Provides ambulatory pharmaceutical care services using approved protocols in collaboration with other health professional, including managing patient drug therapy, providing patient education, and preparing and/or overseeing the preparation and dispensing of medications. Promotes high quality, evidence-based, cost-effective pharmaceutical care. Performs these duties in the clinical areas assigned. Essential Responsibilities: + Upholds Kaiser Permanentes Policies and Procedures, Principles of Responsibilities and applicable state, federal and local laws. + Conducts patient medication history inquiries upon referral from physicians and other medical personnel. + Given a diagnosis, synthesize a treatment plan, which may include drug therapy and lifestyle modifications, and transition within the continuum of care settings. + Identifies, resolves, and prevents potential and actual medication-related problems including: untreated indications, improper drug selection, subtherapeutic dosage, overdose, failure to receive medication, adverse drug reactions, drug interactions, and medication without indication. + Under protocol, initiates, adjusts, monitors, and recommends changes in drug therapy based on information obtained through patient interviews, objective monitoring parameters, review of drug profiles and the patient medical record. + Coordinates and interprets laboratory monitoring of drug therapy, including serum drug levels, electrolytes, and other approved drug-related laboratory testing. + Provides patient monitoring for the purpose of evaluating and optimizing patient outcomes, drug utilization and patient compliance, detecting and/or minimizing ADRs, eliminating unnecessary drug use and duplication. + Provides patient education related to the individuals disease state, including predictable actions, side effects, reasons for proper compliance, and any special instruction. + Assure ongoing performance improvement of the pharmaceutical care services provided. + Provides education to providers and staff via presentations, newsletters, formal utilization reviews, and one-on-one interactions. + Performs drug utilization review, comprehensive medication review, and/or medication reconciliation. + Operate pharmacy computer system(s) and other equipment and software. + Documents information appropriately in the patients medical record and other computer systems according to policies and procedures and regulatory requirements + Supports a collaborative Labor-Management Partnership environment. + Performs other duties as required. Basic Qualifications: Experience + Minimum twelve hundred (1200) hours experience in the last five years working under collaborative drug therapy management protocol including initiating, adjusting, discontinuing, and refilling medication in patients with chronic disease states OR successful completion of an ASHP Accredited PGY-1 residency. + A qualified candidate should have at least ONE of the following credentials or experience: Board Certified Oncology Pharmacist (BCOP) OR, Have completed PGY2 in oncology practice OR, a candidate that has completed 1000 hours over last three (3) years in area of sterile compounding oncology pharmacy with direct clinical protocol management; on condition of achieving BCOP within three (3) years of employment OR, a candidate that has successfully completed an ASHP accredited PGY1 Residency (within three (3) years), with an oncology pharmacy rotation during the residency year and condition of achieving BCOP within three (3) years of employment Education + Degree in Pharmacy meeting minimum mandatory educational requirements for licensure. Performs various maintenance task and repairs most equipment and machinery utilized in the Medical Center. Operates machines vital to performing of maintenance and repair. Operates, maintains, modifies and repairs a variety of air conditioning and refrigeration equipment. Performs various maintenance tasks such as replacing filters, lubricating bearings, repairing cabinets. 2. The Care Connections Nurse Practitioners focus on screening and preventive primary care services delivered in the home, community, and nursing facility settings. Provides needed care in the environment that patients feel most comfortable and are most receptive including home, nursing facilities, and "pop up" clinic. The Nurse Practitioner will be required to work primarily in non-clinical settings and provide medical care to all levels of patients. Some programs may focus on specific populations (e.g., adult and geriatric, pediatric, women's health). Searches for cardiac pathology moving transducer over heart at varying angles to visualize and record various anatomical areas of heart by way of real time imaging. Evaluates direction and speed of intracardiac blood flow using doppler. Asses cardiac valves for pathology. * Utilizes ultrasonic imaging devices (i.e., transducers, monitors) to produce optimized image quality. * Explain examination, test and medical procedures and purpose of it. Review, explain, and clarify physician orders with patient or parent. * Reviews referrals and past echo if applicable and documents per workflow. * Greets patients and explains procedures; completes medical record documentation; reviews order, referrals, and past echo, if applicable, and documents per workflow for any specified area of focus. * Positions patient to obtain the desired images; manipulates equipment to obtain sufficient quality and quantity of images for diagnostic interpretation. * Completes department specific logs as directed. * Computes recorded data to determine response factors, using computation tables and formulas, and prepares reports for interpretation by cardiologist. * Adjusts and sets-up equipment, checks functionality of equipment, sterilizes probes. Performs minor adjustment on the echocardiogram equipment and arranges for service for any major repairs. * Orders necessary supplies to ensure ongoing utilization of the equipment. * Confers with cardiologist concerning special patients to determine if areas other than those considered standard for a particular problem are to be imaged. * Responds immediately to Stat requests from physicians for patient echocardiogram examinations an relays findings to the referring physician by phone or by transmittal of examination reports. * Completes order in Health Connect. Completes all required documentation in echo system and sends echo for interpretation by cardiologist. * Downloads information/images to digital system. Per department protocol, provides CD copies of images. * Performs EKGs EKG tracing with echocardiogram. * Establishes and maintains courteous and cooperative relations when interacting with all customers. * Provides training to physicians and other Cardiac Sonographers. * Participates in on-call rotation. * Participates in the weekend rotation. * Performs other related duties as required.

Physician Assistants (PA) provide health care services to patients who may have acute episodic conditions or chronic problems, as well as those seeking health evaluation, preventive care and/or education and counseling. Under physician supervision, PAs provide care in areas within KP in primary care or specialty departments consistent with their educational preparation following the Regional Practice Guidelines and Protocols for Ambulatory and Hospital-Based Practice. They perform complete physical examinations, including ordering, interpreting and evaluating diagnostic tests and examinations. Under general supervision, operates radiographic equipment, including whole body CT scanners and angiography equipment; develops x-ray film; maintains adequate records; assists physician in fluoroscopic examinations; operates portable x-ray equipment, and follows standard radiation and safety procedures. Assist physician during minor surgical procedures, and in developing new techniques and procedures. Assist in obtaining tissue specimens. Assist provider in patient examinations and procedures. Follow standard infection control procedures. Lift patients. Modify treatment techniques which do not require change in physician orders. Monitor and evaluate test readings during procedures. Observe, monitor and evaluate patient condition during and after procedures. Prepare and position patients for diagnostic imagining tests. Respond to emergencies (i.e., stat pages, Code Blue) and stat requests. Shave and apply antiseptic solutions to patient skin areas. Transport patients to/from procedure area. Assist physician with fluoroscopic examinations. Assist in cardiovascular procedures. Develop digital or film images and make prints if applicable. Load and unload imaging film in radiology equipment. Mix and administer barium to patients. Perform whole body CT scans. Take radiographs. Assist physician with inpatient and outpatient pacemaker procedures. Perform radiation safety monitoring. Clean darkroom, procedures room, and work area. Records patient information. Review patient charts to evaluate patients and identify significant conditions. Compile data for Quality Assurance. Orient and train new or less experienced personnel. Attend conferences, meetings and in-service training programs.
