The Medical Records position is responsible for maintaining, incorporating, and accurately accounting for all medical records information on each individual resident.
PRIMARY DUTIES AND RESPONSIBILITIES

Responsible for accessing, maintaining and auditing all medical charts.
Closes and files discharged residents' medical charts. Reviews charts of discharged residents for the deficient information and ensures compliance for correction of deficient documentation.
Scan documents and file them in a resident's medical chart in a systematic order. Ensure that each section of the resident's record is maintained in a neat, organized and readable format. May require analysis of appropriate medical records to be attached to a particular quality assessment electronic tools.
Provide Documents and guidance for all assessments.
Creates schedules for assessments within the medical record. Prepares assessments for completion and appropriately correlates assessments to the electronic schedule.
Ensures that Physician's orders, telephone orders and other records requiring signatures are received and filed within the required time frame.
Maintains effective communication with others, both inside and outside the department, to give or obtain needed information or coordinate activities of residents (e.g., doctors' appointments).
Ensures that all residents are seen by their physicians, and routine lab work is completed and filed in the required time frames.
Ensures personal and staff compliance with all State and Federal regulations including blood-borne pathogens, infection control, use of hazardous materials, and fire safety.
Ensure personal and staff compliance to Residents' Rights.
In-Service training, and assigned QA meetings. May be assigned to chair or make presentations at these events.
May be called upon to respond to an admission inquiry; to host a prospective resident or family member; or to visit a prospective resident at home or in the hospital.
Is responsible for knowing the relevant State and Federal regulations and laws; especially as related to the provision of resident care and documentation.

Do rounds on your unit at least twice a day to ensure compliance, ensure the corrections are made where necessary.
Follow up on change in condition on residents
Ensure follow up that you assigned from the previous day was completed
Ensure staffing is posted on board for 7-3 then again for 3-11
Monitor dining rooms to ensure resident are serviced correct diet and are consuming meals.

Provide weekly updates to manage care case managers
Complete and have profile form and 672 completed and turned into DON, by Thursday pm. Please provide MDS, Wound care, Weekend supervisor, Assistant Administrator, RD, Risk manager and Soc. Service with copies of profile and 672.

Ensure nurses are updating C.N.A care plan.
Weekly weights need to be completed and recorded on Mondays. Weekly weights of 5 or more loss or gain a reweight needs to be done immediately. Review your weights and submit a copy to the DON and RD. Address any weight changes.
Review MAR's and TAR's for omissions.
Do random audits of charts to ensure documentation, evaluations / assessments are completed. 24 hour chart checks are being done.
Address any staff concerns as needed.
Check with Staffing Coordinator to ensure your unit is covered with the correct amount of staff.
Ensure monthly assessments and Resident data set are completed as assigned.
Complete staff evaluations for the month and submit to the Director of Nursing.
Complete consultant Pharmacy report and submit to Director of Nursing.

GENERAL PURPOSE OF JOB.
The LPN Charge Nurse is responsible for providing direct nursing care to the residents in an assigned unit under the direction of the shift supervisor and supervise the day-to-day nursing activities performed by the nursing assistants.

Observe and report symptoms and conditions of residents/patients
Administer treatments and medications as prescribed by a physician
Maintain records reflecting resident condition and treatment including nurses' notes, discharge plans, resident assessments, doctor's orders, etc.
Notify physician when resident's condition warrants
Admit, transfer and discharge residents according to policy.
Review, monitor, intervene and report complaints/grievances made by residents, families, etc.
Perform emergency procedures such as Cardiopulmonary Resuscitation (CPR) and report accidents or incidents in a timely manner
Assign duties to non-professional nursing personnel and supervise and evaluate work performance
Monitor and update care plans each shift as appropriate
Maintain a physical, social and psychological environment which is conducive to the interest, safety and welfare of the residents.
Investigate and report employee incidents according to company policy and coordinate return to work programs with Human Resources
Participates in appropriate meetings and training
