Title: Care Coordinator
Department: Programs
Reports To: Crossroads Resource Center Manager
Job Classification: Non-Exempt, Full Time
The Care Coordinator is responsible for ensuring the care of guests at KARM. The care coordinator will also be responsible for knowing and connecting guests to relevant health providers and resources to work towards stability. Physical, mental, and spiritual resources are critical in this role. An ability to continually assess and create a safe environment that is conducive to change and that assures individual commitment to personal change. This position also will ensure the development of individualized plans for each participant; facilitate ongoing plan changes that support individual progress, needs, and challenges. In addition, the ability to lead, guide, encourage, and support volunteers who will work closely with KARM guests as mentors, teachers, community resource specialists, and post-graduation support. Finally, the ability to create and sustain a team environment, where God is the center and interdependence is valued.
Responsibilities
Develop relationships with guests that promote and model a well-balanced Christian life
Ensure the development of individualized plans for each KARM guest
Facilitate ongoing plan changes that support individual progress, needs, and challenges
Meet with guests as necessary to ensure maximum benefit for the guest
Make appropriate internal and external referrals for services
Be able to apply the correct resource to the need at hand
Ensure consistency in applying KARM safety guidelines, policies, and procedures that impact guest activities and decisions
Select, coach, and develop volunteer leaders in key support areas (mentoring, aftercare, prayer, etc.)
Ability to lead, guide, encourage, and support volunteers who will work closely with guests
Ensure the maintenance of accurate, up-to-date file records for guests in vendor software
Exhibit a caring Christian lifestyle to guests, staff, volunteers, and donors
Work cooperatively with staff and departments to ensure an effective and efficient guest environment
Coordinate work processes with other departments as needed
Advocate on behalf of guests
Ensure the consistent application of program standards and guidelines, curriculum, and discipleship models
Ability to continually assess and create a safe environment that is conducive to change and that assures individual commitment to personal change

Requirements
Bachelor's in a related field (psychology, social work, counseling, ministry) or equivalent education and experience
Master's level education is highly desirable
Two years working with the homeless and/or those with substance abuse and/or mental health challenges preferred
Ability to clearly explain and present the Christian plan of salvation and lead a person through the process
Demonstrated ability to establish a feeling of trust, safety, consistency, and hope
Experience with and skilled in leading and coaching others; experience working with volunteers a plus
Ability to set and monitor goals for and with individuals
Ability to communicate effectively with individuals dealing with various forms of trauma
Demonstrated skills in individual and group counseling
Ability to maintain organized records
Ensure patient confidentiality with the information a guest shares during appointments
Ability to effectively adapt to change
Good organizational and administrative skills
Ability to manage a busy work calendar
Ability to build personal relationships and maintain appropriate boundaries with residents

The Wounded Warrior Project (WWP) Complex Case Coordination (C3) Liaison responds to crisis referrals of warriors, performs initial assessments, guides warriors towards appropriate internal WWP resources or arranges external resources, acts as a case manager for treatment planning and stabilization, and eliminates barriers to care as appropriate. The Complex Case Coordination (C3) Liaison serves as an advocate to ensure rapid, coordinated, and collaborative response to warriors with difficult, complex, urgent issues that exceed the current capability or scope of any specific WWP program. The liaison coordinates and advocates with local resources, Federal resources (i.e., Veterans Administration, Dept of Defense, etc.), Veteran Service Organizations, and private facilities to establish an effective and comprehensive plan for the Warrior.
DUTIES & RESPONSIBILITIES
Caseload management:
* Serve as subject-matter expert and resource in the planning, coordination, and execution of and participation in mental health programs, enhancing the quality of warriors' resiliency and psychological well-being.
* Maintain a caseload of assigned warriors.
* Perform comprehensive screenings and conduct needs assessments to identify and coordinate appropriate external care for warriors dealing with complex mental and physical health issues requiring immediate intervention.
* Partner closely with warriors' care teams to develop and implement treatment plans aligned with the warrior's goals.
* Develop and encourage a collaborative relationship with warriors to maximize the warrior's ability to make informed decisions, utilizing a warrior-centric model, within organizational parameters and serve as the WWP point of contact, following up with warriors throughout their care, treatment, and recovery.
* Collect and maintain sensitive protected health information according to organizational standards as required to coordinate warrior care.
* Facilitate admission and discharge planning by coordinating with available internal and external resources.
* Coordinate with warriors' families and advocates, as appropriate, to ensure necessary treatment, post discharge services and resources are provided.
* Ensure case documentation, service provided, and data are maintained accurately, timely, and appropriately within WWP internal databases.
* Provide transition support with programmatic referrals to WWP programs.
Program management and development:
* Coordinate comprehensive support and care for warriors by orchestrating involvement in one or multiple internal WWP programs or external organizations, such as the Veteran's Administration (VA), Community Care Network Providers, and social services agencies.
* Collect necessary documentation to facilitate payment for WWP-funded care and manage additional invoices needed to request payments on behalf of warriors and their households, in accordance with WWP guidelines.
* Input data into relevant WWP electronic tracking systems, including warrior, family member, spouse, VA points of contact, and/or caregiver databases such as Salesforce, ensuring accurate and timely updates for each case.
* Empower warriors for lasting recovery by mitigating the challenges that precipitated the crisis while removing barriers to care to enable the warrior to receive needed assistance.
* Develop a comprehensive understanding of external resources, treatment options, mental health diagnosis, medical needs, and treatment programs.
* Provide feedback to Program leadership on process improvement and system needs.
* Identify and escalate issues impacting Warrior care to leadership, as necessary.
Resource management and development:
* Cultivate strong and productive partnerships with internal programs, external governmental agencies, private facilities, community resources, and Veteran Service Organizations to ensure effective collaboration and support for program initiatives.
* Work with Department of Defense (DoD), Veterans Benefits Administration (VBA) and Veterans Health Administration (VHA) to coordinate professional services and care for C3 Warriors.
* Prepare briefings and reports including compiling, organizing, and updating status reports regularly.
* Brief internal and external agencies on WWP C3 Program and referral process.
* Attend and actively participate in required training and/or meetings, including but not limited to New Teammate Orientation, WWP Cares, ASIST Suicide Prevention training, Leadership training, culture/team based training, or departmental huddles.
* Other related duties as assigned.

The Wounded Warrior Project (WWP) Fundraising Events Manager is responsible for driving revenue growth by leading and managing the Carry Forward 5K team. This role involves implementing best practices and strategies to build strong relationships with event hosts, segment/channel leads, sponsors, and donors. This position provides expertise, technical leadership, and support to the fundraising team to maximize WWP's return on investment. * Drive the growth of gross and net revenues for the signature Carry Forward 5K city series, virtual and corporate team programs.
* Increase participation, retention, and fundraising across the city series and virtual Carry Forward events.

Work alongside the other Truck Drivers and Helpers, daily, in the building of the team, by being an example of revealing Christ to all staff and donors, ensuring a Christ-centered environment.
Drive a straight box truck throughout the work day (automatic transmission, 16 ft or 26 ft, under 26,000GVW) while being responsible for observing all traffic laws, in order to safely distribute product from the donor's home to one of the KARM Stores.
Works with a Donation Truck Helper to load and unload furniture and other donated items on and off a straight box truck, with careful attention, to insure that: people are not injured, KTS property and the donor's home or business is not damaged, donated items are not damaged, and space in the truck is properly utilized.
Treats all donors and their homes or offices with the utmost level of respect during donation pick ups as well as effectively communicates gratitude to all donors, on behalf of KARM Stores.
Works with their assigned Donation Truck Helper to safely operate the truck lift gate and other furniture-moving equipment (2-wheel dolly, 4-wheel dolly, tie-down straps, other) as needed during the workday to help provide safety and teamwork.

The Wounded Warrior Project (WWP) Physical Health & Wellness (PH&W) Screening Specialist recruits and screens warriors for multi-week programs and events that educate, coach, and empower wounded service members in pursuit of achievement of their physical health and wellness and/or adaptive sports goals. The PH&W Screening Specialist provides warriors with comprehensive screening, needs assessment, and coordination of internal and external resources to support warriors and enhance their well-being.
DUTIES & RESPONSIBILITIES
* Identify, screen, and recruit warriors for participation in the virtual and in-person PH&W Coaching Program, Soldier Ride, and Adaptive Sports Program. The PH&W Program provides fitness, nutrition, mobility, sleep, and health-focused education to warriors. The Soldier Ride program provides all-ability events that include single and multi-day rides of varying modalities with adaptive and standard equipment. Job Details
Assist the Retail Management team in register transactions including handling of cash and credit card payments and any non-cash purchases, in order to ensure the accuracy and security of all transactions.
Work with the Retail Management team to establish and maintain customer service standards in order to ensure compliance with the company mission statement and to provide a pleasant shopping experience for each customer.
Assist store staff, in stocking store shelves and clothing racks, in order to provide customers with maximum shopping opportunities.
Assist the Retail Management team in the building of the team, by being an example of “revealing Christ” to all staff, donors, and customers.

Summary
The Director of Facilities oversees the training, performance, and operation of their assigned departments, including Facilities and Maintenance and Safety and Security. The Director will handle all aspects of property management, including, but not limited to, facilities upkeep and improvement, relationship management, department integration, and ensuring service, safety, and compliance to all KARM Store locations.
Essential Functions of the Job
Manage the external message and mission of “revealing Christ”.
Allocate any internal or external resources to the proper project/department.
Strategize high-level ways to improve the effectiveness and cohesion of their departments.
Build and/or manage a team of internal team members, as well as potential external contractors.
Maintain effective communication with Operations Directors to ensure that their facilities' needs are being met.
Facilities and Maintenance
Oversees the operation of the Facilities and Maintenance Manager, as well as their team.
Ensure that the Facilities and Maintenance Manager is appropriately prioritizing and completing all service requests submitted to their team,
Ensure that any relationships with vendors, outside contractors, etc. are properly maintained by the Facilities and Maintenance Manager.
Property
Maintain strong relationships with landlords and property management companies.
Have an understanding of lease agreements and any associated obligations.
Manage property-related improvements and compliance issues as outlined within lease agreements.
Maintain an understanding of applicable city, county, and local ordinances related to building, zoning, and code requirements.
Serve as the primary point of contact for property-related concerns across all KARM Store locations.
Safety and Security
Oversees the operation of the Safety and Security Manager to support the ongoing development and execution of safety and security standards across all KARM Store locations.
Ensure that incident response protocols, safety training, and compliance expectations are consistently followed and aligned with organizational standards.
Collaborate with Safety and Security to address trends, reduce risk, and improve prevention efforts across facilities.
Support investigation and follow-up on significant safety or security related concerns.

DUTIES & RESPONSIBILITIES
* Develop and evolve the WWP enterprise architecture framework across business, data, application, and technology domains, ensuring alignment with organizational priorities and scalability for future growth.
* Communicate the strategic vision and direction for WWP enterprise IT systems, ensuring scalability, innovation, and alignment with mission-driven goals.
* Partner with executive leadership to align IT strategy with organizational goals.
* Lead the development of enterprise architecture governance practices and present architecture proposals to governance forums, ensuring compliance with established policies, review boards, and technical standards.

Preferences
