Reporting directly to the Executive Director or Administrator, you will oversee patient care activities to ensure the delivery of high-quality, compliant hospice services. This leadership role involves managing clinical operations, coordinating care, and developing staff to uphold the highest standards of patient care and regulatory compliance.
As a Hospice Patient Care Manager, You Will:
+ O versee and direct patient care services to ensure the delivery of high-quality hospice care
+ M anage visit scheduling, including reassigned, missed, declined, or rescheduled visits-making decisions based on clinical judgment and priority
+ T riage incoming phone referrals and medical concerns in a timely, compassionate, and professional manner
+ L ead or facilitate Interdisciplinary Group (IDG) meetings, including presenting patient updates, reviewing charts, coordinating care plans, and documenting minutes
+ S erve as a mentor, trainer, and clinical support to associates, supporting development, performance, and compliance
+ E nsure all care delivery meets federal, state, and local regulatory standards, as well as company policies and procedures
+ S upport performance improvement initiatives, quality assessments, and operational efficiencies within your location
About You
Qualifications - What You'll Bring:
+ Current RN license in the state of employment
+ Graduate of an accredited nursing program
+ Minimum of 3 years of direct patient care experience, including 2 years in a hospice or home-based care setting
+ Demonstrated ability to apply hospice principles, industry standards, and regulatory compliance (Medicare, Medicaid, JCAHO, ACHC)
+ Knowledge and compassion for terminally ill patients and their families
+ Ability to lead a diverse care team while adapting to the emotional and clinical needs of patients from various backgrounds
+ Strong interpersonal, organizational, and leadership skills
+ Competency in clinical documentation and technology systems
Preferred Qualifications (Not Required):
+ Bachelor of Science in Nursing (BSN)
+ Previous leadership, team management, or supervisory experience in hospice or home health
+ Familiarity with EMR systems and telehealth platforms
+ Certification in Hospice and Palliative Nursing (CHPN)

The CaseManager maintains a direct relationship with the patient and his/her family. Responsible for ensuring we effectively utilize treatment resources while working to improve patient outcomes. Assumes the responsibility for caring for a group of complex patients requiring an initial assessment and ongoing monitoring of their condition. Nursing care reflects independent assessment, planning, implementation, and evaluation of the patient's physical, psychological, and sociocultural needs. In collaboration with members of a health care team, develops and monitors the patient's clinical pathway depending on patient's progress. Works closely with physicians and collaborates with nursing and ancillary staff in implementing the patient's plan of care. Works with Quality Analysis and support services to actively audit the outcome of the caseload.
Position Specific Responsibilities
* Performs initial assessment that assists in development and coordination of plan of care. Establishes a system for coordinating a patient case load throughout home care. Assists and maintains method for tracking patients' progress.
* Articulates role of CaseManager to patient and/or patient's family, physicians and all others involved in the assigned patient's care.
* Reviews aggregate variance and determines a path of action.
* Precepts new employees as assigned.
* Assists in the development, review and evaluation of plan of care.
* Explores strategies to reduce length of stays and resource consumption within the casemanaged populations, implements them, monitors and evaluates the results.
* Works collaboratively with staff.
* Seeks consultation with agency leadership about cases that are presenting problems or have significant variances. Arranges for the continuity of the plan and provides for coverage during long, short, and unexpected absences.

Education and/or Experience

Position Specific Responsibilities
Adheres to professional standards of community and Home Health Care nursing practice and works with the physician and Interdisciplinary Group to plan care.
Develops, maintains and implements written nursing plan of care for the assigned caseload of patients/families which address identified needs and provide continuity of care.
Assists other team members or volunteers in planning care to meet the physical, emotional, spiritual and other needs of the patient and family.
Provides nursing care for the patient/family through the assessment, evaluation, planning and counseling of patients/families.
Teaches, supervises, and evaluates care provided by volunteers, home health aids and primary caregivers.
Re-evaluates the needs of the patient/family at regularly scheduled intervals and modifies the plan of care in accordance with patient/family changing needs, and input from the team members which is consistent with protocol and suggested orders.
Consults with the patient's physician when necessary. Updates the physician on a regularly scheduled basis regarding patients condition and care needs.
Provides a written summary of patient care when a patient is admitted to an extended or acute care facility to ensure the provision of continuity of needed care.
Demonstrates education and preparation of the patient/family on dying process as evidenced by observation and/or documentation.
Establishes and maintains appropriate records of patient care management as required by New Life's policy and guidelines and that adhere to the standard of nursing practice.
Serves as a consultant to other professionals or volunteers as requested or as need as identified.
Coordinates and facilitates patient care by utilizing existing agencies/services when appropriate.
Serves as a resource to other nurses in planning or presenting patient care conferences.
Plans and participates in in-services and continuing education programs.
Actively participates, contributes and attends required meetings.
Identifies personal and educational needs via workshops, seminars, self-study, etc. and keeps current professionally. Maintains professional licenses, which are required. Identifies educational needs and ensures that knowledge base is present before proceeding with implementation of care.
Assists with the development, implementation and evaluation of Performance Improvement indicators relative to the assigned programs. Responsible for addressing indices out of compliance.

About the Role
The Afterhours Care Manager RN is responsible for assessing and coordinating patient and family needs during after-hours shifts and maintaining primary clinical oversight for assigned patients. This includes routine visits, admissions, death visits, urgent after-hours calls, and any visit assigned per the patient's plan of care. This role serves as an active member of the Interdisciplinary Group (IDG) to ensure coordination between physicians, families, and the hospice team. Clinical administrative responsibilities, workflow auditing, and survey readiness support are also part of this role.
Essential Functions
+ Ensure appropriate delivery of hospice services to eligible patients and families.
+ Provide direct in-person patient care during on-call, weekend, and after-hours shifts, including assessments, admissions, routine visits, death visits, and live discharges.
+ Conduct initial and ongoing assessments of physical, emotional, psychosocial, and spiritual needs.
+ Support the IDG process through chart reviews, plan of care updates, and interdisciplinary communication.
+ Develop and revise the Hospice Plan of Care in collaboration with physicians and IDG members.
+ Educate patients and families on disease progression, symptom management, medications, and hospice philosophy.
+ Communicate significant changes in patient condition promptly to hospice and attending physicians.
+ Maintain accurate, timely clinical documentation including assessments, care plans, and notes.
+ Assist with rescheduled, missed, or reassigned visit requests based on triage direction.
+ Participate in calendar and scheduling modifications, including service codes and clinician assignments.
+ Support administrative and compliance activities including workflow audits, survey readiness tasks, and documentation collection.
+ Assist with TPE reviews, QAPI efforts, and regulatory compliance.
+ Ensure infection control standards and universal precautions are consistently followed.
+ Provide leadership and guidance to field staff; conduct supervisory visits when appropriate.
+ Participate in required meetings, after-hours updates, and trainings.
+ Maintain proficiency in pain and symptom management for hospice and end-of-life care.
+ Promote the organization's core values and maintain a culture of compassion, professionalism, and teamwork.
+ Perform other duties as assigned.
About You
Education and Experience
+ Current RN license in the state of employment
+ Graduate of an accredited nursing program
+ Minimum one year of RN experience in acute care, oncology, long-term care, hospice, or home health
+ Must meet any state-specific requirements
Licenses and Certifications
+ Active RN license
+ CPR certification (in-person)
+ Valid driver's license, reliable transportation, and current auto insurance
Skills and Competencies
+ Strong knowledge of hospice principles, Medicare/Medicaid regulations, and best practices
