Job Description
Shift: Morning, Swing and Graveyard.
The Site Monitor oversees the New Beginning's Program participants' activities and ensuring compliance with program rules and expectations. This position helps maintain smooth and consistent facility and residence operations. The New Beginning Program, is dedicated to providing Interim Housing for individuals experiencing homelessness, guiding them towards recovery and an enhanced quality of life. It's important to note that some individuals in the program may also grapple with mental health and substance abuse issues, which can manifest in challenging behaviors such as yelling, cussing, and angry outbursts.
At New Beginning, every staff member, including the Site Monitor, contributes to assisting clients in overcoming barriers and enhancing coping skills to reduce negative behaviors, fostering a successful transition to Permanent Supportive Housing (PSH).
Working under the guidance of the Program Manager, the Site Monitor actively participates in a multi-disciplinary team, contributing to client engagement, retention, safety, and support. In this capacity, the position collaborates with both staff and clients as an integral care team member, building rapport and cultivating a sense of community within the program.
New Beginning's Interim Housing program is designed to support homeless individuals on their journey to recovery and an improved quality of life. By understanding and addressing mental health and substance abuse issues, we aim to guide our clients toward positive behavioral changes and successful integration into Permanent Supportive Housing.
Principal Responsibilities:
Performs job functions as prescribed by established policies and procedures within defined scope of education, training, and responsibilities.
Establish a positive rapport with clients and express a concern for their well-being.
Demonstrate understanding and practice harm reduction methods and trauma-informed care.
Demonstrate non-violent restraint methods, if needed and only as an absolute last resort.
Monitor the site by walking corridors as assigned, and or being rotated.
Assist with clients' room searches and/or temperature control, as needed.
Act as site lead during fire drills and real emergencies, and as lead in performing necessary evacuations.
Attend each shift change and contribute accordingly.
Document any client and/or client-staff incidents and submit to supervisor in a timely manner.
Use appropriate de-escalation techniques; Case Manager for assistance, as needed.
Demonstrate understanding and awareness of substance abuse and/or negative behavioral health symptoms.
Provide supportive communication regarding incarceration, substance abuse, mental health, and other risk behaviors.
Must be able to work extended shifts when emergency situations warrant in a 24 hour 7 day a week setting.
On occasion will need to work an additional 4 hours after regular shift or 4 hours before the regular shift if situation warrants.
Flexibility to adjust with a multiple schedule change, shift change, and days off. The work schedule is subject to change periodically based on the number of Care Team Specialists.
Assist clients in walkers and wheel chairs if needed.
Discusses any client information only with appropriate personnel when related to the care being provided, and ensures these conversations are not near any clients
Responsibility and accountability for adherence to organizational and department standards and policy.
Self-identifies potentially unsafe systems/processes/situations and takes initiative to report to the supervisor,
Maintain shift reports and communicate with NENB Team when client issues arise.
Physical Demands:
Stand or walk constantly (for up to an entire shift) on various surfaces (tile, concrete, carpet). Climb stairs, ramps, or ladders occasionally during shift.
Must be able to kneel, squat, bend for any duration of time and run if needed.
Occasionally bend/twist at waist/knees/neck to perform various duties, including lift or carry up to 80 lbs.
Constant use of hands, arms, and fingers in reaching/handling/grasping while using radio, phone, notepad, writing reports and other administrative tasks.
Performs other duties and tasks as may be required or assigned.

Requirements:
Minimum high school diploma or equivalency
Associate degree in social services preferred
Two years' experience in behavioral health or social services environment
Previous experience working with homeless persons or population.
Strong ability to communicate with homeless individuals who are actively participating in substance abuse and other factors related to homelessness.
Desire to work with homeless population.
Bilingual (English/Spanish) preferred
Ability to assist with the collection of patient's valuables.
Experience handling confidential documents and detail-oriented.
Ability to use email, database program(s) of agency and working knowledge of word processing (Microsoft Word) and spreadsheet programs.
Effective written and oral communication and interpersonal skills with ability to deal with all levels of personnel, general public and patients in a professional and effective manner; must be able to use initiative and independent judgment within established guidelines.
Ability to assist with emergency and behavioral health personnel with aggressive patient situations.

The Crisis Prevention Specialist/Care Team Specialist (CTS) at Crocker Residential plays a vital role in fostering a safe, supportive, and dignified environment for our residents in a home-like RCFE setting. Grounded in a harm reduction approach and a “do whatever it takes” mindset, the CTS serves as a compassionate, observant presence, monitoring shared spaces, upholding facility rules, and ensuring compliance with licensing standards. Through proactive relationship building, trauma-informed engagement, and non-violent conflict de-escalation, the CTS helps maintain a respectful and stable community. Responsibilities include maintaining accurate sign-in/out logs, conducting safety checks of rooms and belongings, leading emergency preparedness drills, and documenting incidents with care and precision.
Program Population:
The Crocker Residential Care is a licensed Residential Care Facility for the Elderly (RCFE) that provides 24/7 non-medical care and supervision to adults aged 60 and older who live with chronic health conditions. Our program offers a home-like environment with assistance in activities of daily living (ADLs), medication reminders, and a focus on social engagement and independence.
Principal Responsibilities:
Establish a positive rapport with participants, providing a safe and supportive environment.
Apply harm reduction and trauma-informed care principles in all interactions.
Monitor common areas and building perimeter to ensure participant safety and security.
Provide crisis intervention and de-escalation support using nonviolent techniques.
Consistently monitor and enforce the facility's House Rules and ensure ongoing compliance with all licensing regulations required for Crocker's operation as a licensed RCFE.
Maintain sign-in/out logs for participants and visitors at the front desk.
Conduct belongings searches and room checks as needed to uphold safety protocols.
Serve as floor lead during fire drills, evacuations, and emergency situations.
Complete detailed reports on incidents, participant concerns, and program operations.
Assist with participant intake, engagement, and connection to supportive services.
Facilitate communication between participants and care team members.
Respond to participant concerns with empathy, de-escalation strategies, and problem-solving techniques.
Ensure program areas remain clean, organized, and welcoming.
Collaborate with emergency responders, behavioral health teams, and harm reduction specialists as needed.
Maintain strict confidentiality and adhere to all organizational policies.
Utilize AIDET (Acknowledge, Introduce, Duration, Explanation, and Thank You) principles in all participant and team interactions.

Job Description
The Program Aid/Driver provides support to the Program Director, Activity Coordinator, and RN across all facets of the ADHC program. Responsibilities:

Contributes to the nutritional component of the program by reporting lunch meal quantities, monitoring participant intake, and addressing any eating challenges.
Assumes responsibility for the preparation and service of snacks and meals. Program Aides provide assistance to participants when cutting food is required and offer feeding support when necessary.
Maintains the cleanliness and orderliness of the kitchen area after meal service.
Ensures the verification and documentation of food temperatures upon arrival at the center, maintaining appropriate levels consistently.
Offers assistance with various aspects of personal hygiene, aiding the RN in maintaining continence and toileting schedules, and facilitating clothing changes as needed.
Participates in the execution and supervision of arts and crafts projects and recreational activities.

Works in close collaboration with the Nurse and Rehabilitation Staff to implement any maintenance programs.

Participate in JWCH's customer service training called AIDET on an annual basis.

Job Overview
The Community Health Workers (CHW) are trusted members of their community who help address chronic conditions, preventive health care needs, and health-related social needs. CHWs have the skills and experience to understand what at-risk or vulnerable patients are going through and help them get through difficult times. CHWs help patients address the social, medical, and community problems that lead to poor health. The goal is to help patients address real-life obstacles (e.g., social driver of health) that keep them from staying healthy, by supporting them with health care navigation, specialty appointments/referrals, PCP appointments/establishing routine care and preventative health screenings, cultural broker, financial services, and provide culturally appropriate health information on topics related to chronic disease management and prevention. CHWs receive training in Epic & Essette, DHCS requirements, trauma-informed care, motivational interviewing, health care navigation, harm reduction, mental health first aid, domestic/intimate partner violence, advocacy, insurance enrollment, social service/resource applications and referrals, and basic case management.
Minimum Requirements
* A High School Diploma or GED is required.
* Minimum of 2 years of experience working in health care or community outreach settings.
* Bilingual English/Spanish/Mandarin/Cantonese, depending on location preferred.
* A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.

The Software Engineer position will be responsible for the analysis, design, development, testing, documenting and support of information systems. This role primary focus will be working with "SHARE" software system. SHARE system is in use by hundreds of internal users daily to process different crucial workflows. The Rehabilitation Aide will perform or assist with personal care ADLs/IADLs for participants in the home if needed.
