JourneyCare Home Health is seeking an experienced and motivated Home Health/Hospice Liaison to build and strengthen referral relationships in the assigned market. This role focuses on promoting our home health and hospice services, fostering community referral partnerships, and helping patients connect with the care they need.
Location: In the assigned market (60601).
Salary: $85,000 - $100,000 annual salary (plus performance-based commissions)

What You'll Do
Promote Skilled Services provided under Home Health Benefits, such as, Nursing, Physical Therapy, Occupational Therapy, Speech Therapy, Home Health Aide and Medical Social Worker. (Covered benefits under Medicare and Medicaid insurances)
Drive home health and hospice business growth by developing strong referral relationships across diverse settings, including hospitals and outpatient providers.
Demonstrate relationship development and public relation skills: research, cultivate, implement, produce and drive comprehensive, effective and coordinated community awareness with Acute Care discharge planners, Skilled Nursing Facilities, Senior living facilities, physicians and business professionals both in and outside of the healthcare community.
Collaborate with the Executive Directors and Directors of Clinical Services in their local markets to identify business development opportunities and coordinate education and partnership meetings
Develop and execute an effective and targeted plan that incorporates the entire local leadership team and generates increased census and promotes our positive image in the community as the leading homecare provider
Priority of the role is to work and develop referral relationship in the field, as well as working from the local branches. Weekend marketing rotations required.
Provide weekly reporting to the Health Partnership Director and Executive Directors with respect to activity and results for the week
Qualifications:
Bachelor's degree in Marketing, Business Administration, Communications or a related field.
At least two years of home care related sales or community education experience, in-home care experience is preferred.
Experience in managing business prospecting software and identifying opportunity for account target selection and volume growth.
Ability to manage coordination of multiple lines of business.
Demonstrated ability to drive client census growth, develop business and professional relationships.
Excellent communication, interpersonal and salesmanship skills.
Valid driver's license and proof of insurance is required.

Job Description
The palliative care physician has responsibility for the medical care and treatment plan provided for seriously ill patients on the palliative care service. The physician shall consult and cooperate with the patient?s attending physician(s). The physician provides direction and guidance to palliative team members to ensure that quality care is provided, and to ensure knowledge of and adherence to palliative care standards, policies and procedures. The palliative care physician will provide education to both interdisciplinary palliative care team members as well as to practicing physicians and others engaged in health care services regarding palliative care and its potential benefits for patients with advanced, serious illness.
Provide consultation in palliative care, symptom management and supportive care to meet the needs of the patient as requested by the referring physician.
Facilitate clarification of patient and family goals of care.
Facilitate access to appropriate support models of care (i.e., hospice, rehabilitation, on-going aggressive treatment).
Assist with diagnosis, treatment and management of acute and chronic health conditions.
Educate facility staff, co-workers and the community on all aspects of palliative care and the services provided by VITAS Palliative Care.
Attend, participate, and lead (when applicable) internal VITAS Palliative Care team meetings.
Consult with the referring physician, the acute care team and other involved providers to contribute to the patient?s plan of care.
Prepare and maintain accurate patient records, charts and documents to support the medical practice and reimbursement for services provided.
Prescribe medications and treatments according to patient?s palliative care needs and in collaboration with the referring physician.
Order diagnostics / treatments as needed after collaboration with the referring physician.
Interpret, evaluate and communicate diagnostic findings to the appropriate parties.
Collaborate with and supervise palliative care team members.
Assist in development of clinical practice guidelines / standards in support of quality palliative care as requested by supervisor.
Utilize a customer service approach in all interactions with VITAS customers.
Adhere to the practice of confidentiality regarding patients, families, staff and the organization.
Participate in on-call rotation as needed.

Experience:
Minimum two years of experience.
Knowledge of the interdisciplinary team concept.

Relief Team Manager - PTO/FMLA Coverage
The Team Manager is the leader of the Patient Care Team and that member of the team whose function is to: supervise, evaluate and coordinate the various component members of the interdisciplinary team; assure continuity of care from admission to discharge or transfer to bereavement; serve as patient advocate and coordinator for other social service and health care providers in the community who are involved in the care of the team's patients; assume responsibility for the maintenance of patient records from intake through discharge or transfer to bereavement.
This is a Relief Team Manager position responsible for covering an assigned team during periods of PTO and/or FMLA leave. There are no direct reports associated with this role. Mileage is reimbursed at $0.70 per mile.
Compensation:
Annual Base Salary Range: $90,000-$110,000
Includes base salary plus incentive opportunities, full benefits package, and travel reimbursement.
Patient Care Services
Directs all patient care services for the team, including volunteer and bereavement, and ensures that services are rendered as integrated components of the interdisciplinary plan of care.
Reviews information on all pending patient admissions, all new admissions (assessments and history), and “alive” discharges to assure that eligibility and appropriateness criteria are met.
Collaborates with both the team physician and the attending physician regarding palliative care measures for pain and symptom management.
Assures complete and accurate data and documentation are available to make recertification decisions and further assures that those decisions are made on a timely basis.
Reviews all imminent “alive” discharges to assure effective discharge planning.
Monitors the type and level of team services provided (staffing, medications, HME, supplies, level of care changes to inpatient and continuous care, etc.) to assure that they are appropriate and responsive to patient/family needs and expectations.
Manages the team schedule and assures coordination of services 24 hours a day, 7 days a week to all patients on the team.
Facilitates team meeting, assuring interdisciplinary input into the update and revision of the plan of care in concert with the changing needs and expectations of the patient and family.
Participates in on-call rotation.
Staff Supervision and Management
Interviews, selects, trains, supervises, evaluates and dismisses team staff in conjunction with the Patient Care Administrator.
Assures staff competence and performance levels through making field visits, evaluating documentation and providing team-based in-services on VITAS values, hospice principles and practices and specific team growth needs.
Acts as a resource and mentor for staff re: clinical issues, documentation, inter- and intra-team problem solving and appropriate customer service behavior.
Oversees staff and volunteer schedules, scheduling and territory assignments to assure that workload is distributed equitably and that all staff are able to meet overall productivity expectations.

Monitors clinical records and all team activities to ensure that they are accurate, complete and meet required standards in accordance with VITAS policy and all regulatory bodies.

Ensures that the team completes proactive and other patient/family satisfaction surveys and implements Quality Improvement measures and standards.

Each day is different. You will spend most of your time caring for hospice patients and families wherever they call home, whether that means a private residence or an assisted living/skilled nursing facility.
Once a week, you'll visit your local program office to discuss cases with members of your interdisciplinary team, including social workers, chaplains, and nurses. All of these roles collaborate to provide holistic support for the patients and families we serve.
WHAT'S EXPECTED FROM YOU
VITAS physicians must be licensed to practice medicine or osteopathy in the state in which their program operates. In addition to being board-certified or board-eligible in hospice and palliative medicine, internal medicine, family practice, or a related specialty, you should be familiar with the principles of hospice and registered with the DEA.
