What You Will Do:
We are looking for a hands-on senior software engineer with deep knowledge of Microsoft Power Apps, including Microsoft Dynamics 365 developer. Key Responsibilities:
Provides orientation to new patients to the programs' rules, policies, procedures, and patients' rights.
Conduct Intakes, Discharge Planning, Initial Assessments, and Initial Treatment Plans professionally and within time guidelines as required by applicable federal and state regulation, CARF guidelines and Pinnacle Policies.
Involve patients in the assessment, treatment planning, and intervention aspects of treatment, which provides the patient with a sense of entitlement to their own recovery.
Administer the biopsychosocial assessment, which evaluates all areas of patients' lives, including their addictions, medical and mental health history, socioeconomics, legal, etc. at intake and on an ongoing basis.

Monitor closely, and document any change of circumstances with the patients, especially as it relates to matters that may affect their recovery, identifying and addressing relapse indicators to promote relapse prevention interventions in an early stage.
Comply with and implement the Medical Q.A (Quality Assurance) and Clinical Risk Management Policies, including:
Evaluate the patients for any high-risk conditions (e.g., liver failure, pregnancy, overdose, bipolar, schizophrenia, etc.), which may endanger the patient (suicidal) or other parties (homicidal).
Administer the BAM-R, C-SSRS, GAD-7, and PHQ-9 screening tools to determine the severity of the patient's mental/emotional status.
Evaluate the risk factors concerning each such case, considering the appropriateness of, and professionalism in treating such individuals in an OTP in comparison to treatment in a residential facility or other treatment modalities.
Review and ask about other practitioners' involvement in the treatment of these high-risk conditions. Recommend immediate referral to such a specialist in case of need. Request the patient sign a waiver for release of information which will allow communication with the other practitioner in case of need (e.g., deterioration of the patient's condition).
Flag high-risk patients to the Medical Directors, Physician Extenders, and Regional Clinical Leadership (RCL) and highlight the conditions concerned.
Refer such patients to the Medical Directors, Physician Extenders or RCLs (Regional Clinical Leadership) based on the urgency, utilizing the informal Referral Policy or the scheduled Treatment Team/Case Conference session.
Review physician notes, following the Medical Directors or Physician Extenders' session with the patient, verifying that they noted and addressed the high-risk factor, or otherwise bring it immediately to the attention of the supervising counselor, as well as the clinic manager, until the matter's proper and professional handling by the physician or physician extender.
Advise, in writing, other staff members (including the clinic manager, supervising counselor, dispensing nurse and front office) of high-risk patients and their condition. Maintain and circulate minutes from treatment team/case conference sessions to the other staff members.
Provide professional counseling and referral services by:
Addressing patients' clinical and special needs by maintaining referrals to specialized (“second tier”) services rendered by physicians, physician extenders and counselors within Aegis, as well as referrals to Keys to Recovery (or other twelve-step) support groups, and other community resources (e.g., agencies and practitioners outside of Aegis), acting as a liaison to these agencies, as necessary.
Providing individual and group counseling as clinically necessary, to address the problem issues identified in the Initial Assessment and Treatment Plans. The counseling service will include protocols for the treatment of Relapse Prevention, Peer Pressure, Anger Management, Domestic Violence, Parenting, Family Preservation, Vocational Rehabilitation, etc.
Utilizing advanced counseling techniques such as Cognitive Behavioral Therapy, Motivational Interviewing, etc.
Scheduling regular counseling sessions with patients; issue patient appointment cards and record scheduled or rescheduled sessions in EHR scheduler.
Documenting counseling entries, including signature and date, as required by applicable federal and state regulations, CARF guidelines and Pinnacle policies.
Engaging in Community Relations and Outreach efforts.

Collecting research data when requested.
Observing the collection of patients' urine samples when required

DUTIES AND RESPONSIBILITIES:
CAREER READINESS LEADERSHIP:
Lead the Don B. Huntley College of Agriculture career team, which will include all the functional areas necessary to support career and professional readiness including FYE career coaches, college career coaches, employer engagement, internship coordinator, jobdevelopers, event coordinators, etc. This includes coordinating career team activities and workload.
Recommend for hire, develop and train the College of Agriculture career team members who will help support and deliver a comprehensive and customized career-readiness model within the College of Agriculture.
Set strategy for industry aligned career development services that are responsive to student needs and aligned with college, departmental and institutional goals.
Ensure that day-to-day operations of College of Agriculture career team are appropriately directed, efficient and effective.
Work collectively and collaboratively with the other College Lead Career Coaches to develop innovative approaches and implement new and best practices, share resources, and produce institution-level events such as campus-wide career fairs.
Design and deploy appropriate assessment activities to measure outcomes and impact.
CAREER READINESS PROGRAM DEVELOPMENT:
Lead programs and services for the Don B. Huntley College of Agriculture that provide personalized, career counseling and advising that focuses on self-assessment, research, job and internship search strategies, reflection, and strategic decision making.
Develop a comprehensive set of events such as lectures, speaker series, workshops, conferences, trainings, cultural events etc. in collaboration with internal & external resources.
Build students' career confidence and social capital through College of Agriculture strategic programming and creating career content/materials.
Conduct classroom and student organization presentations for the College of Agriculture on various career-related topics, including promoting programs and skill development to support career readiness and planning.
Oversee event logistics and assist in creating marketing materials/communication for the Don B. Huntley College of Agriculture to effectively promote to faculty, staff and students.
CAREER STAKEHOLDERS:
Closely collaborate with and support the deans, faculty, and staff in the Don B. Huntley College of Agriculture in relation to career and professional readiness of their students. Develop and implement trainings incorporating and integrating career education into curricula and co-curricular activities.
Present data and metrics to College of Agriculture faculty and staff to create strategies for increasing the career engagement of students.
Partner with internal/external departments, faculty, alumni and employers to identify career pathways for College of Agriculture students and create an outreach plan to engage organizations that will result in opportunities spanning a broad range of industries.
Establish and cultivate effective working relationships with key stakeholders on-campus and off-campus. Attend events and professional meetings to stay current on career and employment trends and ensures the Career Center maintains a presence on-campus as the experts in career development and programming. Collaborate with campus departments to coordinate career programs. Serve on University committees and provide insights and information to the community on career services.
Identify opportunities and strategies for connecting students with alumni and industry experts who will provide academic and/or career resources such as mentoring, coaching, lectures, workshops, etc.
Establish and enhance employer relationships, increase the quantity, quality, and diversity of job and internship opportunities, and engage employers and alumni to provide programs to help students compete effectively for opportunities of interest.
Support the development of partnerships between the Don B. Huntley College of Agriculture and external communities (corporations, public sector, and nonprofit entities) to build project-based experiential learning, internship, and full-time employment opportunities for College of Agriculture students.
Perform outreach to the College of Agriculture alumni, Cal Poly Pomona alumni, and a wide range of employers, industry organizations, and networks for the purpose of fostering peer-to-peer networking, developing and strengthening specific employer relationships, and identifying employment networks to benefit current students and alumni.
CAREER READINESS PROGRAM ASSESSMENT:
Lead the evaluation and assessment efforts of the Don B. Huntley College of Agriculture career team, generating surveys and reports to facilitate data collection and progress reports.
In collaboration with Career Center Director develops & implement evaluation processes, including quantitative & qualitative measurements that ensure that program benchmarks are successfully attained and communicated in a timely and efficient manner.
Collaborate with internal/external resources to develop meaningful outcome measurements and methods of program evaluation that will lead to enhancement.
Ensure accurate student reporting in CPP Connect and other areas that require career related data tracking.
QUALIFICATIONS:
Bachelor's Degree - Equivalent to graduation from a four-year college or university in a related field plus upper division or graduate course work in counseling techniques, interviewing and conflict resolution where such are job related.
Driver License, Valid and in State
Possession of these knowledge and abilities is typically demonstrated through the equivalent to four years of progressively responsible professional student services work experience which includes experience in advising students individually and in groups, and in analysis and resolution of complex student services problems. A master's degree in Counseling, Clinical Psychology, Social Work or a job‑related field may be substituted for one year of professional experience. A doctorate degree and the appropriate internship or clinical training in counseling, guidance or a job‑related field may be substituted for two years of the required professional experience for positions with a major responsibility for professional, personal or career counseling.
The following knowledge and abilities as well as those listed at lower levels in the Student Services Professional series are required for appointment to this classification. Thorough knowledge of the policies, procedures and practices of the program area to which assigned or the ability to quickly acquire such knowledge. CSU Classification Salary Range
This position is part of the Academic Professionals of California (APC) bargaining unit 4. The CSU Classification Standards for this position are located on the CSU Classification Standards website. The CSU Salary Schedule is located on the CSU Salary Schedule website. The classification salary range for this position according to the respective skill level is: minimum $6,320 and maximum $9,014. per month. About the Position
With the goal of improving child care quality, the Professional Development Coach is responsible for specialized technical assistance and support to child care providers, including on-site quality improvement coaching and promotion of professional development access and attainment. Essential Duties and Responsibilities:
Maintain accurate progress notes and documentation of client services

Responsible for providing direct services to clients:
Assist with client crises as needed (life skills, conflicts resolution, safety planning, etc.) using trauma-informed de-escalation and mediation techniques.
