Key Responsibilities: Provides orientation to new patients to the programs' rules, policies, procedures, and patients' rights. Conduct Intakes, Discharge Planning, Initial Assessments, and Initial Treatment Plans professionally and within time guidelines as required by applicable federal and state regulation, CARF guidelines and Pinnacle Policies. Involve patients in the assessment, treatment planning, and intervention aspects of treatment, which provides the patient with a sense of entitlement to their own recovery. Administer the biopsychosocial assessment, which evaluates all areas of patients' lives, including their addictions, medical and mental health history, socioeconomics, legal, etc. at intake and on an ongoing basis. Comply with and implement the Medical Q.A (Quality Assurance) and Clinical Risk Management Policies, including: Evaluate the patients for any high-risk conditions (e.g., liver failure, pregnancy, overdose, bipolar, schizophrenia, etc.), which may endanger the patient (suicidal) or other parties (homicidal). Administer the BAM-R, C-SSRS, GAD-7, and PHQ-9 screening tools to determine the severity of the patient's mental/emotional status. Evaluate the risk factors concerning each such case, considering the appropriateness of, and professionalism in treating such individuals in an OTP in comparison to treatment in a residential facility or other treatment modalities. Review and ask about other practitioners' involvement in the treatment of these high-risk conditions. Recommend immediate referral to such a specialist in case of need. Request the patient sign a waiver for release of information which will allow communication with the other practitioner in case of need (e.g., deterioration of the patient's condition). Flag high-risk patients to the Medical Directors, Physician Extenders, and Regional Clinical Leadership (RCL) and highlight the conditions concerned. Refer such patients to the Medical Directors, Physician Extenders or RCLs (Regional Clinical Leadership) based on the urgency, utilizing the informal Referral Policy or the scheduled Treatment Team/Case Conference session. Review physician notes, following the Medical Directors or Physician Extenders' session with the patient, verifying that they noted and addressed the high-risk factor, or otherwise bring it immediately to the attention of the supervising counselor, as well as the clinic manager, until the matter's proper and professional handling by the physician or physician extender. Advise, in writing, other staff members (including the clinic manager, supervising counselor, dispensing nurse and front office) of high-risk patients and their condition. Maintain and circulate minutes from treatment team/case conference sessions to the other staff members. Provide professional counseling and referral services by: Addressing patients' clinical and special needs by maintaining referrals to specialized (“second tier”) services rendered by physicians, physician extenders and counselors within Aegis, as well as referrals to Keys to Recovery (or other twelve-step) support groups, and other community resources (e.g., agencies and practitioners outside of Aegis), acting as a liaison to these agencies, as necessary. Providing individual and group counseling as clinically necessary, to address the problem issues identified in the Initial Assessment and Treatment Plans. The Post Adoption Social Worker will provide preventive, supportive, educational services specifically to post-adoptive families. This is a remote position with co-locating in the CDSS Chico and Sacramento offices as needed and to collaborate for CDSS office meetings and needs. The position requires travel to assigned counties to provide support groups, family events, family support/intervention, and/or AAP. A successful BHCC/ECM Case Manager will meet individuals “where they are at” and provide non-judgmental, non-coercive services, and resources to individuals seeking help. In addition, the role will provide consultation, education, training, and support in their work with participants through utilization of harm reduction strategies. Proficient and detailed documentation within the EHR (Electronic Health Records) system in order to meet billing requirements for the ECM program. This is a billable ECM provider position and correct monthly documentation is essential to the position. Use Motivational Interviewing skills when communicating with patients and de-escalation in crisis situations as needed Communicate with patients and the medical team when there is an adjustment in the care plan including a request to change provider. Job improvement training to remain current on MHSA laws, 5150 guidelines. Track patient appointments include outreach, scheduling new appointments and following up on missed appointments in order to assist in the coordination of integrated behavioral health. In house and field case management including check in calls, transportation, and patient advocacy, assistance with medication adherence and as needed home visits to assist with the coordination of care for patients. Assist in outreach and data collection of patient care in order accurately track and record patient progress. Help with specified grants as assigned. Completion of Treatment Authorization Requests TAR's, document submittal into Collective Medical and Partnership. Understanding of all complex care programs within the community and Hill Country. This role supports youth both in placement and in the community by completing comprehensive assessments, developing individualized reentry plans, facilitating skill-building, and coordinating access to critical services such as housing, employment, education, and behavioral health care. The Case Manager works collaboratively with youth, families, Hill Country treatment teams, and justice-involved partners to promote successful transition planning and long-term stability. This position will serve the Redding Area and Eastern Shasta County. Essential Duties & Responsibilities: Provide in-depth, wrap-around case management services to a caseload of youth in placement and in the community. Conduct assessments, develop individualized reentry plans, and provide ongoing case management at intake and at regular intervals. Provide frequent follow-up and support through phone, office visits, and outreach in home, school, and community settings. Coordinate services across multiple systems and link youth to job training programs, housing resources, employment opportunities, social services, SUD treatment, and mental health supports. Assist youth in accessing resources by transporting or accompanying them to off-site programs, job sites, or provider appointments. Reacts well under pressure, Treats others with respect and consideration regardless of their status or position. Facilitate groups as needed. Assist youth with building skills to manage daily stressors, improve impulse control, and reduce negative or harmful behaviors. Collaboration & Communication: Work closely with justice-involved agencies, Hill Country treatment teams, families, and community partners to support each youth's reentry plan. Documentation & Reporting: Assess and document client progress at regular intervals. Maintain accurate case notes, update the agency database, and maintain consistent communication with program manager and Hill Country Team regarding youth progress. Qualifications/Education Requirements: Significant knowledge and experience working with justice-involved youth and an understanding of the social forces, systems, and community resources impacting them Associate or bachelor's degree in human services, social work, psychology, criminal justice, or a related field preferred. Experience in motivational interviewing, strengths-based case management, or trauma-informed care preferred. Ability to work independently and collaboratively in both on-site and remote settings. Strong work ethic, positive attitude, and a sense of humor. Background checks and required clearances must be completed prior to employment. Ability to define problems, gather relevant information, evaluate options, and draw valid conclusions. Computer Skills: Proficiency with Windows operating systems, Microsoft Office Suite (Word, Excel, Outlook) Ability to use electronic health records and telecommunication tools such as Zoom, Teams, or Skype. Equipment Skills: Proficient with standard office equipment including computers, copiers, fax machines, printers, and multi-line telephone systems. Work Environment: The work environment includes indoor office settings, health clinic locations, vehicles, partner agencies, schools, homes of clients, and occasional outdoor or evening activities. Noise levels are generally moderate and typical of office and clinic environments. Physical Demands: Regularly required to sit, talk, hear, write, and use hands to handle or feel objects. Frequently required to stand, walk, and reach with hands and arms. Occasionally required to climb, stoop, kneel, crouch, or crawl.
