The Full-Time HomeHealth Admissions Registered Nurse at KeystoneCare plays a vital role in providing nursing services to patients in need of homehealth care. This position involves assessing the patient's condition and developing individualized care plans in collaboration with the physician. The Admissions RN is responsible for monitoring, documenting, and implementing treatment plans designed to enhance patient comfort, educate patients and families, and promote independence in self-care whenever feasible.

Evaluates patients for appropriateness for homehealth services, ensuring regulatory compliance and agency standards are met.
Obtains treatment authorization and processes patient admissions to the agency.
Conducts comprehensive clinical evaluations to inform care decisions.
Collaborates with physicians to communicate patient needs and secure appropriate care orders.
Initiates and develops the plan of care based on the evaluated needs of patients.

Requirements
Graduate of an accredited school of nursing
Current RN license in good standing to practice in the Commonwealth of Pennsylvania
Proven clinical and professional competence
CPR certified
Valid Driver's License and vehicle insurance in good standing
Access to a reliable vehicle
Ability to follow oral and written instructions accurately
Effective communication skills with all members of the health care team
Strong organizational skills
Proof of COVID-19 Vaccination or an approved Medical or Religious exemption

As a Hospice On-Call Registered Nurse, you will:
Provide skilled professional nursing care to patient/family/caregivers as prescribed by the physician and in compliance with regulations by the State Board of Registered Nursing and company policies when called from 5:00 pm to 8:00 am Friday evening through Monday morning.
After clinical and social staffs report findings and interventions, determine appropriateness and if any further instruction is needed.
Work closely with all members of the Interdisciplinary Team to ensure proper patient follow up on previous night after hour's calls.
Adhere to company policies and procedures and regulatory compliance requirements and directives.
Timely documentation of all after-hours calls and visits including problem and intervention.
Requirements
Current Pennsylvania Registered Nurse license.
Valid driver's license, automobile and auto insurance required.
Available Friday 5 PM until Monday 07:00 AM for on call and visits as neeed
Minimum of one-year general nursing preferred with current hospice, homehealth, medical, surgical or critical care experience
Experience with pain and symptom management interventions
Ability to work within an interdisciplinary setting
Understanding of hospice philosophy, principles of death/dying

Obtains treatment authorization and processes patient admissions to the agency.

Requirements
Graduate of an accredited school of nursing

CPR certified
Valid Driver's License and vehicle insurance in good standing
Access to a reliable vehicle
Ability to follow oral and written instructions accurately
Effective communication skills with all members of the health care team
Strong organizational skills
