OBJECTIVE/SUMMARY: The Mental Health Worker is responsible for participating in the care of psychiatric clients within the limitations of training and experience of a mental health worker. CLERK, ACCOUNTS PAYABLENON-EXEMPT TITLE OF IMMEDIATE SUPERVISOR:DIVISION: ACCOUNTING MANAGERACCOUNTING OBJECTIVE/SUMMARY: The Accounts Payable Clerk, will be an integral member of the Accounting team that is responsible for providing accurate financial information. In this role, you will perform the day to day processing of accounts payable transactions to ensure that finances are maintained in an effective, up to date and accurate manner. GENERAL STATEMENT OF FUNCTIONS: Under the general direction of a licensed clinical program director, supervises and coordinates multidisciplinary and ancillary support staff in the planning and provision of mental health services to emotionally or mentally disturbed adults, and older adults. Provides daily clinical and administrative oversight to the team, with technical supervision of licensed clinical staff activities exercised in accordance within scope of practice guidelines. Also oversee other professionals, paraprofessionals and/or general service staff. Additionally, assist in coordinating social work and treatment activities with medical, psychological and other professional services. Lastly, persons in this position are expected to administer and interpret policies and procedures. MINIMUM QUALIFICATIONS REQUIRED FOR THIS POSITION: Ph.D. in Clinical Psychology or Master's Degree from an accredited school of social work including supervised field work placement in psychiatric social work, Registered Nurse, or Licensed Marriage, Family, Child Counselor, or Licensed Clinical Social Work. Completion of two year fellowship in adult therapy from an approved postgraduate training program desirable but not required. Knowledge of coordinated service and Rehab-option essential. At least three years clinical work experience in a psychiatric clinic or hospital involving problems of mental and emotional adjustment of adults, and the use of psychiatric consultation desirable. Ability to formulate diagnoses, conceptualize and implement appropriate treatment plans. License to practice psychology, social work, nursing, or marriage and family therapy in the state of California, and fulfillment of requirement to function as a supervisor in a Children, Youth, and Family Services milieu. Experience in Program Administration and Clinical Supervision desirable. Must be capable of flexibility in service delivery within coordinated services model and its interrelationship with the total structure of the Adult Services Program, Mental Health, Acute and Residential care and adjunctive services. SPECIFIC DUTIES: 25% - 35 hours/month * Provide clinical and administrative supervision of professional and para-professional staff providing services in the FSP program. OBJECTIVE/SUMMARY: Under the direction of the Chief Medical Officer, provides continuing, comprehensive health wellness, preventive and medical care to adults and children. Coordinates and manages the care of these vulnerable patients and that they receive optimal care, including acute illness management, chronic disease management, and preventive care. Follows established departmental policies, procedures and objectives, continuous quality improvement objectives, and other control standards. Works with other healthcare providers, non-medical personnel, and participates in quality assurance, quality improvement, peer review and special projects. OBJECTIVE/SUMMARY: A Medical Assistant (MA) at Kedren Community Care Clinic (KCCC) performs a wide variety of duties and responsibilities in a manner that places emphasis on quality of care and customer service. The incumbent must work collaboratively with all clinical services staff in support of direct patient services, exhibiting flexibility and a “can-do” attitude. Patient services are the key priority in this position requiring the MA to serve as a point of contact with other internal and external departments, all with the goal of fostering an environment which promotes patient comfort and trust. This position must exemplify the core values and mission of the organization, always exercising utmost discretion, diplomacy and tact in patient/staff interactions. REQUIRED SKILLS AND QUALIFICATIONS: 1. High School Diploma or equivalent (GED) required 2. Graduate of a Medical Assistant Program/Certified Medical Assistant preferred 3. Prepares for patient appointments by reviewing and updating patient records prior to appointment using clinic's EHR. This includes checking for updates of recent diagnostic test results, lab results, approved/denied referrals, telephone encounters, and all correspondence received. Also reviews patient chart in EHR to determine needed services and whether referrals are in process or have been completed since last visit. 2. Maintains exam rooms according to protocols: Cleans rooms between patient visits to ensure patients' comfort and compliance with infection control standards; ensures all necessary supplies are stocked. Promptly reports to clinic manager when supplies are low to ensure timely re-ordering of supplies. 3. Appropriately prepares patient for visit and alerts licensed provider via EHR when patient is ready to be seen. When a licensed provider is unavailable, the MA keeps the patient updated on the status of their appointment and when necessary assists the patient in re-scheduling or scheduling follow-up visits. Instructs patient as to general clinic policies, patient's rights and responsibilities as needed. 4. Provides initial medical screening: Greets patients and directs to appropriate exam room after recording weight, height, temperature, blood pressure, pulse, obtain basic medical history, and completes screening questionnaires; accurately documents in EHR according to general accepted medical standards. 5. Carries out all provider orders including administration of immunizations, injections, oral medications, and procedures. Verifies the correct medication and dosage with licensed provider prior to administering. Verification of competency is required. 6. Assists providers upon request during examination of patient and according to general accepted medical standards. Practices universal precautions with respect to established safety protocols. Facilitates communication between the provider(s) and patient as needed, including language translation as appropriate. 7. Discharges patients after directed by provider. During the discharge process, MA must provide patients with visit summaries, lab orders, diagnostic imaging orders, medication prescriptions, follow up appointments, and checks out patients in EHR. 8. Utilizes time in a constructive and efficient manner by assisting in such duties as scanning of patient documents in EHR, restocking rooms/shelves/trays, checking supplies and ensuring that all areas and exam rooms are clean and orderly. 9. Performs general clerical duties in support of patient services according to the needs of the clinic and as directed by the Clinic Manager or Director of Nursing and provider(s). These include but are not limited to answering phones, photocopying, mailing letters, faxing, typing, scanning, completing patient forms, etc. Works with EHR practice management system to assist Front Office Clerks with orderly and efficient patient flow. Provides administrative back-up support to Clinic Manager, Lead NP and/or Medical Director as assigned and as necessary for smooth and efficient operation of the clinic. 10. Accepts floating assignments to other areas of KCCC (e.g. Lab, Amity, Bldg. B, AOP, Wellness, etc.) as needed and assigned. 11. Maintains work station according to protocols. Performs routine quality check of equipment and promptly reports damaged or missing equipment from patient rooms. - Keeps equipment operating by following operating instructions; troubleshooting breakdowns; maintaining supplies; performing preventive maintenance; notifying manager for equipment repairs in timely manner. 12. Ensures clinical areas are organized with respect of standards in safety and quality patient care. 13. Works collaboratively with Clinical staff and other departments in solutions-oriented problem solving with the goal of providing quality patient care. 14. For safety and security reasons, the MA is responsible for knowing the whereabouts of a patient any time the patient is in clinical areas. In the event a patient or clinic emergency, the MA may be called to activate the proper ‘code', assisting provider(s) and clinical staff with the appropriate emergency response. In the event of evacuation, the MA will assist in the evacuation of patients being seen by her/his provider(s) as scheduled. 15. Performs EKGs and acts as backup for lab technologist, when directed by licensed provider, Supervising NP, Lead MD and/or Medical Director. 16. PHYSICAL DEMANDS AND ENVIRONMENTAL REQUIREMENTS: Medical Assistants may spend 80% of their professional day walking, standing, moving and manipulating equipment, as well as lifting and moving objects, equipment and supplies. It is reasonable to anticipate lifting 20-25 pounds in any given day. Lifting up to 40-50 pounds may occasionally be required, depending on the specific style or type of care provided. Job Description JOB CLASSIFICATION: ECM Lead Care Manager NON-EXEMPT TITLE OF IMMEDIATE SUPERVISOR: Chief Operations Officer - Primary Care DIVISION: COMMUNITY CARE CLINIC OBJECTIVE/SUMMARY: The ECM (Enhanced Care Management) Lead Care Manager will outreach and enroll clients in enhanced care management and be responsible for overseeing the development and implementation of comprehensive care plans for high-need, high-risk patients within the ECM program. They will work with leadership, providers and managed care to determine the needs of high acuity, vulnerable patients and provide basic housing assistance and patient-tailored intensive case-management, developing a care/service plan, providing linkages to medical, psychiatric, social, educational and other services as needed. This role will coordinate care activities, collaborate with interdisciplinary teams and ensure optimal care delivery to improve patient outcomes. They will also collaborate with the Community Supports Program staff to provide team-based, patient-centered care management for homeless and at-risk of homelessness patients. They will develop and maintain Care Management Plans and review with the ECM team for compliance.
