This position manages the full admissions process, collaborating with referral sources, physicians, case managers, and families to ensure each patient's transition to hospice care is handled with urgency, accuracy, and empathy. You will communicate by phone and in the field, coordinating services, verifying insurance, and ensuring documentation compliance-all while supporting the mission to provide high-quality, patient-centered care. Key Responsibilities + Serve as the primary contact for new referrals and coordinate all aspects of patient admission within the company's 3-hour standard. + Communicate promptly with patients, families, referral sources, and internal clinical staff to ensure a seamless admission experience. + Obtain and verify insurance information, complete payer source verification, and assist billing staff to ensure accurate and timely reimbursement. + Participate in daily team meetings, marketing initiatives, and community education events to strengthen relationships with referral sources. + Maintain accurate admission and referral data in the company's information system. + Ensure compliance with all federal, state, and Medicare regulations, as well as company policies and procedures. + Support after-hours and weekend admissions, as needed, to meet patient and family needs. About You Qualifications + High school diploma or equivalent required. + Experience in healthcare admissions, hospice, or medical office administration preferred. + LPN/LVN certification or Bachelor's degree in business, social work, psychology, or nursing preferred. + Strong organizational, communication, and customer service skills. + Knowledge of hospice services, Medicare guidelines, and community resources beneficial. Qualifications * License or Certification: * Must be qualified to independently complete an assessment within the scope of practice of his/her discipline (for example, RN, SW, OT, PT, ST, and Rehabilitation Counseling). * If licensure is required for one's discipline within the state, individual must hold an active license. * Must meet eligibility requirements for CCM or ACM certification upon entry into this position OR within two years of entry into the position. * CCM or ACM certification required OR must be obtained within two years of being placed in the Case Manager II position. * Minimum Qualifications: * For Nursing, must possess minimum of an Associate Degree in Nursing, RN licensure with BSN preferred. A diploma is acceptable only in those states whose minimum requirement for licensure or certification is a diploma rather than an associate degree. * For all other eligible licensed or certified health care professionals, must possess a minimum of a bachelor's degree and graduate degree is preferred. * 2 years of rehabilitation experience preferred. Experience * Prior medical/surgical and/or emergency/trauma experience and corrections experience is preferred. * Must have the patience and tact to deal with the inmate patient population and ability to work effectively in the corrections environment. * Scheduling flexibility is also required to be able to rotate hours and shifts, if needed, and to be called during emergency situations to provide coverage. Licenses/Certifications * Must have and maintain current licensure as a Nurse Practitioner within the state of employment. * Must have and maintain current CPR certification.
