Job Description As a Hospice On-Call Registered Nurse, you will: Provide skilled professional nursing care to patient/family/caregivers as prescribed by the physician and in compliance with regulations by the State Board of Registered Nursing and company policies when called from 5:00 pm to 8:00 am Friday evening through Monday morning. The Home Care Visiting RN provides safe, holistic care to patients with multiple comorbidities and advanced chronic illnesses. This role involves educating patients on self-management and providing support to meet the needs of the patient, family, and community. Responsibilities Conduct home visits, including initial assessments, recertifications, and discharges. Manage a caseload of 6 patients per day, with a productivity target of 30 per week. Complete and document visits in a timely manner using Homecare Home Base EMR. Educate patients and families regarding care plans and self-management techniques. Travel within the designated territory, with mileage reimbursed at $0.62 per mile. Required Experience / Certifications / Licensure 1+ years of recent Home Health experience - required.
