Job Summary:
Under the direction and supervision of the Center Manager, the Personal Care Attendant (PCA) provides home care services to Jordan Valley Senior Care participants. The Personal Care Attendant (PCA) will also participate in other activities and tasks as assigned to assist the home care, therapy, and adult day health staff.
Key Responsibilities:
Provides personal care and escort assistance in accordance with individual treatment plans including peri-care, grooming, dressing, showering, bathing, toileting, feeding, ambulation.
Reports observed changes in participants conditions to Center Manager and/or other center-based clinical staff.
Proactively communicates participant health and safety concerns to Center Manager and/or other program staff.
Demonstrates safe practices utilizing proper body mechanics and complying with universal precautions.
Interacts in friendly, courteous, and professional manner with participants and their families/caregivers.
Participates in social and recreation activities with participants as appropriate.
Implements interventions, as appropriate to training, as directed by rehabilitation staff, including use of mechanical assistance for participant transfers and mobility.
Occasionally accompanies participants to outside clinical appointments, when necessary.
Assist with food service delivery and monitoring during mealtimes.

Job Summary:

Responsible for completing daily supply chain activities.

Facilitate returns or transfers of overstocked, short-dated, and outdated items.
Recording and reporting inventory inaccuracies as required by the MO Board of Pharmacy.

Coordinate 24-hour care delivery.
Document participant changes appropriately in the medical records and communicate participant changes to IDT in a timely manner.
Supervise home care staff based in the community.
Manage human resources by determining qualifications, competence, and performance expectations of home care aides; implementing and monitoring staffing plans; providing orientation, in-service training and continuing education of assigned staff and applying policies/procedures pertaining to human resource management.
Keep up to-date on home care standards and best practices.

Job Summary:
The Recovery Care Navigator provides community-based and office-based care management services to patients with substance use disorder (SUD). The Recovery Care Navigator may be the first point of contact for patients. The Recovery Care Navigator builds strong relationships with patients to help them stay engaged in medical care, adhere to their medications, assist with transportation needs, and improve overall patient outcomes. Care Navigators are committed to removing the patient's barriers to care by identifying critical resources for patients, helping them navigate through health care services and systems, and promoting patient health. They will work closely with the Care Team, which may include doctors, nurses and other clinical staff to support positive patient health outcomes. This position works with the patients to provide warm handoffs and work through social determinants of health barriers that they may have. They will be working collaboratively as a team to improve patient outcomes along the entire continuum of care.
Key Responsibilities:
Maintain a professional relationship with patients, their families, community, treatment team, and other professionals.
Encourage patients to medical appointments to develop that relationship and foster warm handoffs to and from community partners.

Job Summary:
The Phlebotomist will obtain and process lab samples from patients per providers requests while following protocols required by CLIA, CLSI, AAAHC, OSHA, and other JVCHC, state, or Federal regulations.
Key Responsibilities:
Performs phlebotomy skillfully utilizing proper laboratory procedures as outlined by CLSI with minimal discomfort to the patient.
Obtains and collects high quality specimens that are appropriate for the ordered laboratory tests.
Disinfects laboratory surfaces with daily cleaning.
Verifies and records identity of patient using 2 patient identifiers.
Effectively interacts with patient to minimize the apprehension of the phlebotomy procedure.
Correctly labels, processes and properly stores blood and body fluid specimens according to the specimen requirements.
Conducts all waived in-house lab tests efficiently as ordered by prodders, and correctly documents.
Maintains a strict level of patient confidentiality.
Assists with orientation of laboratory procedures for students and/or new laboratory employees.
All other duties as assigned.

Job Overview:
Under the direct supervision of the Center Manager, the Jordan Valley Senior Care Home Care Coordinator is responsible for the development and implementation of homecare services for program participants, including the coordination of all Durable Medical Equipment (DME) and community-based care.
Key Responsibilities:
Assesses the home and personal care needs of the frail and elderly population and identifies and works with the Interdisciplinary Team (IDT) to develop specific plans of care. Initial and periodic assessments to be conducted minimally every 6 months and must be completed prior to the scheduled team meeting. Receives reports from home care staff and communicates participant changes to IDT members in a timely manner.
Schedules and coordinates home care staff -both contract and/or employed, for in-homecare delivery and the implementation of all personal care services to ensure that quality services are provided to meet participant needs.
Coordinates all DME provisions in home setting; works with IDT with DME authorization requests.
Coordinate 24-hour care delivery.
Document participant changes appropriately in the medical records and communicate participant changes to IDT in a timely manner.
Supervise home care staff based in the community.
Manage human resources by determining qualifications, competence, and performance expectations of home care aides; implementing and monitoring staffing plans; providing orientation, in-service training and continuing education of assigned staff and applying policies/procedures pertaining to human resource management.
Keep up to-date on home care standards and best practices.
Ability to drive to community locations and residences to provide on-site training and competency checks of home care staff.
Provide effective leadership by coordinating and integrating services within the department, with other departments and with the IDTs primary functions.
