Responsibilities
Carry out assigned tasks on clients and follow Home Health Aide Care Plans. Perform delegated tasks after instruction by RN or PT.
Bathe clients, assist with showers and partial baths as appropriate.
Assist clients with proper exercises and assist PT in the rehabilitation for client when applicable.
Help client maintain personal hygiene and assist with all aspects of activities of daily living.

* The general manager assures policies in the following manner:

* Based on the corporate business plan and revenue budgets, the general manager develops and maintains key relationships with hospitals, nursing homes, physicians and other healthcare organizations.
* The general manager manages the implementation phase of contracts to assure that discharge planning and case review functions are understood and agreed to by contracting institutions and implemented with positive results to the hospice, according to applicable state and federal law.
* The general manager establishes collegial, on-going relationships with the Executive Management of contracting entities, including those who have line authority over discharge planning personnel to promote the smooth and efficient functioning of such contracts.
* The general manager has specific responsibility for hospice financial functions including management of monthly profit and loss statements, accounts receivables, collections and local accounts payable processes.
* Based on a plan developed with the director of market development and program senior leadership team, the general manager develops and maintains relationships with physicians.
* The general manager initiates new relationships by educating physicians about the potential hospice need in their practices.
* The general manager improves the value of existing relationships by ongoing contact with selected physicians and their office staff - including the resolution of complaints, the providing of educational services and seeking to improve and refine patient care services.
* The general manager supervises the admissions function through the admissions manager.

Each day is different. You will spend most of your time caring for hospice patients wherever they call home, whether that means a private residence or an assisted living/skilled nursing facility. Most VITAS nurses make 4-6 visits each day.
Once a week, you'll visit the local program office to meet with an interdisciplinary team, including social workers, physicians, chaplains, and nurses, who collaborate to provide holistic support for the patients and families we serve.

Responsibilities
* Performs initial nursing evaluation, including behavioral health/psychiatric nursing needs.
* Observes and evaluates the physical and mental status of the patient for complications and medication side effects.
* Maintains awareness for indicators in patients' behavior that call for change in the treatment modality (crisis, suicidal ideation) as well as other medical signs and symptoms.
* Instructs patient/caregiver in cognitive and technical aspects of care, medication regimes, disease processes, safety interventions, ADL care, coping skills, symptom management and other interventions.
* Administers IM medications, performs lab draws and specimen collections.
* Acts as a case manager in coordinating care for patients as assigned.
* Completes documentation timely and accurately.
* Supervises LPNs and HHAs.
* Regularly communicate patient progress to the clinical manager, psychiatrist and care team.
Qualifications
* Current RN license, specific to the state(s) you are assigned to work. Must have one of the following to meet CMS requirements for psychiatric nursing in the home care setting:
* An RN with a master's degree in a mental health nursing or psychiatric specialty. Must have nursing experience in an acute treatment unit or other psychiatric setting.
* An RN with a bachelor's degree in nursing. Must have one year of recent nursing experience in an acute treatment unit or other psychiatric setting.
* An RN with a diploma or associate's degree in nursing. Must have two years of recent nursing experience in an acute treatment unit or other psychiatric setting.
Please note: Experience must have been within the last three to five years. If not, documentation must support psychiatric retraining or classes or CEUs to update psychiatric knowledge.
* Current CPR certification.
* Valid driver's license, reliable transportation and liability insurance.

Graduate from a Licensed Practical Nursing program.
Currently licensed as a Practical Nurse in the state of employment.
Possesses an active CPR certification.
Remains knowledgeable about specific state laws and regulations governing practice.

Responsibilities

Contributes to the assessment of the health status of patients.
Participates in the development and modification of a patient's plan of care.
Performs delegated tasks, activities and functions that are consistent with prior educational preparation and within scope of practice.
Provides direct basic care to assign multiple clients within the structure of the healthcare program at their facility.
Provides safe and effective nursing care and assumes responsibility for the care delivered.
Implements patient education based on established teaching plans.
Utilizes a problem solving approach as the basis for their decision making.
Monitor offsite and onsite scheduling with a collaborative approach.
Monitor consult process as it relates to policy and contract.
Participates in Quality Improvement.
Participate in Patient Care Committee Meeting
Monitor CARES- with coordination as needed
Ensure onsite and offisite follow up care of patients along with telemedicine appointments
Will work in collaboration with Regional Medical Director, site medical providers, Regional Nursing Director and other site nursing leaders.
Performs other nursing duties as assigned.
