The Licensed ClinicalSocialWorker provides behavioral health services at a Recuperative Care Center in a manner that upholds the JWCH mission to improve the health status and well-being of under-served segments of the population of the Los Angeles area through the direct provision of health care, health education, residential services, and research. This includes adherence to all behavioral health contract requirements (staffing, scope of work, and documentation), utilization of funding, staff productivity, and patient satisfaction.
Population/Needs:
Experience working with chronic disease treatment compliance issues, knowledgeable of chronic diseases and dealing with high rates of depression, anxiety, and other major mental disorders, with a predominantly homeless population. ClinicalSocialWorker must be familiar with the geographic area to better collaborate with other agencies.
Principal Responsibilities:
1.Work at a Recuperative Care Center as a part of a multidisciplinary team to ensure that the patient's clinical treatment goals and behavioral management needs are met by attending case conferences, chronic disease management meetings, staff consultation, and individual meetings with staff to ensure quality of care.
2. Conduct functional and strength-based bio psychosocial assessments including comprehensive diagnosis for mental health, substance abuse, trauma, and psychosocial needs.
3. Develop patient care plans that are coordinated and integrated with a comprehensive range of medical and psychosocial services to address social, emotional, and practical needs.
4. Conduct groups for clients in a therapeutic setting.
5. Provide psychotherapy, counseling, medical case management and psychoeducation utilizing individual, family, and group modalities. Employ Evidenced Based brief oriented therapies for patients and their significant others that include social, emotional, and practical life skills to facilitate safe coping, distress tolerance, stress reduction, mindfulness, and relaxation as well as access to necessary resources with the goal of improved functioning and positive health outcomes.
6. Utilize both short-term and intensive longer-term interventions, as approved by funding source that target trauma, mental health symptoms, increase psychosocial functioning and reduction of substance use. Interventions are client centered; strength based and identified based on a comprehensive treatment plan that is developed, after completing a thorough biopsychosocial assessment.
7. Consult with primary care providers to enhance understanding of the patient, provide support for treatment approaches that incorporate behavioral health needs, and assist with implementation of bio-psychosocial treatment and care plans.
8. Participate in Multi-Disciplinary Team meetings that are held weekly.
9. Provide consultation to and coordinate care for patients needing specialty mental health and/or substance use disorder services
10. Advocate for patient within organization and community to ensure access to appropriate care
11. Be available to primary medical care staff during working hours for crisis intervention in the event of a psychiatric emergency and assist staff in the event of any other kind of emergency.
12. As Officer of The Day be available for urgent consultation and clinical sessions with patients who have urgent needs and provide consultation to providers who may have urgent needs and consultation and to clients who present with immediate psychosocial needs.
13. Be available as assigned to provide after-hours emergency psychiatric assistance.
14. Provide phone consultation as needed
15. Complete all charting as required by funding sources, including assessments, progress notes, care plans, and billing documentation
16. Productivity goal is to average 8-10 patient per day.
17. Participate in structured and “as-needed” case conferences
18. Assist with provider/staff education for integrated behavioral and primary care services
19. Participate in behavioral health quality assurance/improvement activities to maintain and strengthen standards of care, in accordance with the organization CQI Plan
20. Participate in professional development activities
21. Support client involvement in planning, implementing, and evaluating services
22. Complete data collection forms and prepare reports as required by the program.
23. Other duties as assigned

Requirements:
1. Master's degree in social work, social welfare, psychology, marriage, family and child counseling, and or marriage and family therapy from an accredited college or university.
2. LCSW with current California BBS license
3. Experience /knowledge in human, social, and/or behavioral health services, especially for homeless and/or low-income underserved individuals
4. Experience working on an Interdisciplinary Team
5. Demonstrated understanding of the impact of trauma on behavioral and medical conditions
6. Knowledgeable of evidence-based treatments (DBT, CBT, Seeking Safety, Motivational Interviewing)
7. Knowledge of principles, procedures, methods and techniques of mental health emergency procedures such as: 5150, CPS reporting, APS reporting, and Tarasoff,
8. Experience working with substance abuse disorders, preferred
9. Experience working in a medical setting preferred
10. Bilingual (English/Spanish) ability needed.

Position Purpose :
The Licensed ClinicalSocialWorker (LCSW) plays a vital role in supporting the emotional and behavioral health of residents at Wesley Health Centers - Skid row Residential Care Facility, a licensed Residential Care Facility for the Elderly (RCFE) serving individuals in the unique location of Downtown Los Angeles Skid-row area. This full-time, in-house position focuses on mental health care, substance use recovery support, crisis intervention, and care coordination of individuals who are experienced homelessness. The LCSW collaborates closely with administrators, Certified Nurse Assistants, outside providers, and community agencies to ensure residents receive comprehensive, trauma-informed support that honors their dignity and enhances quality of life. The role of LCSW in this facility will assist us in a transformative process as individuals' transition from living in interim housing placements or the streets to a supportive living facility.
Program Population:
The Crocker Residential Care is a licensed Residential Care Facility for the Elderly (RCFE) serving older adults, including individuals with histories of homelessness, substance use disorders, behavioral health challenges, and/or chronic medical conditions. Residents may require support accessing community resources, engaging in recovery, or managing psychiatric symptoms in a supportive housing environment.
Principal Responsibilities:
Conduct comprehensive psychosocial and behavioral health assessments upon admission and as clinically indicated.
Provide crisis intervention and crisis stabilization which may include initiated a 5150 hold and working with County Mental Health regarding timely disposition.
Develop and implement individualized care plans addressing mental health, substance use, trauma history, and recovery goals.
Provide direct clinical services including supportive counseling, crisis intervention, harm-reduction support, and relapse prevention strategies.
Monitor resident behavioral health status and intervene in decompensation or high-risk situations.
Collaborate with program staff to ensure trauma-informed, person-centered practices are integrated into daily care.
Coordinate with County Mental Health, local outpatient services, and Medi-Cal Managed Care Plans for behavioral health referrals and continuity of care.
Assist residents in understanding and accessing benefits through Medi-Cal and CalAIM programs (e.g., Enhanced Care Management).
Maintain confidential, timely, and accurate documentation in compliance with RCFE and County standards.
Participate in resident care team meetings, quality assurance reviews, and staff training on behavioral health best practices.
Requirements:
License: Must hold a current, active Licensed ClinicalSocialWorker (LCSW) license issued by the California Board of Behavioral Sciences.
Must be Lanterman-Petris-Short (LPS) designated to initiate 5150 holds.
Education: Master's degree in Social Work (MSW) from an accredited institution.
Clinical Knowledge: Demonstrated experience or training in trauma-informed care, co-occurring disorders (mental health + substance use), harm reduction, and housing-first models.
Regulatory Knowledge: Familiarity with Medi-Cal, CalAIM Enhanced Care Management, and applicable RCFE and HIPAA regulations.
Skills: Excellent interpersonal, crisis de-escalation, and written communication skills. Ability to work with residents from diverse backgrounds in a respectful, empowering manner.
Additional Requirements:
Must clear a DOJ/FBI criminal background check, physical exam, and TB screening prior to hire.
Must maintain annual training compliance per CCLD standards.
CPR and First Aid certification required or obtained within 30 days of hire.

The Behavioral Health Clinician III (BHC III) provides screening, consultation, behavioral intervention, referrals and counseling and therapy to patients within an integrated medical and behavioral health setting and also provides clinical supervision to ACSW and AMFT providing clinical services within their units. Makes clinical recommendations and refers to appropriate levels of care in a team-based setting. Must be a Licensed ClinicalSocialWorker (LCSW), or Licensed Marriage and Family Therapist (LMFT) in the State of California. This position requires a strong team player and self-starter. The BHC III will provide mental health and substance abuse assessment and treatment of patients across multiple sites. The Licensed ClinicalSocialWorker provides behavioral health services at a Recuperative Care Center in a manner that upholds the JWCH mission to improve the health status and well-being of under-served segments of the population of the Los Angeles area through the direct provision of health care, health education, residential services, and research. This includes adherence to all behavioral health contract requirements (staffing, scope of work, and documentation), utilization of funding, staff productivity, and patient satisfaction.
Population/Needs:
Experience working with chronic disease treatment compliance issues, knowledgeable of chronic diseases and dealing with high rates of depression, anxiety, and other major mental disorders, with a predominantly homeless population. ClinicalSocialWorker must be familiar with the geographic area to better collaborate with other agencies.
Principal Responsibilities:
1.Work at a Recuperative Care Center as a part of a multidisciplinary team to ensure that the patient's clinical treatment goals and behavioral management needs are met by attending case conferences, chronic disease management meetings, staff consultation, and individual meetings with staff to ensure quality of care.
2. Conduct functional and strength-based bio psychosocial assessments including comprehensive diagnosis for mental health, substance abuse, trauma, and psychosocial needs.
3. Develop patient care plans that are coordinated and integrated with a comprehensive range of medical and psychosocial services to address social, emotional, and practical needs.
4. Conduct groups for clients in a therapeutic setting.
5. Provide psychotherapy, counseling, medical case management and psychoeducation utilizing individual, family, and group modalities. Employ Evidenced Based brief oriented therapies for patients and their significant others that include social, emotional, and practical life skills to facilitate safe coping, distress tolerance, stress reduction, mindfulness, and relaxation as well as access to necessary resources with the goal of improved functioning and positive health outcomes.
6. Utilize both short-term and intensive longer-term interventions, as approved by funding source that target trauma, mental health symptoms, increase psychosocial functioning and reduction of substance use. Interventions are client centered; strength based and identified based on a comprehensive treatment plan that is developed, after completing a thorough biopsychosocial assessment.
7. Consult with primary care providers to enhance understanding of the patient, provide support for treatment approaches that incorporate behavioral health needs, and assist with implementation of bio-psychosocial treatment and care plans.
8. Participate in Multi-Disciplinary Team meetings that are held weekly.
9. Provide consultation to and coordinate care for patients needing specialty mental health and/or substance use disorder services
10. Advocate for patient within organization and community to ensure access to appropriate care
11. Be available to primary medical care staff during working hours for crisis intervention in the event of a psychiatric emergency and assist staff in the event of any other kind of emergency.
12. As Officer of The Day be available for urgent consultation and clinical sessions with patients who have urgent needs and provide consultation to providers who may have urgent needs and consultation and to clients who present with immediate psychosocial needs.
13. Be available as assigned to provide after-hours emergency psychiatric assistance.
14. Provide phone consultation as needed
15. Complete all charting as required by funding sources, including assessments, progress notes, care plans, and billing documentation
16. Productivity goal is to average 8-10 patient per day.
17. Participate in structured and “as-needed” case conferences
18. Assist with provider/staff education for integrated behavioral and primary care services
19. Participate in behavioral health quality assurance/improvement activities to maintain and strengthen standards of care, in accordance with the organization CQI Plan
20. Participate in professional development activities
21. Support client involvement in planning, implementing, and evaluating services
22. Complete data collection forms and prepare reports as required by the program.

Requirements:
1. Master's degree in social work, social welfare, psychology, marriage, family and child counseling, and or marriage and family therapy from an accredited college or university.
2. LCSW with current California BBS license

Principal Responsibilities:
Work at a Federally Qualified Health Center multidisciplinary team to ensure that the patient's clinical treatment goals and behavioral management needs are met by attending case conferences, chronic disease management meetings, staff consultation, and individual meetings with staff to ensure quality of care.
Conduct functional and strength-based assessment along with comprehensive diagnosis for mental health, substance abuse, trauma, and psychosocial needs.
Develop student care plans that are coordinated and integrated with a comprehensive range of medical and psychosocial services to address social, emotional, and practical needs.
Provide in-person and remotely (via HIPAA Compliant software) psychotherapy, counseling, medical case management and psycho-education utilizing individual, family, and group modalities using evidence-based interventions for patients and their significant others that include social, emotional, and practical life skills to facilitate safe coping, distress tolerance, stress reduction, mindfulness, and relaxation as well as access to necessary resources with the goal of improved functioning and positive health outcomes.
Utilize both short-term and intensive longer-term interventions, as approved by funding source that target trauma, mental health symptoms, increase psychosocial functioning and reduction of substance use.
Interventions are client centered; strength based, and identified based on a comprehensive treatment plan that is developed, after completing a thorough biopsychosocial assessment.
Consult with primary care providers to enhance understanding of the patient, provide support for treatment approaches that incorporate behavioral health needs, and assist with implementation of bio-psychosocial treatment and care plans.
Work with ECM Case Management staff to coordinate care and provide referrals, coordinate services and reduce barriers.
Participates in weekly case conferences to assist in problem solving related to patients and to ensure that professional guidance and high-quality case management services are being provided.
Participates in community meetings pertaining to the needs of JWCH patients.
Participate in Multi-Disciplinary Team meetings that are held weekly in the clinics.
Be available as assigned to provide after-hours emergency psychiatric assistance.
Provide phone consultation as needed
Productivity goal is to average 8-10 patients per day.
Complete all charting as required by JWCH, including assessments, progress notes, care plans, and billing documentation. Encounters must be closed by the end of each shift.
Assist with provider education for integrated behavioral and primary care services
Participate in behavioral health quality assurance/improvement activities to maintain and strengthen standards of care, in accordance with the organization CQI Plan

Requirements:
Master's degree in social work, social welfare from an accredited college or university.
LCSW with current California BBS license
Demonstrated understanding of the impact of trauma on behavioral and medical conditions
Knowledgeable of evidence-based treatments (sand and play therapy, DBT, CBT, Seeking Safety, Motivational Interviewing)
Knowledge of principles, procedures, methods and techniques of mental health emergency procedures such as: 5150, CPS reporting, APS reporting, and Tarasoff reporting
Experience working with substance abuse disorders preferred.
Bilingual (English/Spanish) ability needed.

Note: If you become a JWCH provider, you may be able to participate in Loan Repayment Program. A majority of our clinics are NHSC approved sites. The National Health Service Corp may be accepting applications Loan Repayment Program awards. The program is open to licensed primary care mental and behavior health providers who are employed or have accepted an offer of employment at an NHSC-approved site. Awardees receive up to $50,000 in exchange for two years of service if they practice in a HPSA of 14 above (Tier I). Awardees can also receive up to $30,0000 in exchange for two years of service if they practice in a HPSA of 13 and below (Tier II).
