Schedule: Part-Time; 24 hours/week. ***Monday - Friday, 8 am - 4:30 pm with flexibility on evenings and weekends as needed. Salary: $25-$28/hour In this role, you'll get to: Provide direct skill training, case management, supportive counseling and crisis intervention as directed by the individual. Promote individual choice and self-expression based upon quality service principles. Facilitate and coordinate, with the individual, access to needed supports, including generic community services. Provide support and assistance to individuals necessary for them to develop and maintain relationships with others. Provide transportation as appropriate in response to individual schedules and desired activities and/or appointments. Work cooperatively/collaboratively with other community service providers and medical professionals to ensure coordination of supports and services. Ensure that individuals are treated with respect and dignity at all times, and maintain confidentiality regarding their records and other information. Ensure completion and maintenance of documentation as required by KDC and all applicable funding agencies. Perform other job-related responsibilities as assigned. The Respite Case Manager provides direct services to adult Department of Mental Health clients who are enrolled in short-term transitional clinical and rehabilitative services, both in a residential setting and in the community. The Case Manager assists persons served in setting individual goals for their time enrolled in the respite program and takes all steps that may be needed to help the person achieve their goals. Respite services are strengths-based and person-centered, and are intended to support persons served to maintain, enter or return to a stable living environment. The Case Manager is able to motivate and empower a population with severe mental health disabilities, while also maintaining the daily operations of a 6-bed residential setting, all while demonstrating a high level of administrative and organizational efficiency. The Respite Case Manager is also the principal staff involved in medication administration and documentation. This is a non-exempt position Supervisory Responsibilities: None Essential Functions of Position: Engages in relationship building with Persons Served to manage symptoms and/or stressors and to increase his/her ability to remain in a current living environment or transition to another community living situation. Provide direct support and short-term case management for assigned Persons Served, both in a residential shelter setting as well as in the community. Assist Person Served in writing and achieving individual treatment goals, including assistance with housing, rehabilitative goals, appointments, and other community living supports. Job Summary: The Case Manager provides high quality, culturally sensitive, person-centered intensive case management services utilizing evidence-based practices of motivational interviewing, trauma-informed care, and harm reduction principles and employs creativity and perseverance when necessary to engage individuals toward their housing goals. The Case Manager contributes to Bay Cove's mission by engaging and empowering individuals to meet their personal housing goals. This is a non-exempt position. Essential Functions of Position: Upon intake and at least annually thereafter, assess participant concerns and/or barriers to housing including, but not limited to, financial instability, legal issues, behavioral and physical healthcare needs, substance use disorders, employment, and housekeeping. Engage participants to create person-centered, strengths-based, and individualized service plans (ISP), to be periodically evaluated and updated as necessary/appropriate, but at least once every 90 days. Assist individuals with housing search including contacting prospective landlords and/or management companies, accompanying individuals to view and apply for units, and attending appointments at housing agencies. Assist individuals with move-in transition supports including furniture and furnishings assistance, budget support, and other resources and services as needed. As needed seven days a week - 2pm-12am - 10 hour shifts - Per Diem Job Summary: The Outreach Advocate provides on-street relationship-building and information and referral services for men and women experiencing homelessness, substance abuse problems, and/or are at risk for HIV in the catchment area of Cambridge, Somerville, Arlington, Watertown, and Belmont. Outreach Advocates collaborate with external agencies including other social service programs, area hospitals, police departments, and the community-at-large to develop effective responses to substance abuse problems, HIV/AIDS, and homelessness. Individuals holding this position are the “face of the agency” in the community and must be able to represent CASPAR in an appropriate and professional manner to the community-at-large. Essential Job Duties and Responsibilities: Establish and maintain professional relationships with clients in the catchment area. Meet clients “where they are at” both geographically (e.g., on a park-bench, in a train station, on the street, etc.) and personally (e.g., intoxicated or sober). Establish and maintain professional relationships with other service providers, local businesses, and concerned citizens in at least the catchment area. Thorough awareness of substance abuse, HIV/AIDS, and homeless resources (e.g., shelter and housing services, drop-in centers, clinics, detox and other health care programs, prevention programs, etc.). Connect clients with desired programs through direct referrals (e.g., calling a detox, confirming a bed, and transporting client to the detox) and indirect referrals (e.g., pass along information about particular programs).
