The Quality Management (QM) Specialist utilizes knowledge of the Florida child welfare system, critical thinking/assessment skills, and leadership experience, to effectively develop, manage, and perform quality improvement activities to support partners in improving the quality of programs, services, and engagement.
ESSENTIAL FUNCTIONS:
Perform case record(s) reviews, including creating/updating review tools, guides, compiling data roll-ups/written summaries, and communicating results.
Perform review(s) of high-profile cases and write comprehensive reports. Work with case management to address immediate child safety concerns.
Create and lead quality improvement projects, including creating materials/presentations.
Collaborate with other departments to provide guidance, coaching, and training to case management staff & other partners.
Establish/maintain positive partnerships/interactions with the Department of Children and Families (DCF)/Office of Quality Innovation (OQI), case management, and others.
Perform Automated Accountability Review and Response Tool (AARRT) reviews using the Mindshare system.
Participate in writing/updating of policies/procedures, newsletters/reports, annual schedules, presentations, and other activities. Maintain knowledge of contract measures/goals.
Lead/manage assigned area(s). Participate in Council on Accreditation (COA) activities.
Participate in ongoing professional development. Attend and participate in Kids Central events, meetings, and activities.

QUALIFICATIONS:
Education/Experience
Minimum bachelor's degree in social/human services/sciences or related field. At least two years of child welfare experience preferred in Florida dependency case management or Florida child protective investigations. Bilingual (Spanish) applicants, encouraged.
Licensure/Certifications
Certification in Child Welfare preferred. Be willing to achieve Quality Reviewing/Child Welfare certification, if required.
Other Requirements
Must successfully clear Level II Background Screen.
Must possess an acceptable driving record and have reliable transportation.

JOB SUMMARY:
The Administrative Assistant II is responsible for providing administrative and general clerical support to the Initial and Re-Licensing units.
ESSENTIAL FUNCTIONS:
Provides general clerical support, which includes answering and screening telephone calls, notarizing, scanning, sending, emailing, and making copies of licensing and re-licensing packets; opening, date stamping, and routing incoming mail; checking incoming faxes and forwarding to respective recipients; preparing meeting minutes; scheduling meetings.
Preparing ongoing correspondence/handouts/flyers for training, meetings, and special events for foster parents
Assists in the coordination of venues for in-service, pre-service, and special foster parent events to include securing facilities, preparing training binders for P.R.I.D.E. trainings, and creating and maintaining the tracking spreadsheets.
Requests and obtains documentation for licensing and re-licensing files, such as, law enforcement (FDLE), health inspections, references, etc., then delivering and filing said requests.

QUALIFICATIONS:
Education/Experience
High School diploma or General Education Degree (GED) with 6 months to one-year experience required. Associate degree and one-year experience in the human services field preferred.

This position provides basic social services to individuals and/or families participating in the Healthy Start Program in Lake County. The responsibilities include case management for Healthy Start clients and education and ensuring clients receive appropriate referrals and services in compliance with Healthy Start Guidelines. ESSENTIAL FUNCTIONS:
Perform case management services for individuals/families receiving Healthy Start services.
Assist clients in accessing community services and provides information and education to clients about smoking cessation, childbirth education, breastfeeding support, parenting and infant growth and development.
Initiate tracking procedures for client, family of client and client's provider to manage appointments and referrals for services.
Maintain/update client files, tickler cards and ensure Healthy Start database is updated.

The Initial Licensing Specialist is the primary point of contact for licensed foster homes and those in the process of being licensed. The incumbent is also responsible for accurately and in accordance with strict timeframes, completing and submitting initial licensure and re-licensure packets to the Department of Children and Families, along with recommendations for approval or revocation of license. The incumbent will also be responsible for completing adoption home studies and submitting them along with a recommendation for approval or denial.
The Re-Licensing Specialist is the primary point of contact for licensed foster homes and is responsible to accurately and in accordance with strict timeframes, completing and submitting the re-licensure packet to DCF, along with a recommendation for approval or revocation of license. The incumbent is also responsible for investigating and making recommendations for resolving all incident reports, foster care referrals, exit interviews, and Licensing concerns related to existing foster homes.
ESSENTIAL FUNCTIONS - INITIAL LICENSING:
Develop timely and complete licensing packets for prospective new foster homes, which includes attending PRIDE classes upon request to become familiar with the family, conducting a minimum of two home visits to the prospective home for initial assessment; assisting prospective foster parents in completing all licensing and background screening paperwork; evaluating background information; evaluating and assessing all family members, household members and backups/sitters for inclusion in the initial licensure foster home study. The expected timeframe for initial packet submission to the Licensing Supervisor is within 30 days of the family completing PRIDE class, unless an exception is made by the Supervisor or Director.
Screening prospective foster/adoptive parent applicants to include reviewing preliminary paperwork and completing background screening.
May conduct Parent Resources for Information, Development, and Education (PRIDE) or other approved pre-service training to prospective foster/adoptive parents in Citrus, Hernando, Lake, Marion, and Sumter counties.
Assist the Kids Central Placement Department and Family Care Managers in determining appropriate foster parent and foster child matches.
Back-up the Placement Specialist in the actual placement of children during occasional weekend coverage.
ESSENTIAL FUNCTIONS - RE-LICENSING:
Create re-licensing packet for submission to DCF, which includes: one home visit to assist foster parents in completing paperwork, assess and evaluate all household members and temporary alternate caregivers/sitters for inclusion in the home study, complete background screening process for new members of the household
Conduct minimum of quarterly home visits to all foster homes in the caseload
(approximately 30 homes)
Conduct an ongoing assessment to ensure a family's skills, training, household composition, etc., are suitable for their licensed capacity, and complete paperwork, as necessary, to increase or decrease capacity

This position provides basic social services to individuals and/or families participating in the Healthy Start Program in Lake County. ESSENTIAL FUNCTIONS:
Perform case management services for individuals/families receiving Healthy Start services.
Assist clients in accessing community services and provides information and education to clients about smoking cessation, childbirth education, breastfeeding support, parenting and infant growth and development.
Initiate tracking procedures for client, family of client and client's provider to manage appointments and referrals for services.
Maintain/update client files, tickler cards and ensure Healthy Start database is updated.
Assess the educational needs of Healthy Start clients who are at risk for poor pregnancy outcomes.
Assist with providing office coverage and be available to receive any phone calls or assist any visitors to the Healthy Start Office during required office hours.
Complete all Healthy Start assessments and home visits as required per program guidelines.

The Post Adoption Coordinator is available to adoptive parents in Circuit 5 (who have finalized the adoption) in need of guidance, information, and advocacy after the case is closed by the adoption agency. Proactively, the Post Adoption Coordinator works to resolve issues before they escalate whenever possible. The coordinator advocates on behalf of a family when needed, providing linkages, referrals, support, and monitoring while responding with a sense of urgency. Identifies systemic gaps and barriers and makes recommendations for solutions and/or improvement to the Assistant Director of Out of Home Care and the Senior Director of Out of Home Care.
ESSENTIAL FUNCTIONS:
• Create and send a Newsletter to post-adoption families to include educational opportunities, summer camps, tutoring, and other services available within Circuit 5 and statewide. The Newsletter is to be created and distributed semiannually and should feature at least two (2) post-adoptive families within Circuit 5.
• Send meeting notices to all adoptive families and facilitate support groups in each county with an Adoption Agency Specialist to provide support, networking opportunities, and education related to adoption issues.
• Responsible for securing the location and childcare services for the Adoption Support Group.
• Facilitate the Adoption Support Group monthly for each county and document the meeting in FSFN within 48 hours of occurrence.
• Contact all adoptive families one year after their anniversary. The families will be offered services and address any issues and make the appropriate referrals as deemed necessary.
• Develop a tracker to ensure all the Post Adoption Families are contacted as outlined in House Bill 7013.
• Ensure the report for the children finalized quarterly is reviewed in advance in anticipation of the projected workload.
• Make contact regarding services offered as part of the 1 (one) year post-adoption communication program for adoptive families that reside outside the State of Florida and outside Circuit 5.
• Coordinate with the Children's Mental Health Specialist and/or the Kids Central Behavioral Specialist regarding the adopted child's mental health needs and discuss all recommendations.
• Consult and work with the Adoption Specialist, Eligibility, and other departments as needed to assist families with Medicaid, subsidy, social security, and other questions
• Follows up on all inquiries within 24 business hours of receipt and ALL home visits will be discussed with the Assistant Director of Out of Home Care.
• Review all open cases and make referrals to the appropriate services as deemed appropriate for case management services.
• Establishes and maintains linkages with businesses, organizations, groups, vendors, and individuals within the community whose support and/or assistance will add value to the agency.
• Collaborate with the Protective Investigator when the investigation involves an adoptive family.
• Request services for families outside the State of Florida and contact the state agency in which the family resides to notify them that the family needs post-adoption support and services.
• Develop a tracker to submit the following information every quarter: 1) The number of attempts made to each family; 2) The number of successful attempts to contact each family; 3) Services Requested by each family 4) Services provided to each family 5) If services were not provided to the family the reasons why they were not provided; 6) Any recommendations in the improvement in the communication of the post adoption program.
• Provides Community Education in conjunction with the Adoption Specialist.
• Provide a log of families receiving Post-Adoptive Services for the monthly client satisfaction survey.
• Tracks and reports on all children with disrupted adoptions, families served in support groups, as well as families served through general inquiries.
