Responsibilities:
Service Plans:
Monitor resident with self-administration of medications and complete documentation of such monitoring as outlined in the state regulations. Order, receive, store and document such activities in a timely manner.
Provide watchful oversight for residents; conduct walking rounds of the community regularly throughout the shift.
Measure and record temperature, pulse, blood pressure and weight for resident as instructed.
Assist daily with identification of at-risk residents with regard to behaviors, falls, or elopement; timely notification to supervisor.
Essential Job Functions:
Assist resident with eating (e.g. by plating meals, cutting meat into smaller pieces, positioning food within their range of vision, assisting with condiments, escorting to the dining room); maintain a simple, consistent mealtime routine.
Assist resident with hair care, shaving, and mouth care to promote their self image and good appearance.
Assist resident with dressing by encouraging their selection of clothing, ensuring cleanliness of clothing, providing hands-on assistance with those aspects of dressing which the resident cannot safely manage for his/herself.
Assist resident with bathing (whether in a tub, shower, or bed) by providing a safe environment, ensuring their privacy, and protecting their modesty at all times.
Assist resident with ambulation by encouraging consistent use of walking aides (cane, walker, corrective shoes), escort resident to and from their destination, and educate resident on fall prevention measures.
Assist resident with toileting by establishing a consistent toileting schedule, provide verbal reminders to toilet, escorting to the bathroom if necessary, encouraging a diet with plenty of fluids and fiber, and providing assistance with use of incontinence undergarments.
Document in the residents' health records regarding delivered services, changes in health condition, occurrence of incidents, and other pertinent health related matters for each resident.
Participate in community activities or special events; encourage residents' involvement in activities and events.
Assist with mealtime service in the dining room to include serving meals, clearing tables, resetting tables, and any other related duties as required.
