:30PM - Lunch: Sit with your assigned bunk during lunch
1:30PM - Rest Hour: Work closely with campers who need support during this unstructured down time. Facilitate social relationships and unstructured activities in the bunk.
2:30PM - Activities: You'll be assigned to one of the bunks in your unit. Support the campers who need the most help to succeed in the activities. Implement accommodations and modifications where needed.

Key Responsibilities:
Provides orientation to new patients to the programs' rules, policies, procedures, and patients' rights.
Conduct Intakes, Discharge Planning, Initial Assessments, and Initial Treatment Plans professionally and within time guidelines as required by applicable federal and state regulation, CARF guidelines and Pinnacle Policies.
Involve patients in the assessment, treatment planning, and intervention aspects of treatment, which provides the patient with a sense of entitlement to their own recovery.
Administer the biopsychosocial assessment, which evaluates all areas of patients' lives, including their addictions, medical and mental health history, socioeconomics, legal, etc. at intake and on an ongoing basis.
Recognize and utilize patients' Strengths, Needs, Ability, and Preferences (SNAP), to develop and implement individualized Treatment Plans that effectively help patients to meet their short & long-term goals. • Determine the clinical necessity of counseling services based on the clinical assessment and evaluation of the patients.
Monitor closely, and document any change of circumstances with the patients, especially as it relates to matters that may affect their recovery, identifying and addressing relapse indicators to promote relapse prevention interventions in an early stage.
Comply with and implement the Medical Q.A (Quality Assurance) and Clinical Risk Management Policies, including:
Evaluate the patients for any high-risk conditions (e.g., liver failure, pregnancy, overdose, bipolar, schizophrenia, etc.), which may endanger the patient (suicidal) or other parties (homicidal).
Administer the BAM-R, C-SSRS, GAD-7, and PHQ-9 screening tools to determine the severity of the patient's mental/emotional status.
Evaluate the risk factors concerning each such case, considering the appropriateness of, and professionalism in treating such individuals in an OTP in comparison to treatment in a residential facility or other treatment modalities.
Review and ask about other practitioners' involvement in the treatment of these high-risk conditions. Recommend immediate referral to such a specialist in case of need. Request the patient sign a waiver for release of information which will allow communication with the other practitioner in case of need (e.g., deterioration of the patient's condition)
Flag high-risk patients to the Medical Directors, Physician Extenders, and Regional Clinical Leadership (RCL) and highlight the conditions concerned.
Refer such patients to the Medical Directors, Physician Extenders or RCLs (Regional Clinical Leadership) based on the urgency, utilizing the informal Referral Policy or the scheduled Treatment Team/Case Conference session.
Review physician notes, following the Medical Directors or Physician Extenders' session with the patient, verifying that they noted and addressed the high-risk factor, or otherwise bring it immediately to the attention of the supervising counselor, as well as the clinic manager, until the matter's proper and professional handling by the physician or physician extender.
Advise, in writing, other staff members (including the clinic manager, supervising counselor, dispensing nurse and front office) of high-risk patients and their condition. Maintain and circulate minutes from treatment team/case conference sessions to the other staff members.
Provide professional counseling and referral services by:
Addressing patients' clinical and special needs by maintaining referrals to specialized (“second tier”) services rendered by physicians, physician extenders and counselors within Aegis, as well as referrals to Keys to Recovery (or other twelve-step) support groups, and other community resources (e.g., agencies and practitioners outside of Aegis), acting as a liaison to these agencies, as necessary.
Providing individual and group counseling as clinically necessary, to address the problem issues identified in the Initial Assessment and Treatment Plans. The counseling service will include protocols for the treatment of Relapse Prevention, Peer Pressure, Anger Management, Domestic Violence, Parenting, Family Preservation, Vocational Rehabilitation, etc.
Utilizing advanced counseling techniques such as Cognitive Behavioral Therapy, Motivational Interviewing, etc.
Scheduling regular counseling sessions with patients; issue patient appointment cards and record scheduled or rescheduled sessions in EHR scheduler.
Documenting counseling entries, including signature and date, as required by applicable federal and state regulations, CARF guidelines and Pinnacle policies.
Engaging in Community Relations and Outreach efforts.
Attending all recommended training including Physician Training of Counselors, RCL training, and case conferences with the program physicians and physician assistants.
Collecting research data when requested.
Observing the collection of patients' urine samples when required
Attend team meetings and complete all training courses timely as required.
Other duties as assigned.
Qualifications:
Counselor I: $20/hour to $21/hour
Possess an associate degree in relevant field and has the registration/credentials necessary to provide counseling services to our patient population.
Kern County requires a bachelor's degree or High school diploma/GED with 75% certification education.

Counselor II: $21/hour to $24/hour

2-5 years of experience OR Master level
Counselor III: $24/hour to $27/hour (certified), $27/hour to $30/hour (licensed)
Degrees must include being licensed, licensed eligible, or certified. Clinical skills must be effective in complex cases. Along with the below:
Possess certification as an Alcohol and Other Drug Counselor with a CA approved certification agency or current credentials as licensed or license-eligible with the Board of Behavioral Sciences or the Board of Psychology. If license-eligible, must maintain the Board's requirements to be able to practice including receiving the necessary amount of clinical supervision by a Board approved supervisor.

Job Description: Tawonga counselors live in camper cabins and work with bunks of 12 campers. Counselors strive to raise self-esteem and foster the bunk group dynamic by actively participating in all camp activities offered by specialists, including: ropes course, backpacking, arts & crafts, Jewish programs, and more, as well as creating programs of their own. Counselors work most sessions as a Bunk Counselor (with a co-counselor in a 12 camper cabin), but may also spend one or more sessions as a Unit Counselor supporting multiple bunks.
Job Responsibilities
Parent
As a parent you must be aware of the physical and emotional well-being of all 12 campers' at all times.
Physical needs: Ensure campers have everything they need to stay healthy at camp
Emotional needs: Make sure that the campers are feeling good about themselves, understanding that campers' identities are multifaceted, and their relationships in the cabin.
Being a “parent” to 12 kids, 19 hours a day means giving kids plenty of TLC (tender loving care) and forming relationships with every camper in the cabin.
Cheerleader
Be excited and positive about the day's schedule regardless of what your personal feelings are. Be positive about both the bunk as a whole and individual campers.
Bring positivity, energy, enthusiasm, and excitement! Make it fun!
Limit Setter
Make sure that the campers are on time and prepared for each activity.
Use different tones and energy for different activities such as a sports block versus a meditation block.
Manage camper behavior, enforce rules and hand out appropriate consequences as needed.
A Day in the Life of a Counselor
7:00AM - Wake up: get entire bunk of kids up and ready for the day
8:00AM - Breakfast (followed by song session): ensure all campers are eating and engaged
9:30AM - Activities Begin: your role is to support the facilitator or facilitate and encourage all campers to participate
12:30PM - Lunch: make sure the campers are eating and drinking, serve food and talk to kids
1:30PM - Rest Hour: this is time for campers to clean the bunk, read mail, relax or write home
2:30PM - Activities
3:45PM - Snack
4:00PM - Free Swim or Floats (Floats is a time when campers choose individual activities)
6:00PM - Dinner (followed by song session): serve food, ensure all kids are eating, talk to kids
7:30PM - Evening Activities: good time to do stories, yoga, campfires, and mellow activities
9:15PM - Bedtime: set the right tone, do a closing ritual, put kids to bed
10:00PM - Counselors are off when bedtime rituals are done. Counselors occasionally have bunk coverage from 10pm-12am
12:00AM - Counselors check in to their cabins and go to sleep with the campers
Additional Responsibilities:
Paperwork - writing camper observation forms and postcards home.
Shmira - bunk supervision between 10pm-12am. Counselors have 5-6 shmira shifts a summer.
Communication with Co-Counselor - working to have a healthy professional relationship with your co.
Communication with supervisor - Bring concerns or questions to your supervisor, there for support and advice.
Unit Counselor - Counselors are assigned to be a bunk counselor or unit counselor. A unit counselor lives in a bunk with campers, but rotates between all four bunks in a unit, keeps track of camper medications, and maintains constant communication w/ bunk counselors.
Ruach Ride- supervising campers on the bus back to the Bay Area, and on the bus up to Camp Tawonga.
Additional Requirements:
Ability to live in a cabin with others, eat meals in a vibrant, communal dining hall, address challenging situations appropriately in a fast-paced, interactive environment required.
Counselors should be comfortable navigating 4-8 miles daily across a variety of terrain while employed at camp, ensuring access to different program areas around camp.
CPR and First Aid certification
