PACT (Program for Assertive Community Treatment) is an intensive and highly integrated approach for community mental health service delivery. The multidisciplinary team shares a caseload of individuals who experience symptoms of the most serious forms of mental illness, predominantly but not exclusively the schizophrenia spectrum disorders. Many individuals also experience substance abuse, victimization and trauma, psychiatric hospitalizations, arrests and incarceration, homelessness, and additional significant challenges. The symptoms and complications of mental illnesses lead to serious functioning difficulties in several areas of life, often including work, social relationships, residential independence, money management, and physical health and wellness.
Essential Duties & Responsibilities:
Collaborate with a multi-disciplinary team including mental health professionals, nurses, prescribers, peer counselors, SUD specialists, and vocational staff.
Deliver recovery-oriented services such as case management, treatment planning, resource assistance, crisis intervention, and group/individual services.
Assist clients in identifying and accessing resources and collaborate with community resources on behalf of clients.
Assist clients in the development of healthy coping mechanisms in individual or group settings.
Conduct risk assessments, develop and implement safety plans, and provide crisis intervention and stabilization services as needed, including facilitating access to inpatient facilities for behavioral health care.
Provide ongoing support to clients in their homes and community environments.
Participate in the 24/7 on-call rotation and deliver after-hours support as needed.
This position requires regular driving and mobility in community settings.

Familiarity with trauma informed care, the administration of long acting injectables and de-escalation techniques, a plus.

Position Summary:
A RegisteredNurse holds ultimate responsibility for direct and indirect nursing care, including providing nursing care, health maintenance, teaching, counseling, planning and restoration for optimal functioning and comfort. Skills include demonstrating a comprehensive understanding of the department's immediate needs and long term growth goals, showing initiative, using critical thinking to problem solve independently, exhibiting leadership qualities, and effectively engaging with the clinical team.
Essential Duties & Responsibilities:
Assess a patient through obtaining patient history and performing appropriate tests and observations.
Communicate with the patient's provider and educate the patient on test results, new care plans and/or other relevant health care information including the use of newly prescribed medications and/or changes in the current medication regime.
Demonstrates sound clinical judgment/decision making when triaging sick patients, both on the phone and in the waiting room.
Safely and effectively administers medications, tests and treatments as ordered.
Teaches patients and families health care regimens. Explains and provides information regarding disease and wellness, tests, results, medications, diet, exercise, and other aspects of the plan of care to patients.
Qualifications and Requirements:
Bachelor of Science in Nursing preferred.
Completion of Accredited Nursing Program.
Licensure in Massachusetts as an RN.
BCLS certification per department policy.
1 year relevant nursing experience in a community health and/or ambulatory care setting preferred.
