PURPOSE: As a member of the Emergency Response and Assessment Team (ERAT), the Prevention Counselor provides support to families and children in maintaining permanency by providing in-home services and assistance to ensure the child's safety and welfare, health and well-being, while attempting to strengthen and preserve the child's family. KEY RESPONSIBILITIES: Respond to all assigned referrals in a timely manner. Conduct all visits, with the exception of the first visit, at the convenience of the family. Provide appropriate level of care as directed by supervisor (frequency of visits, phone call, etc.); Complete the Family Situational Assessment that addresses the following but not limited to: assesses child, adult and family functioning, identifies strengths and protective factors. Assesses risks and safety. Identifies environmental factors that impact the family. Provide individual and family support services when needed through: Empowering, engaging and building relationships with families. Communicating respectfully and effectively with families from a wide range of backgrounds assesses child, adult and family functioning. Support and mentor families as they manage their homes, parent their children, and use community resources. Intervening in stressful and crisis situations. Complete and implement the Prevention Plan. Key ResponsibilitiesClient Engagement & Support Meet with clients during Professional Development sessions to assess skills and prepare forecast reports for job matching in the ETO database.
