    Scheduled Shifts : Friday - Sunday (work hours may vary based on operational needs) The Care Team Specialist (CTS) at Crocker Residential plays a vital role in fostering a safe, supportive, and dignified environment for our residents in a home-like RCFE setting. Grounded in a harm reduction approach and a “do whatever it takes” mindset, the CTS serves as a compassionate, observant presence, monitoring shared spaces, upholding facility rules, and ensuring compliance with licensing standards. Through proactive relationship building, trauma-informed engagement, and non-violent conflict de-escalation, the CTS helps maintain a respectful and stable community. Responsibilities include maintaining accurate sign-in/out logs, conducting safety checks of rooms and belongings, leading emergency preparedness drills, and documenting incidents with care and precision. Program Population: The Crocker Residential Care is a licensed Residential Care Facility for the Elderly (RCFE) that provides 24/7 non-medical care and supervision to adults aged 60 and older who live with chronic health conditions. Our program offers a home-like environment with assistance in activities of daily living (ADLs), medication reminders, and a focus on social engagement and independence. Principal Responsibilities: Establish a positive rapport with participants, providing a safe and supportive environment. Apply harm reduction and trauma-informed care principles in all interactions. Monitor common areas and building perimeter to ensure participant safety and security. Provide crisis intervention and de-escalation support using nonviolent techniques. Consistently monitor and enforce the facility's House Rules and ensure ongoing compliance with all licensing regulations required for Crocker's operation as a licensed RCFE. Maintain sign-in/out logs for participants and visitors at the front desk. Conduct belongings searches and room checks as needed to uphold safety protocols. Serve as floor lead during fire drills, evacuations, and emergency situations. Complete detailed reports on incidents, participant concerns, and program operations. Assist with participant intake, engagement, and connection to supportive services. Facilitate communication between participants and care team members. Respond to participant concerns with empathy, de-escalation strategies, and problem-solving techniques. Ensure program areas remain clean, organized, and welcoming. Collaborate with emergency responders, behavioral health teams, and harm reduction specialists as needed. Maintain strict confidentiality and adhere to all organizational policies. Utilize AIDET (Acknowledge, Introduce, Duration, Explanation, and Thank You) principles in all participant and team interactions. Other duties as assigned. Physical Demands: Stand or walk for extended periods on various surfaces (tile, concrete, carpet). Climb stairs, ramps, or ladders as needed. Kneel, squat, bend, and run if necessary. Lift or carry up to 50- 70 lbs. occasionally. Use hands, arms, and fingers for reaching, handling, and grasping objects. Requirements: High school diploma or equivalent Experience working with individuals experiencing homelessness. Strong communication skills, particularly in de-escalation and crisis intervention. Ability to work with individuals actively using substances in a nonjudgmental, harm reduction-focused manner. Ability to work in a non-traditional clinical setting (e.g., outdoor park-lets, drop-in spaces). Demonstrated knowledge of harm reduction, trauma-informed care, and housing-first principles. Proficiency in basic computer skills (email, data entry, Microsoft Word/Excel). Ability to work flexible hours, including evenings and weekends. Bilingual (English/Spanish) preferred.     The JWCH- Wesley Health Centers Mobile Dental Unit Driver is responsible for providing customer satisfaction through on-time, accurate, delivery of the JWCH mobile dental units to designated locations within regional territories of Los Angeles County in order to promote our objectives of empowering children to practice good oral health, partnering with government and the profession to improve children's oral health, and reducing the prevalence of dental caries worldwide. This position is located in Los Angeles area. JWCH Mobile Dental Unit Drivers are ambassadors who represent JWCH - Wesley Health Centers to our consumer base. They are the point of contact and liaison for JWCH - Wesley Health Centers visits in a range of venues, including schools, colleges, Health fairs, conventions, festivals, and other community events. They are empowered to resolve customer issues to ensure consistent demonstration of service excellence and community outreach. Principal Responsibilities: Operate and safely drive the Mobile Dental Unit (up to 32') over the road, which includes navigating in crowds and following parameters set by sponsoring agencies of event locations. Must be flexible to work various hours and weekends based on the schedule of the event and organizational needs. Perform daily setup of the Mobile Dental Unit at scheduled locations. This includes setting up the mobile unit, cleaning, and restocking supplies. Provide effective customer service as the "first point-of-contact" to dental partners, event coordinators, volunteers, and the public, which may include answering basic JWCH - Oral Health Department program information, answering logistics questions, and directing inquiries/media requests to designated representatives. Liaise with third-party agencies, including maintenance and leasing services, retail establishments, mobile unit storage providers, and events management companies, as part of ongoing duties. Ensure safety during events, including safe entry and exit of the mobile dental unit and staffing registration area, including assisting individuals with mobility challenges using ramps and lifts. Promote and employ safe work practices when handling products on and off company vehicles. Coordinate with dental staff to ensure equipment and supplies are properly loaded and secured for transit. Working under the guidance of the Program Manager, the Site Monitor actively participates in a multi-disciplinary team, contributing to client engagement, retention, safety, and support. Principal Responsibilities: Performs job functions as prescribed by established policies and procedures within defined scope of education, training, and responsibilities.
