Position Details:
We are in search of experienced, dynamic advanced practice providers to join one of our primary care sites. As a primary care medical home, we serve 28,000 patients annually throughout our six health care centers, 34-chair dental clinic, and mobile health center - a 40-foot converted RV. We are an innovative non-profit organization, with the focus on providing exceptional health care to all who have barriers to care and include integrative behavioral and dental health care in our practices.
At all our primary care sites, our multi-disciplinary teams focus on providing excellent medical care and supporting our patients' emotional and social needs, both adults and children. These teams include a mix of physicians, family nurse practitioners, physician associates, integrated behavioral health team, nurses, MAs, registration personnel, and team leads. Each team has a clinical leader, and our practice also employs a psychiatric nurse practitioner and a child and adolescent psychiatrist. Our primary care sites provide outpatient services with no inpatient or obstetrical responsibilities.
Position highlights:
La Clinica encourages a work/life balance with full-time employees generally working a four-day week, with four weeks of paid vacation per year to start, in addition to a week of CME.
Participation in the on-call rotation is optional, with additional compensation.

The Facility Maintenance Support's primary job function is to perform all entry level support and assistance with all aspects of building maintenance to ensure the safety and efficient function of all La Clinica facilities.

The Personal Health Navigator serves as an extended care team member who supports high-risk, high-barrier patients in navigating the healthcare system to follow through on co-designed care plans. The role is critical in reducing no-show rates, closing referral loops, enhancing communication between patients and providers, and improving overall health outcomes. The navigator works closely with the care team to assess barriers, empower patients, and support both virtual and in-person visits.

Required:

The Integrated Behavioral Health Clinician (IBHC) is part of the primary care team and works closely with the primary care provider, nurse, medical assistant, and other team members targeting behavioral health needs to include the mental health and substance use challenges of our patient population. The IBHC provides evidence-based time-limited assessment, consultation, and therapy services to promote behavioral health across the lifespan through easy-access, excellent interdisciplinary teams, and integrated care to children, teens, adults, and families, both individually and in group settings making referrals when necessary. Our IBHCs work in collaboration with the primary care team, as well as community agencies to foster patient wellness and satisfaction in a warm, therapeutic, and respectful manner.
