Position Summary:
The Inpatient Diabetes Educator, RN provides expert clinical guidance, education, and consultation for hospitalized patients with diabetes and related endocrine disorders. This role supports both direct and indirect patient care through staff education, individualized patient teaching, interdisciplinary collaboration, and evidence-based practice. The educator serves as a hospital-wide resource and clinical leader for diabetes management and works closely with providers, nursing staff, and ancillary teams to ensure safe, effective, and coordinated care across the continuum.
Minimum Requirements:
Current RN license in the State of Washington.
Bachelor's degree in Nursing required; Master's preferred.
Certified Diabetes Care and Education Specialist (CDCES/CDE) credential required (or obtained within 6 months of hire).
Recent acute care and clinical experience in diabetes education/clinic or endocrinology preferred.
Current BLS certification.
Proficiency in EMR documentation (Epic experience preferred).
Key Responsibilities:
Conducts individualized diabetes education for inpatients and their families, focusing on disease understanding, medication management, insulin administration, nutrition, blood glucose monitoring, and discharge planning.
Collaborates with physicians, advanced practice providers, nursing, dietitians, and pharmacists to optimize glycemic control and reduce complications.
Reviews and updates patient diabetes education materials and clinical protocols in alignment with ADA and hospital standards.
Provides consultative support to nursing and medical staff regarding complex diabetes management, including insulin pump and CGM utilization.
Leads and participates in quality improvement initiatives related to glycemic management, hypoglycemia prevention, and transitions of care.
Collects, tracks, and reports data on key performance indicators (e.g., hypoglycemia rates, insulin protocol compliance, patient education completion).
Educates and mentors nursing staff and new hires on inpatient diabetes care best practices.
Serves as a liaison between inpatient and outpatient diabetes programs to ensure continuity of care post-discharge.
Participates in hospital committees focused on quality, safety, and patient outcomes as related to endocrine care.
Maintains required documentation within the EMR in accordance with hospital policy and regulatory standards.

Position Summary
Provides individualized diabetes self-management education/ training to individuals and groups according to the Scope of Practice, Standards of Practice, and Standards of Professional performance for Diabetes Educators (AADE, 2005).
The Registered Dietitian is a member of the interdisciplinary team working in the out-patient diabetes education program.
The RD utilizes the care process consistent with their license or certification including initiation of interventions within individual scope of practice.
The RD incorporates diabetes education principles that are current and reflective of the standards set forth by the American Diabetes Association, The American Association of Diabetes Educators and The American College of Endocrinologists in respect to the management of Type 1, Type 2 and gestational diabetes.

PACT (Program for Assertive Community Treatment) is an intensive and highly integrated approach for community mental health service delivery. Many individuals also experience substance abuse, victimization and trauma, psychiatric hospitalizations, arrests and incarceration, homelessness, and additional significant challenges. The symptoms and complications of mental illnesses lead to serious functioning difficulties in several areas of life, often including work, social relationships, residential independence, money management, and physical health and wellness.
Essential Duties & Responsibilities:
Collaborate with a multi-disciplinary team including mental health professionals, nurses, prescribers, peer counselors, SUD specialists, and vocational staff.
Deliver recovery-oriented services such as case management, treatment planning, resource assistance, crisis intervention, and group/individual services.

Position Summary

Responsibilities
* You will provide age-appropriate care utilizing nursing education and knowledge of normal states of growth, development, and aging
* You will deliver and execute upon daily routine care and services to patients from diverse ethnic and social groups utilizing your strong interpersonal skills
* You will assist the RegisteredNurse or care team in implementing the nursing process
Requirements
* Graduate of an accredited Licensed Practical Nurse program

Position Summary:
In general, all registerednurses are prepared by training and experience to hold ultimate responsibility for direct and indirect nursing care, including providing nursing care, health maintenance, teaching, counseling, planning and
restoration for optimal functioning and comfort. The scope of practice for the BH Psychiatric RegisteredNurse is focused on the indirect assistance of patients and the psychiatric provider team in the management of critically important communication with respect to coordination of the psychiatric care of patients. In the role, direct care of patients includes psychoeducation and
teaching, counseling, especially with respect to medication management, and triage with respect to symptom and crisis management.
Excellence in nursing practice also includes clinical skills and capabilities beyond baseline competence. These include the ability to conduct a comprehensive assessment that incorporates an advanced and sophisticated integration of clinical information, and the ability to translate that assessment into a superior plan of care for the patient/family/care team. Psychiatric RegisteredNurse is expected to possess and demonstrate competence in these more advanced clinical skills.
This position is primarily focused on in-basket and care coordination work-triaging messages, managing refills, responding to patient concerns, and supporting psychiatric providers embedded in both standalone psychiatry departments at the South End site and in primary care-integrated settings in East Boston. This position may involve direct patient visits and requires
strong clinical judgment, excellent communication skills, and the ability to work efficiently within an EHR system (Epic).
In addition, excellence in this position includes global skills and capabilities beyond baseline competence. These include demonstrating a comprehensive understanding of each BH departmental service line, demonstrated interest in long term growth goals, showing initiative, using critical thinking to problem solve independently, exhibiting leadership qualities, and effectively engaging with the multidisciplinary clinical team in the BHUCS, the Satellite Emergency Facility (SEF), the other clinical departments at EBNHC and affiliated sites, and NSMHEssential Duties and ResponsibilitiesGeneral Practice Competencies
Assess the health status of individuals and accurately record the related health data:
Navigate the medical record
Collect data on medical and behavioral health/symptoms from a patient
Analyze and interpret recorded data, and make informed judgments as to the specific elements of nursing care mandated by a particular situation:
Correlate new data and symptoms with the existing medical records
Determine indication for the appropriate response level from the other team members within established guidelines
Plan and implement nursing intervention, including all appropriate elements of nurse care, prescribed medical or other therapeutic regimens mandated by the particular situation, scientific principles, recent advancements and current knowledge in the field:
Review documentation to assure consistence with current planned regimen
Communicate with the multidisciplinary team and educate the patient on test results, new care plans and/or other relevant health care information including the use of newly prescribed medications and psychotherapies and/or changes in the current regimen
Dispense medication by mouth, nebulizer, IM, IV, and SC as ordered by an MD/NP/PA/DO or per protocol
Provide and coordinate health teaching required by individuals and families to maintain the optimal possible level of health:
Educate on health maintenance, current health status, and changes in health status
Evaluate outcomes of nursing interventions, and initiate change when appropriate:
Assess and reassess patients as needed
During data collection and patient interviews, review the current plan of care to determine appropriateness and areas of weakness
Communicate status of plan of care with other members of the health care team
Collaborate, communicate and cooperate as appropriate with other health care providers to ensure quality and continuity of care:
Report to the other team members through documentation in the medical record a comprehensive integration of newly acquired information with the patient's history and related changes in the nurse's plan of care
Report to other caregivers, through the medical record or verbally as needed, the patient's physical and behavioral health status. Incorporate information from other caregivers into the patient's care plan as appropriate
Serve as a patient advocate, within the limits of the law:
Integrate the vital patient advocacy function into the nursing role in a continuous effort to encourage patient self-determination to the fullest degree possible through patient and family education and facilitation of a process whereby patients are encouraged to give voice to the concerns, questions, and preferences they may have regarding their care. Utilize the information gathered in the role of patient advocate to inform other providers caring for the patient of special needs and concerns and ensure the appropriate systemic changes and care of plan modifications to the degree possible based on specific patient need.
Knowledge, Skills, and Abilities RequiredEducation
Graduation from an accredited nursing program required. BSN preferred.
Experience
Minimum of two years of relevant nursing experience, in community health behavioral health, emergency and/or ambulatory care setting, psychiatric facility. Previous experience in telehealth, patient portal messaging and care coordination.
Familiarity with trauma informed care, the administration of long acting injectables and de-escalation techniques, a plus.
Skills/Abilities
Current Massachusetts RN licensure
ACLS, BLS, and PALS certification required
Psychiatric Mental Health Board Certification preferred
Strong ability to communicate effectively both in writing and verbally
Bilingual (Spanish) preferred
Strong understanding of psychiatric conditions, medications and symptom management
Proficient in EHR systems management (Epic experience strongly preferred)
Strong ability to multitask and work in a fast-paced, high volume, hybrid (in person and telehealth care) environment
Schedule and Pay:
Monday - Friday; 8:00am - 5:00pm
Starting Pay at $44.00 and up to $64.00 hourly based on experience
