About the Program
The TLP program provides longer-term, supportive housing and casemanagement for homeless youth ages 16 to 21 who are unstably housed. The program helps participants transition to independence by offering safe, stable housing and comprehensive support services such as life skills training, education and employment assistance, mental and physical health referrals, and family reunification efforts when appropriate. The ultimate goal of TLP is to equip young people with the skills, resources, and stability they need to achieve self-sufficiency and long-term housing stability.
Position Summary
This role is hands-on, relationship-focused, and impactful. You will walk alongside youth as they work toward stability - celebrating milestones, building trust, and helping them move into adulthood with confidence.
Job Duties Include:
Conduct intake assessments to determine eligibility, risk level, and individualized service needs.
Develop, implement, and monitor individualized case plans focused on achieving stable housing, employment, education, and self-sufficiency.
Provide trauma-informed, strengths-based support through regular one-on-one meetings with youth.
Support youth in developing independent living skills such as budgeting, cooking, cleaning, transportation, and time management.
Coordinate with schools, courts, child welfare, and other service providers to ensure continuity of care.
Support youth in locating, applying for, and maintaining safe, affordable housing upon program exit.
Provide aftercare follow-up to monitor ongoing stability and offer continued support or referrals.

OBJECTIVES OF THE POSITION Under the direction of organizational leadership, the Specialized Services CWS is part of the child welfare team that achieves the LCFS vision for all children and families having the opportunity for safe, healthy and meaningful lives. This role is part of a child welfare team consisting of child welfare specialist, medical and mental health personnel in building relationships, supports, partnerships and strengths amongst the youth, their identified caregivers and families to ensure youth who are involved in the Illinois foster care system and whom have a history of medical, mental health issues and/or multiple placements, delinquency or other level needs. The goals of the role are to ensure:

ESSENTIAL ROLES AND RESPONSILITIES (NON-EXHAUSTIVE LIST)
Serve with a team of supervisors and direct service child welfare, professional's in partnership with children, parents and community providers to assess, plan, evaluate and make linkages for safety, permanency, and well-being outcomes
Implementation and referral of individualized and targeted services for youth and families, such as counseling, respite, mentoring, educational support, casemanagement, crisis intervention, and continual language to services to meet the specialize needs of children/families
Collaborate with multi-disciplinary teams and partners for continuous quality improvement, interventions, service delivery and the achievement of program outcomes
Participate in System of Care principles (i.e.: collaborative service delivery for coordinated/integrated, youth-focused, family centered/engaged orientation) to include ensuring the provision of social/emotional/mental/behavioral/mental health specialty service provision. Complies with all requirements regarding utilization of the DCFS electronic record-keeping system (SACWIS). Prepares written service plans for each client as required by DCFS contract. Maintains up-to-date and accurate client files and databases, as required by agency/programmatic policies and procedures, professional standards, and external requirements of all relevant, regulatory, licensing, or accrediting body's. Implements, continuous quality improvement in all aspects of performance
Maintains regular communication with supervisor regarding case issues, and properly notifies the same of all significant issues, particularly those that may impact the youth, permanency, safety and well-being, as well as risk management and contractual compliance
Engages the child, the birth parents, and the foster parents incompetently assesses the dynamics of the relationships and establishes a therapeutic alliance. Teaches effective parenting skills specific to the needs of the family
Maintains knowledge of community resources and coordinates appropriate referrals and follow up as needed and required for child, birth, parents, and foster parents. Establishes collaborative relationships with agency team members such as Case Aide, other child, welfare, specialists, service, providers, and institutions
Correlates service plan outcomes and interventions to the integrative assessment and safety determination with specific interventions and at meeting the needs of the child and birth parents.
Ensures expenditures are appropriate and according to agency policy and DCFS guidelines and reports expenditures in an accurate and timely manner.
Maintains knowledge of changing regulations, policies, and rules from agency, DCFS, legal and accrediting bodies. Attends and participates in all required agency and DCFS training and timeframe required.

Implementation and referral of individualized and targeted services for youth and families, such as counseling, respite, mentoring, educational support, casemanagement, crisis intervention, and continual language to services to meet the specialize needs of children/families.
Collaborate regularly with multi-disciplinary teams and partners for continuous quality improvement, interventions, service delivery and the achievement of program outcomes.
Actively participate in System of Care including the regular participation in clinical and medical staffing's and level of care.
Actively participate in regular supervision, mediation, problem resolution, and crisis response by effectively responding to parent, child, provider and other stakeholder concerns and demonstrating the ability to effectively manage crisis and difficult case situations. (This includes on-call support.)
Assesses and maintains knowledge of child welfare and population specific laws, policies and trends for recommending program adjustments and implementation that aides program effectiveness, children, families and communities.
Achieve organizational performance outcomes on Key performance indicators metrics/dashboard towards safety, permanency, well-being and overall agency performance outcomes
Represents clients' needs and Agency interest to referral sources, DCFS, the judicial system and governmental agencies.
Continuously assesses the child's needs related to safety, education, medical and social and psychological well-being. Prepares accurate, timely required written, and verbal reports to the court and collateral contact information as appropriate upholding all consents and confidentiality issues. Handles all crisis situation and client complains with sensitivity and consults with supervisory chain when appropriate to resolve the matter.
Complies with all requirements regarding utilization of the DCFS electronic record-keeping system (SACWIS). Prepares written service plans for each client as required by DCFS contract. Maintains up-to-date and accurate client files and databases, as required by agency/programmatic policies and procedures, professional standards, and external requirements of all relevant, regulatory, licensing, or accrediting body's. Implements, continuous quality improvement in all aspects of performance.
Engages the child, the birth parents, and the foster parents in competently assessing the dynamics of the relationships and establishes a therapeutic alliance. Teaches effective parenting skills specific to the needs of the child and family.
Maintains knowledge of community resources and coordinates appropriate referrals and follow up as needed and required for child, birth, parents, and foster parents. Establishes collaborative relationships with agency team members such as Case Aide, other child, welfare, specialists, service, providers, and institutions.
Provides a welcoming and receptive environment in all contacts with clients, agency, staff and collateral contacts. Develops understanding and continuously develops self-awareness related to diversity, inclusion, and equity issues.

Job Summary

Serves as a liaison with collaterals such as DCF, schools, etc.
Shadows an experienced transporter within the first 30 days of hire.
Submits expense vouchers, renewed driver's license, and automobile insurance coverage on due date
Communicate any barriers to completing any request immediately to ensure other arrangements can be made. This should be communicated via email, text or face to face with the assigned supervisor.
Works cooperatively with other agencies, caseworkers, transporters, and family support services
Works in collaboration with Office Assistant and fellow Family Support Specialist to ensure the needs of the agency and clients are met for transportation; coordinates with assigned supervisor, client and caregivers to facilitate the transport
Ensure safety of the child during transports.
Courier documents to courthouse, agency locations, attorneys or other required areas and obtain signatures of completed deliveries when required.
Supervise and document parent/child/ visitation. Provide a written summary of the visit to the assigned casemanager supervisor within 48 hrs of the visit. Immediately report any child safety concerns to the supervisor.
Maintain mileage log and documentation reports in a timely manner to receive authorized travel reimbursement for services provided.
Assist in file maintenance when requested, this will include preparing documents to be filed in the physical case file.
Must be able to lift, carry, and move a 25 lb child in a car seat
Required Qualifications
Education: High School diploma/GED
Experience: up to 2 years' experience with transportation; 1-2 years in a childcare field
Other: Reliable transportation, valid driver's license and acceptable driving record with OHU required levels of coverage for auto insurance; geographic knowledge of the area

MST Family Therapist - St. Charles, IL We are seeking a therapist with a systemic perspective and a desire to work with families and their adolescents (12-17.5 years old) in their natural home environment. The therapist will be responsible for providing family therapy services that address severe behavioral issues for referred youth.
Qualifications
Education:
Master's degree in Psychology, Social Work, Counseling, or a related subject area
Bachelor's degree with substantial social service experience working with families and youth
Job Responsibilities
Conduct MST assessment including review of referral information, identifying and engaging key participants, identifying systemic strengths and weaknesses, and developing interventions to address the referral behaviors
Engage primary caregiver and other key participants in active change-oriented solutions
Conduct sessions/meetings in the youth's natural environment (homes, schools, attends court dates, community). Must have a car to get to all these meetings.
Maintain clear and concise documentation of treatment efforts that promote peer and supervisory review and feedback, and that demonstrate compliance with the 9 MST Principles and the MST Analytic Process.
Ability to engage and collaborate with all relevant systems and key participants within each system, to maintain alignment throughout MST treatment.
Participate in all MST training, supervision and consultation activities.
Maintain caseloads of 5 families at a time.
Staff are available to families via phone 24 hours a day, 7 days a week, with exceptions to vacations and personal time off
Must be able to work independently and be flexible

This staff member will receive MST certification through a five-day training on the MST model and will receive additional training as required
