Department: DMP Admissions Work Type: Per-Diem **Scheduled Work Hours:** Variable Shift 9 **Job Description** **Under the direction of the Site Manager or Supervisor, Patient Access Services, this position is responsible for completing the inpatient and outpatient scheduling, registration, insurance verification, financial clearance processes, chart preparation, and creating the first impression of Kaleida Health's services to patients, family members and other external customers. Additional responsibilities include interviewing patients/guarantors to obtain demographic and confidential clinical and financial information necessary to appropriately schedule, register, obtain treatment authorizations and verify insurance eligibility and corresponding benefit levels. Critical to this position is the ability to gather and record all appropriate information, consents and forms that lay the groundwork for the remaining or future fiscal efforts of Kaleida Health.**

24. Schedule appointments for various services referred via the Patient Access Navigator Program and notifies patient, documents, and follows-up with all requests.
25. Case Management Support: Logs approval and denials in Epic for Peer-to-Peer discussions
26. Notifies case managers of all correspondence from insurance carriers and health partners requesting clinical reviews, discharge summaries and other documentation.
27. Processes patient's discharge appeal as appropriate.
28. Assist Director as needed with schedule and timekeeping, training, and other managerial responsibilities.

1.Works cohesively with the Primary Care Provider to facilitate patients care plan by participating in daily huddles/monthly case discussions, facilitating appointments and coordinating information and continuing care requirements.
2. Collaborates with other clinical staff, to resolve issues impacting patients care progression.
3. Monitors and updates dashboard with reminders and alerts, to organize daily tasks such as: visit status reminders, follow-up appointments, authorizations for medical review, coordination of specialty appointments and resolving billing errors (when applicable).
4. Contact insurance carriers for patients pre-certification; consults with Department/Practice Manager with respect to patients needs or addressing escalated issues.
5. Provides front-end coverage as needed; records telephone encounters for routing to appropriate Providers.
6. Works as liaison with Physicians, patients and clinical staff to connect patients with relevant internal/external resources.
7. Provides recommendation for process/service improvement to promote patients satisfaction and address issues.
8. Performs other duties as assigned.
