Purpose and Impact:
The Case Manager provides services in our Guardianship program to assist in the coordination of direct services for up to 40 wards of the Agency to include: placement, monitoring of facilitating the provision of medical, social, psychiatric and financial needs of the ward.
Essential Functions:
Visit clients a minimum of one time monthly and monitor the care provided.
Prepare the initial and annual plans for the court.
Maintain client files and case notes.
Assess the clients for needs, coordinate, facilitate and monitor services to meet client needs.
Assess client needs and arrange for appropriate placement of our clients, monitor same.
Review and monitor client needs, attend care plans, provide consents for medical treatment etc.
Assist in marshalling assets and apply for benefits on behalf of clients as applicable.
Document all client activity.
Correspondence relating to the wards affairs and personal relationships.
Advocacy on behalf of all clients.
Conduct case conferences with Program Director, Client Assets staff and other caseworkers as needed.
Interact with the volunteers of the program.
Coordinate with other professionals involved in the continuum of care for all wards.
Assume 24 hour emergency call for after hours, to be shared with the Program Director and other caseworkers.
May be required to assist the Program Director with community outreach, education and marketing duties.

Physical Requirements:
Hours may be irregular as one may be on call 24 hours.
Spend a significant time out in the field monitoring the needs to assigned cases.
Subject to unsanitary homes, physical exertion, and possible physical or verbal abuse from clients.
Need to lift some heavy items at times.
Work is performed with limited supervision. Case Manager is entrusted and required to communicate confidential and/or sensitive information outside the Agency. Work is performed on a highly independent basis and it is necessary to make client decisions and to continue to develop new methods and approaches in dealing with clients.
Required to drive frequently using own vehicle.
Must have Florida Drivers' License, reliable vehicle and current auto insurance.
Education:
BA or BS in Social Services, Accounting, or LPN, RN required.
Experience:
Experience in guardianship preferred.
Background in the social services, medical nursing, legal, insurance or financial field required.
Knowledge of community resources and services is helpful.
Skills:
Self-starter, whom can work independently and is unafraid to make decisions.
Must be able to seek out supervision and help when needed.
Interpersonal, organizational, and problem solving, written and verbal communication, and time management skills are required along with the ability to transfer knowledge to others.
Computer knowledge preferred.

Must demonstrate sensitivity to our service population's cultural and socioeconomic characteristics and needs.

Friary Pensacola provides access to comprehensive mental health care and substance misuse treatment by providing outpatient counseling, intensive outpatient counseling and veterans-specific counseling. Overview:
Primary duty is the performance of work requiring advanced knowledge (Master's Degree, License/Certification, and at least two years of experience) in the field of Human Services, and to use such advanced knowledge to consistently exercise discretion and independent judgment in providing the highest quality client-centered therapeutic services. The Assignment Ready Counselor (ARC) will provide coverage in short-term, surge and on demand situations resulting from the Military needs such as a post-deployment event or to cover an existing assignment for counselors who are absent. The Summer ARC counselor position covers assignments during the May 15th through September 15th time period, coinciding with the school summer break. With minimal guidance and oversight, provides the full breadth of Adult or Child and Youth Behavioral (CYB) counseling services to military service members and their families at military installations. These services may include non- medical counseling, training/health and wellness presentations, consultation with parents, personnel at child daycare centers and schools, and consultation to installation command regarding behavioral health issues affecting military personnel and their family members. ARC CYB counselors may be assigned to DoDEA schools and community schools and will need to have experience working in a school setting to qualify for ARC role in backing up a school position. CYB counselors may also be assigned to youth programs, summer camps, and on demand assignments. ARC adult counselors may be assigned to support MFLC services in a variety of military community and readiness centers, as well as on-demand and surge assignments. The counselors work closely with the installation and military branch Points of Contact (POC) to assure that the program is provided within scope and meets the needs of the installation.
Provides non-medical, short term, solution focused, counseling directly to adults, children, and youth of service members. Services include assessment, brief counseling and consultation, action planning, referral to resources (assuring linkage as appropriate), and follow-up as indicated.
CYB counselors will also have a focus on supporting the staff and personnel of CDCs, DoDEA and community schools, youth programs and summer camps as well as providing parent training and guidance.
Provides training and health and wellness presentations, participate in health fairs and other base/installation activities.
Enters counselor activity data daily through smart phone or web application assure that reporting is accurate from assigned installation while maintaining confidentiality and anonymity of service / family member.

General Program Development and Management:
Act as the liaison with the Florida Department of Health (FDOH) HIV/AIDS Section, the local regional Departments of Health, and the community on HIV/AIDS issues. Participate in statewide, regional, and county HIV/AIDS program planning, development, and evaluation, as needed. Initiates and directs the development of policy material. Responsible for providing technical assistance and monitoring of regional program services. Plans workloads, workflows, deadlines, work objectives and time utilization with employees.
Maintain positive communications with the Administration and Directors of Nursing in the regional counties. Maintain knowledge of HIV/AIDS policies, guidelines, statutes, rules, and protocols. Supervise the Early Intervention Consultant (EIC). Assist the EIC in assuring HIV trainees are meeting Technical Assistance Guideline requirements.
Ensure that the local counseling and testing programs are managed and evaluated correctly (direct responsibility of the EIC). Promotes or assists in promoting the development of community resources to address program needs. Ensure local outreach activities are occurring in the target populations identified in the local prevention plan. Ensure timely, complete, and accurate reporting by coordinating with surveillance staff.
Surveillance:
Ensure the surveillance staff are providing education on reporting requirements and the importance of reporting to providers. Ensure that reporting is done in a timely manner and reports are shared with Area 1 CHD Administrators and other programs within the Health Department.

Position Summary:
Master's level counselor providing intensive outpatient treatment to adults in a virtual and in-person setting. Primary duties include advanced knowledge in the field of human services and to use such knowledge to consistently exercise discretion and independent judgement in providing the highest quality therapeutic services. Role Requirements:
Master's degree in psychology, social work, counseling, rehabilitation, or a related human services field (a related human services field is one in which major course work includes the study of human behavior and development).
At least two (2) years' experience working with individuals who have mental health, substance use and/or co-occurring disorders or developmental disabilities.
OR completion of an internship at Lakeview Center, Inc. through an accredited counseling/social work program can be substituted for the two years of post-graduate experience.
Must be able to provide individualized, integrated mental health, substance use and co-occurring services to meet the needs of individuals served and their families.
Must be able to pass organization screening requirements, including state or federal background screenings as appropriate.
Compensation:
Salary range is between $49,056 to $52,890 per year. Salary based on experience.

This position requires you to be licensed at the independent level in any state as an Mental Health Counselor, Therapist, or Social Worker. This position would work in support of On Demand Events, Surges, Summer Camps, and Backfills supporting the MFLC program.
The Assignment Ready Counselor (ARC) will provide coverage in short-term, surge and on demand situations resulting from the Military needs such as a post-deployment event or to cover an existing assignment for counselors who are absent. The Summer ARC counselor position covers assignments during the May 15th through September 15th time period, coinciding with the school summer break. With minimal guidance and oversight, provides the full breadth of Adult or Child and Youth Behavioral (CYB) counseling services to military service members and their families at military installations. These services may include non- medical counseling, training/health and wellness presentations, consultation with parents, personnel at child daycare centers and schools, and consultation to installation command regarding behavioral health issues affecting military personnel and their family members. ARC CYB counselors may be assigned to DoDEA schools and community schools and will need to have experience working in a school setting to qualify for ARC role in backing up a school position. CYB counselors may also be assigned to youth programs, summer camps, and on demand assignments. ARC adult counselors may be assigned to support MFLC services in a variety of military community and readiness centers, as well as on-demand and surge assignments. The counselors work closely with the installation and military branch Points of Contact (POC) to assure that the program is provided within scope and meets the needs of the installation.
Provides non-medical, short term, solution focused, counseling directly to adults, children, and youth of service members. Services include assessment, brief counseling and consultation, action planning, referral to resources (assuring linkage as appropriate), and follow-up as indicated.
CYB counselors will also have a focus on supporting the staff and personnel of CDCs, DoDEA and community schools, youth programs and summer camps as well as providing parent training and guidance.
Provides training and health and wellness presentations, participate in health fairs and other base/installation activities.
Enters counselor activity data daily through smart phone or web application assure that reporting is accurate from assigned installation while maintaining confidentiality and anonymity of service / family member.

Advanced knowledge of brief therapy and solution-focused counseling methods.
Prior military service/military family member and/or strong familiarity with military culture desired.
For CYB positions must meet the Magellan MFLC CYB criteria for experience with children and youth and specialty in child and adolescent development/psychology.
Creates a presence on the installation in which the service and family members feel comfortable approaching the counselor and recognize the program to be confidential; for CYB counselors -- creates a presence in child and youth settings, is available to children, youth, and staff. When working with children, counselor must abide by line of site protocol.
Establishes and maintains working relationships with community resources and provides appropriate linkages.
Partners with POC to provide Adult and CYB services in a manner that addresses the needs of the installation/facility. Develops an excellent working relationship with the installation/facility POC.
Manages duty to warn and restricted reporting situations according to DoD protocol and staffs the cases with Regional Supervisor/Regional Director.
Communicates with Regional Supervisors and participate in regular individual and group supervision, sharing information regarding trends and issues on the installations and in facilities to which they are assigned for substitute or on call services.
Responds to critical incidents and special requests as directed by the POC and approved by the OSD program manager.
