Under the supervision of the Nurse Manager and Program Director, the Certified Nurse Assistant has the responsibility to engage with patients and to assist in delivering personalized attention and support to patients living at the Cecil Safe Landing Program. The CNA will help patients with navigating around the site or community, eating, dressing, and bathing as needed. This role works in close collaboration with the patient care team, further enriching the well-being of our patients and contributing to the positive ambiance and exceptional level of care maintained throughout our facility.
Program Population:
The Martin Luther King-Recuperative Care Center (MLK-RCC) is an Interim Housing Program that helps homeless individuals in their recovery journey, helping them improve their health conditions and enhance their social and Activities of Daily Living (ADL) skills, all with the focus of securing stable housing opportunities. Some of these individuals may also experience mental health and substance abuse issues, which can occasionally result in behaviors such as yelling, using strong language, and displaying anger outbursts. The core responsibility of each staff member at the MLK-RCC is to support clients in overcoming obstacles and improving their coping mechanisms.
Principal Responsibilities:
Performs job functions as prescribed by established policies and procedures within defined scope of education, training and responsibilities.
Observes and documents patient response to care.
Reports and records observations of patient conditions.
Monitor client ability to care for bed pans, urinals, commodes, and empty urine collection bags.
Assists with patient navigation around the facility for meals, rehabilitative and recreational activities, laundry, and to vans for community appointments.
Assists with meals as needed.
Assists patients with hygiene and grooming activities.
Assist partners within the client care team to support client health and safety.
Assists in promoting patient safety and environmental cleanliness.
Assist medical team with obtaining vital signs and weights as needed.
Documents pertinent patient information, following established policy and procedures.
Documents accurately regarding observations of patient response to care or physical conditions.

Follows all DHS requirements for logging/documenting/reporting patient status.
Maintains confidentiality at all times.
Responsibility and accountability for adherence to organizational and department standards and policy.
Observes department guidelines concerning absences and reports absences to LVN Supervisor in accordance with departmental procedures,
Self-identifies potentially unsafe systems/processes/situations and takes initiative to report to the supervisor,
Participates in assigned staff and other meetings, for operations and training
Adhere to all organizational safety policies, procedures, and regulatory requirements to ensure a safe environment for patients, staff, and visitors.
Participate in required safety and compliance trainings and apply learned practices in daily work activities.
Identify, report, and help mitigate unsafe conditions, incidents, or hazards in the workplace.
Use equipment, tools, and supplies safely and responsibly in accordance with organizational and regulatory standards.
Maintain awareness of infection control, occupational health, and emergency preparedness procedures appropriate to the role.

Requirements:
Minimum High School diploma or equivalence
Licensed as Certified Nurse Assistant in accordance with the laws of the State of California
Minimum one-year experience in a health care setting.
Current BLS Certificate from the American Heart Association

Some of these individuals may also experience mental health and substance abuse issues, which can occasionally result in behaviors such as yelling, using strong language, and displaying anger outbursts. The core responsibility of each staff member at the LAG/USC-RCC is to support clients in overcoming obstacles and improving their coping mechanisms.
Principal Responsibilities:

The Medical Assistant facilitates health care service to women and children residing in Residential Treatment Program for Women and Children in South Los Angeles. The Medical Assistant upholds the JWCH mission to improve the health status and wellbeing of under-served segments of the population of Los Angeles County. This position is funded through contracts with Los Angeles County Substance Abuse Prevention and Control (SAPC) Sexual and Reproductive Health (SRH).
Duties and Responsibilities:
Performs routine care functions as prescribed by established policies and procedures within defined scope of education, training and responsibilities
Responsible for obtaining Physical Examination and TB compliance for each patient through care coordination
Supervises self-administration of medication. Logs medication, maintains Medical Administration Record (MAR), and audits MAR daily in collaboration with licensed nurse.
Documents in Nextgen and PCNX, electronic health records (EHR) system.
Organizes and maintains medication cabinet and patient medication boxes; to include removing medications left by patients and packaging them for proper disposal.
Meets regularly with patients to review medications, appointments and refills needed and refers to the nurse.
Assists with Medi-Cal eligibility and coordinates with Enrollment Specialist for out of county transfers
Assesses if patient has a medical provider and make change to Wesley Health Centers as appropriate. Assists in completing enrollment paperwork
Schedules appointments in Nextgen for psychiatric, family, prenatal and postpartum health care and other medical services
Works with patients on self-management goal under the direction of a licensed health care provider
