Your function is a first line administrative supervisor whose responsibilities includes but not limited to directing the nursing and patient care activities of subordinates, i.e. licensed practical nurses, geriatric nursing assistance, unit techs and registered nurses, if applicable. The primary purpose of your job position is to assist in planning, organizing, developing and directing Nursing service in accordance with the current applicable federal, state and local guidelines and regulations, and as may be directed by the Director of Nursing or Administrator to assure the highest degree of quality patient care can be maintained at all times. Our goal 1. Work with facilities consultants as necessary and implement recommended changes as directed by DON/ADON. Arrange for a diagnostic, therapeutic, consulting services, as ordered by physician either outside the facility or inside facility per policy. Notify the attending physician and DON/ADON when a patient is discharged without approval. Assure that appropriate documentation is made per facility policy. Report all hazardous conditions and equipment to maintenance immediately. Patient care Provide direct care as staffing patterns demand Adhere to facility policies and procedures and participates in facility Quality Assurance and Performance Improvement Program. Ensure that patient rights to fair and equitable treatment, self-determination, the individuality, privacy, property and civil rights including that to wage a complaint are followed by our department personnel. Review and evaluate your assigned personnel and make recommendations to department director. Maintain confidentiality (by following current HIPAA laws) of all pertinent patient care information to assure patient rights are protected. Notify the patients physician as well as patients guardian/ representative of any changes in condition i.e. ill, injured, as outlined within our established policies. In case of death provide end of life care, offer support to family, may call the funeral home as requested by the family or otherwise outlined by our establish policies. Assured that the body is properly prepared/cleaned for pick up. Make independent decisions regarding nursing care within the scope of practice of your licensure. Obtain vital signs (from unit secretary if available) assess patient's condition or change in condition and chart as required. Order patient medications Prepare/administer medications and treatment as ordered by physician (verify ID of patient before administration of medications) Admit, transfer and discharge patient. Assist in arranging transportation, packing, etc. complete required forms upon patients admission, discharge, transfer, etc., file in the patients chart and/or forward relevant information to appropriate department. Administer professional service such as, Catheterization, tube feeding, suction, I.V. applying, wound dressing, colonoscopy and drainage bags, collect sputum, urine, cultural specimens per facility policy. Check food in patient rooms that families/ visitors brought in per facility policy. Make rounds with physicians and assist in examination and treatment. Notify physicians of auto shop orders on specific medication's as establish per facility policy. Reviews and documentation Encourage attending physicians to review treatment plans, record and sign physician orders, progress note etc. Give/ receive nursing report upon reporting in an ending shift duty hours. Ensure monthly summaries are completed per given schedule. Answer telephone inquiries per policy. Chart nurses notes in informative and descriptive manner that reflects the care provided as well as the patient response to treatment. Receive verbal orders from physicians and chart and transcribed per facility policy Review patient care plans, MAR/TAR at least monthly and make recommendations to DON/ADON. Check and monitor in patient charts, specific medication and treatments, i.e. Procrit, lovenox. Report med errors, documentation errors, physician order errors to DON/ADON. Review care plans for appropriate goals, problems, approaches etc. to assure appropriate nursing care is being delivered. Complete and review all end of month paperwork in turnovers including Schedule of all lab routines Review, update physician order summary, and sign physician order summary review form. Review medication records for complete list of information, accuracy in transcription of physician orders, and inherence to stop order policies. Review physician orders, transfer/pre-admission admission information etc. for completeness. Perform routine charting as required. Unit managers route task include, but is not limited to: daily morning and evening rounds with nurses Daily review of: Lab results, applicable orders and follow up labs; Order listing reports Review and sign all change in condition assessment from previous day. Update care plan if needed Review and sign all incident reports Complete skin sheets Update care plan for orders as applicable Review consult notes, orders, and need for follow up appointments Review/update open charting list Review/update follow up charting list daily and PRN Review staffing schedule to ensure adequate staffing for the day Review of EMR for missed entry Review of POC/kiosk for missed entries Review “Orders in queue” in PCC to ensure that they have been saved Review and confirm “ Pending Orders ” in PCC Check and link supply messages in Pharmacy Orders in PCC Next day after, review admissions/re-admissions (if applicable) Review by initiating and mission review checklist form Sign physician order review form Weekly review of: Review/update care plans Attend resident care plan meetings Review updated PCC skin sheets Review Narcotic count sheets Review Glucometer quality control record logs Review crash cart log Refrigerator temperature logs checked and signed Review weekly round report from wound doctor/consultant Ensure treatment in medicine carts are clean twice/week and log signed Check the Drug Record Book to see if a record of all orders that have been received. Check GNA skin book for skin issues reported Check GNA report daily to ensure follow up Ensure that utility rooms are clean and clutter free (O2 room, shower room, clean and dirty utility room, attending ‘s room ) Ensure expired medication and medical supplies are disposed of and re-ordered if needed On Friday's check all available 02 small tanks and alert DON if less than 5 tanks on unit On Fridays review staffing schedule to ensure adequate staffing for the weekend Monthly review of: Ensure monthly vitals completed- due by 7 th of each month Ensure monthly summaries completed- due by the 20th of each month Ensure that wheelchairs are washed monthly and log signed-by the 22nd of each month Ensure that refrigerators are cleaned and defrosted monthly and log signed- by the 22 nd of each month POS reviews completed and physician order summary reviewed form signed- due by last day of month Ensure all routine labs for the month completed as ordered Ensure monthly labs turnover is done by the first of the next month Complete staff evaluation Call concern family members Destruction of unused narcotics When new schedule is posted, review to ensure adequate staffing for entire period of posted schedule Supervise and educate Orient new patient/family members as necessary Assist in orientation of newly assigned personnel as well as supervise other nursing personnel assigned to your unit, assuring care is delivered adequately and in accordance with the current applicable federal, state and local guidelines and regulations and our policy and procedure. Make daily assignments for GNA on unit. Complete employee reviews timely. Actively participate in quality improvement process for the facility Make sure labs, x-rays, pacemaker checks and other diagnostic procedures are scheduled done and followed up timely.
