We're looking for aHospice RN - Regional Support to join our team. In this dynamic role, you'll provide direct care to patients and support to families, while also serving as a floating clinician-temporarily assigned to different hospice branches based on census needs or staffing gaps. If you're energized by new settings, adaptable to change, and passionate about purpose-driven care, this role could be a perfect fit.
As a Hospice RN - Regional Support, You Will:
• Deliver high-quality, compassionate nursing care for hospice patients
• Manage and implement individualized plans of care
• Support and lead interdisciplinary team members (LPNs, CNAs, HHAs, etc.)
• Assess and address patients' physical, emotional, spiritual, and environmental needs
• Accurately document all care provided and ensure compliance with hospice protocols
• Educate families and caregivers on care instructions, medications, and safety
• Participate in the on-call rotation and provide after-hours care as needed
• Collaborate with physicians, referral sources, and the care team
• Assist with referrals, regulatory documentation, and payer compliance
• Travel to various Gentiva hospice locations as assigned to fill urgent needs
Schedule & Travel:
This role typically follows a Monday-Friday schedule, with occasional on-call responsibilities. Travel is required and may include:
• Option 1: Overnight travel required to cities within a 100-mile radius.
• Option 2: Frequent overnight travel to multiple states is required.
• Option 3: Regional travel with overnight stays based on assignment needs.
A company fleet car is provided for eligible employees.
About You:
Qualifications - What You'll Bring:
• Current, unencumbered RN license in the state of employment
• Minimum of 1 year of RN experience (hospice, home health, acute care, long-term care, oncology or other high acuity setting)
• Current CPR certification
• Valid driver's license, auto insurance, and reliable transportation

Key Responsibilities:
Process new referrals in the manor best fitting the needs of the referral source and in alignment with pending management and intake processes.
Offers and/or confirms a same day appointment and/or the appointment time that is as close as possible based on the request of the patient and/or referral source to ensure a timely admission.
Participates in morning check in meetings and activities as assigned, to review staffing/scheduling needs for the day.
Collaborate with Director of Admissions to ensure adequate, available resources are present in the admission scheduling tool throughout the work week.
Provides a concierge level of customer service in every interaction.
Cisco Phone System - must be logged in and “on ready” throughout the work shift, taking inbound calls whenever necessary.
Strong, professional communication and collaboration with Program Admissions, Care Connection Center Intake, and Sales teams for timely dispatching of all Admission visit resources.
Quality - Calls (audio/video) are recorded, monitored, and reviewed for training, quality and performance feedback and recognition purposes.
Supports Admissions team to exceed program goals.
· Establishes a rapport and maintains an effective relationship with all internal and external customers.
Enters accurate information to support data integrity in computer applications.
Responds accurately to all questions received regarding services provided.
Escalates matters to Director of Admissions or Admissions clinical staff when assistance or clarification is needed.
Participates in ongoing training and development based on the changing healthcare environment.
Notifies Admissions / Sales Staff of all scheduled appointments per standards.
Utilizes Scheduling tools to effectively facilitate the scheduling/updating of appointments.

Responsibilities
The regional program director represents the region in activities involving professional contacts with physicians, hospitals, public health agencies, associations, executive level opportunities, and similar health groups and institutions, to apprise them of the availability of Medicare services.

Responsibilities
Enhabit Home Health & Hospice is searching for a Chaplain with hospice experience to join our team. The Chaplain is a member of the interdisciplinary team and reports to the Hospice Branch Director.
Providing direct spiritual support and/or counsel to patients/families in keeping with patients/families' beliefs.
Working with staff, clergy and community groups to enhance their sensitivity to the spiritual needs of patients/families and reporting on services as indicated.
Providing consultation and education for patients/families and interdisciplinary team members.
Providing bereavement follow-up services as defined by Hospice.
Maintaining proper records of visits to patients/families.
Performing occasional liturgical assignments such as the annual Service of Remembrance and monthly memorial service when indicated.
Qualifications
Education and Experience (ESSENTIAL):
Must be an individual who, by ordination or by ecclesiastical endorsement from the individual's denomination, has been approved to function in a pastoral capacity.
Must have demonstrated experience in working with patients and families dealing with life-threatening illness and death.
Education and Experience (DESIRED):
Training in clinical pastoral education, meeting the requirements for the college of chaplains, may be considered in lieu of ordination or endorsement.
Qualifications:
Must be able to effectively communicate, both orally and in writing.
