Teach and assist with daily living skills like cooking and light housekeeping
Provide and assist with direct care such as personal care and hygiene
Promote and advocate for the health and wellness of the people supported
Support individuals to become independent, make decisions, and live a meaningful dignified life with valued social roles (ex: friend, neighbor, student, employee)
Provide transportation within the community and to medical appointments as necessary
Support Program Director with administrative and operational tasks including scheduling, training and mentoring staff, fire drills, medication checks, cleanliness of home, doctor appointments, etc.
Provide back-up coverage as needed

The Community Inclusion Specialist (CIS) provides assistance to adults with developmental disabilities. The CIS creates and facilitates daily outings designed to promote community integration, build cultural identity, and connect participants with relevant opportunities to help meet their larger goals within the community. Participants may spend time in the community engaging in anything from cultural activities, museum visits, life skills-based excursions, volunteer opportunities, parks and more. The CIS may accompany up to three people at a time in the community, using public transportation or in your own personal vehicle.

Essential Duties and Functions
• Supervise and oversee participants in the community daily to achieve their personal goals and take part
in activities and/or events.
• Meet the basic needs of our participants while in the community. This may include assistance with other
Activities of Daily Living (ADLs) as required by the participant.
• Implement IP's (Individual Plans) and behavior plans, as needed.
• Maintain sufficient knowledge of participants' IPs and personal needs required to work with them in a
meaningful and productive way.
• Maintain tracking of receipts and expenditures for reconciliation, assist will bill paying and budgeting.
• Complete all required paperwork, including but not limited to case notes at the end of the visit.
• Drive personal vehicles or use public transportation to assist individuals in accessing the community.
Mileage is reimbursable.
• Assist families with obtaining/maintaining basic entitlements and benefits as necessary
• Attends all internal and external meetings on the treatment planning for each client on assigned
caseload.
• Attend basic first aid, when needed
• Assist with and/or provide and meal preparation. light housekeeping
• Document supports given in the electronic visit verification system.
• Maintain annual PMT certification
Qualifications

The role of the Registered Nurse is to provide person-centered nursing care, through the promotion of health and welling being. The Registered nurse participates as team member within the interdisciplinary team (IDT), in the plan of care for consumers served by the agency. The RN is required to practice in accordance with the code of professional conduct as set out by the Connecticut Board of Examiners of Nursing (BOEN), and within the polices, guidelines, established by the Department of Developmental Service.
The Registered Nurse provides health and clinical oversight to individuals with intellectual Disability and comorbid conditions. The RN performs assessments of individuals in their care to actual or potential health problems, participates in the development and implementation of the plan of care, and reviews and evaluates the effectiveness of the plan on a continuing basis. The RN provides clinical oversight / direction and delegates responsibility for performance of certain nursing task/responsibilities to trained non-licensed staff according to the standards identified by the Connecticut Nursing Board and the Department of Developmental Service regulation. The RN implements the plan of care as ordered / executed by the direction of the licensed physician, dentist, Physician Assistance, or APRN.

DIRECT DUTIES:
Perform RN job duties in accordance with RHD policy.
The RN is responsible for the health care oversight of individuals residing in his/her current program assignment.
The RN shall help develop a plan of care in collaboration with assigned individuals and key interdisciplinary team members who support the individual in the assigned programs.
The RN participates as a team member in the multidisciplinary team to achieve a plan of care that is optimal for the consumer, which may include individual specific health recommendations.
The RN participates in scheduling medical appointments, participates in transition, and/or discharge plan of care.
The RN monitors the overall healthcare, promotes health and wellness, as well as prevention of illness. This may include but is not limited to:
Nursing assessment, planning and implementation of individual plan of care.
Individual specific education in the plan of care including medical orders and /or medical recommendations, areas of personal hygiene, key health areas, diet, family life and sex education, as needed.
Acts as a liaison to community medical providers to provide programmatic staff with education and support relating to the medical needs of the individual.
The RN may be required to attend medical appointments.
Develop nursing reports, which may include nurse recommendations in plan of care.
The RN provides health care oversight, which may include the monitoring of medical appointments, orders, and recommendations.
Ensure that relevant documentation of medical visits / follow-ups has been completed.
RN provides timely nursing documentation as per CT DDS regulation.
RN is responsible for direct oversight for the medication administration program for assigned programs.
Direct oversight includes Medication Administration as defined by CT Medication administration policy.
The RN should ensure the implementation of all nursing and medically related policies and guidelines are in place, which may include the implementation of task specific nurse delegation and procedures, to ensure that tasks are delegated safely.
The RN may participate in the development and revision of agency policies.
The RN participates in staff training annually and as determined by program needs. Staff training may include but is not limited to:
Individual Specific Nurse Delegation, Vital signs, Signs and Symptoms of Illness, Protocol and procedure, Infection Control and Individual Specific Medical Information.
RN may be required to function as a medical resource for assigned programs.
The RN acts as a direct liaison between medical providers, interdisciplinary team members and the programmatic staff.
The RN may be required to provide direct medical crisis support and /or support via phone, during regular business hours (8am to 5pm on Monday to Friday) and as assigned.
Ensure proper and effective utilization of emergency services when necessary.
The RN may provide dietary oversight as per Connecticut DDS safe eating and drinking guidelines to ensure proper food and fluid intake.
OTHER RESPONSIBILITIES:
Professional nursing practice.
Regulation set forth by the Department of Developmental Service, which bears direct oversight for homes that are licensed and funded by DDS.
Develop a work schedule, which allows for adequate oversight and supervision of assigned programs.
Comply with RHD policies and procedures.
