Job Summary
Works in coordination with interdisciplinary teams to organize, prepare, and deliver evidence based clinical services across the continuum of care. Maintains ethical principles and professional standards of practice in adherence to the discipline's professional code of ethics.
Job Responsibilities
Conducts a comprehensive bio-psychosocial assessment and establishes a care plan, as appropriate.
Collaborates with the interdisciplinary Team to assess for appropriate level of care and risk factors; develops and implements care/discharge plan and coordinates consults/referrals to remove barriers and promote optimal clinical and financial outcomes.
Utilizes specific therapeutic methods/strategies/interventions in accordance with individualized care plan.
Monitors progress of interventions and revises plan to meet patient/family needs.
Provides education to patient/family with respect to meeting established goals including but not limited to disease prevention, impact of illness and disease progression, advocacy for benefits, health maintenance, and adherence to treatment regimens.
Provides direct patient care that may include: case management, care coordination, interdisciplinary collaboration for care transition needs, crisis intervention/safety planning, counseling, and emotional support to patients and families coping with illness, trauma, or end-of-life care, referral and discharge planning.
Refers patient to appropriate levels of care and available resources that promote the individualized care plan.
Maintains timely patient-centered documentation that reflects the patient and client systems' pertinent information for assessment, interventions and outcomes in accordance with regulatory licensing requirements and third-party agencies.
Independently able to provide clinical social work interventions to all patient populations.
Participates in administrative and development activities including team meetings, clinical supervision of staff and staff education.
Provide social work field supervision to second year social work students.
Supports the development of hospital and/or unit-based programming to meet the clinical and strategic needs of the patients and the milieu.
Works as a team member to contribute to improved patient experience.
Ensures the cost-effective utilization of resources.
Qualifications

Program Summary
In the Adult Partial Hospitalization Treatment and Intensive Outpatient Treatment programs, we focus on creating a therapeutic environment where people can learn strategies for coping and improve their functioning. Each client's treatment plan is centered on group therapy which allows for sharing of experiences while gaining support and validation. When people need intensive, comprehensive treatment for psychiatric and chemical dependency issues but do not need inpatient or residential hospital care, Natchaug offers intensive, structured clinical treatment in programs that may include up to six hours of treatment a day, five days a week. These intensive treatment programs can help prevent the need for hospitalization. They can also help people make the transition to home after inpatient or residential treatment. Natchaug brings this clinical care closer to people's homes by offering programs throughout Eastern Connecticut.

Awake Overnight Counselors support individuals with a developmental disability and/or mental illness live as independently as possible, while providing guidance and performing various levels of assisted daily living skills. Counselors act as strong advocates for individuals served and understand the importance of community integration.
Other Key Responsibilities:
Assist in implementing individualized service plans.
Assist with limited personal care, which includes direct care duties, occasional lifting, medication administration (with training), documentation, transportation, behavioral intervention, role modeling, and recreational activities.
Implement all procedures that ensure the health, safety, and well-being of individuals and general cleanliness and safety of the physical site.
Assist individuals to access and/or provide transportation.
Review and complete all required documentation.

How you'll contribute
The Inpatient Therapist is a licensed mental health professional responsible for providing high-quality clinical services within an inpatient behavioral health setting. This position focuses on delivering individual, group, and family therapy to patients experiencing psychological, emotional, or substance use issues. The Therapist actively participates in treatment planning, crisis intervention, and interdisciplinary collaboration to ensure comprehensive patient care. The role also includes documentation, communication with families and referral sources, coordination of discharge planning and aftercare. The Therapist upholds confidentiality standards and complies with all clinical, legal, and regulatory guidelines, including HIPAA, TJC, and state-specific requirements.
