These School Based and Community Based TRP Family Support Counselor positions require a natural ability to form positive supportive relationships with children in caring, family and community based environments using ways that adhere to the AZ 12 Principles. Assist with the coordination of case management services and communication of community-based resources. Perform procedures to support clinical operations including problem resolution and customer service; documentation, facilitates Child and Family Teams, and perform other duties as assigned, which may include mentoring children, assisting with living skills, providing respite for families, and other supportive services. This role requires a high level of work within the individual adolescent community, as well as Group Facilitation. Based in our Glendale or Tempe office.
Essential Responsibilities:
Promote and protect the health, safety, and emotional well being of children and families by assisting, supporting and educating them to prevent illness and avoid unsafe activities.
Provide mentoring, life and social skill building to children and their families in their home and in community-based settings.
Collaborate with various stakeholder groups.
Provide psychosocial rehabilitation living skills training in children's homes or community-based settings.
Provide respite for families and assist with living skills in the home community-based settings.
In-home family and individual counseling.
Work with the child, family, and any support network to identify areas of risk and to create safeguards specific to these concerns.
Assists the CFT in crisis stabilization and transitional moves to and from higher levels of care.
Consistently address challenging behaviors proactively, respectfully, and by avoiding the use of aversive or deprivation intervention techniques in family-centered, strengths-based manners.
What you'll provide:
High School Diploma/GED is required.
BHT level as stipulated by AZDHS/BHS is required.
Experience working with children with behavioral needs and different diagnosis.
Experience working with parents on parenting skills, modifying behavior and modeling parenting style.
Ability to engage with others easily, coordinate with DCS and follow up in timely manner.
Previous use of behavior charts and behavior modification skills.
Ability to work a flexible schedule including evening and/or weekend hours.
Comfort level working with and transporting youth in company vehicles.
Detail oriented and ability to multi-task in a fast-paced environment.
Bilingual Spanish speaking is preferred.

Empact is proud to be part of a Premier Crisis system being modeled throughout the country. Come join a team that is driving change. This is a great opportunity for a Crisis Specialist to join our dynamic department in Tempe or Glendale, AZ and provide crisis services. The job responsibilities will include but are not limited to responding to crisis calls, completing crisis interventions for ongoing clients, case management and coordination of care, as well as interfacing with first responders. Services will be provided according to training, experience, and certification as assigned. This position will require an individual with skills in working in a high-paced environment in a Crisis setting. Wage Differential available.
Work Schedule: Four 10-hour work days per week (DAY, WEEKEND and EVENING shifts available).
*Due to the nature of business in Crisis work, breaks/mealtimes are generally taken while on shift, in between calls. If an employee needs an extended break for personal reasons, this needs to be discussed with the department supervisor or On-Call Coordinator.
Essential Responsibilities:
Provide direct in-home, in-office, in-school and community based crisis services. These services will include but not be limited to crisis intervention and de-escalation, transportation, maintaining or establishing community stabilization, client support and/or supervision, parenting training, behavior management training and assistance, and provision and assistance in accessing appropriate community referrals.
Ensure quality service delivery including clinical care and all associated documentation. Maintain all client records in accordance with agency policies and procedures.
Provide comprehensive services to clients and assist in coordinating communication with other departments of the agency to accomplish continuity of care. Develop and maintain effective working relationships with all related state, community behavioral health, and contracting organizations.
Comply with the agency's standard of care for service delivery. Assist with Quality Management and Utilization Review as needed.
Perform job duties in a safe manner at all times; provide support for team members and perform cooperatively and professionally as part of a cohesive team.
Will be required to oversee everyday functioning of department vehicles and to immediately report incidents involving damage or theft. May be required to do basic safety checks on vehicles (oil, tire air pressure, etc.).
Will be required at times to work in potentially volatile situations and to react in a professional manner to crisis situations regardless of the nature of the environment or circumstances.
Other Duties and Responsibilities:
This position receives a differential in pay, which is specific to this position only. The stated responsibilities accompany the position and the high-risk nature of the account for the differential.
Will be required to be flexible in terms of shift location and to change location if requested at the beginning of the shift to better meet the needs of the department.
May be required to use "down time" for special projects to benefit Crisis and/or other departments within the agency.
Attend all required meetings and trainings, even if they fall outside of the regularly scheduled shift. Evening and night hours may be required. Receive supervision each month from a Master's level clinical coordinator. If department meetings or scheduled supervision is missed because of a crisis call, the specialist is required to reschedule the meeting to assure that all supervision requirements are met.
Will be required to work beyond his/her shift with no advanced warning if the crisis situation warrants or if required paperwork is not complete at the end of the shift. The Department Manager or Director of Programs must approve overtime associated with staying over shift.
May be required to use own personal vehicle if agency van is out of service or is unavailable. However, will not be required to transport clients in own vehicle.

What you'll provide:
Minimum of a Bachelor's Degree in psychology, counseling, social work or a directly related field Plus 1 yr of Behavioral Health experience OR the qualifications to meet BHT level requirements
Experience in Crisis Services, Outpatient Services or Community Outreach is preferred. Special consideration given to those who have experience in sub-acute or urgent psychiatric settings.
Flexibility to extend work shift to complete crisis work, paperwork, etc. as needed.
Demonstrated ability to remain calm in stressful situations and collaborate/work in a team environment.

We are looking for an experienced Case Manager (General Mental Health) to join our team of highly committed clinical staff. The Case Manager will provide case management and coordination services to include: direct service, crisis intervention, transportation, and education to adults, children, and their families. This position is responsible for the coordination of services in accordance with the Arizona Practice Model and the 12 Principles.
Essential Responsibilities:
Ensure quality service delivery, including clinical care and all associated duties. Provide direct in-home, in-office and community services. These services will include, but not be limited to, crisis intervention, transportation, maintaining established client stabilization, additional client supervision, behavior management and support needed to maintain client in the community.
Facilitate, schedule, and provide follow up on Child and Family Team Meetings/Adult Recovery Team Meetings. Complete written documentation of this meeting and provide a copy to all team members. Engage and invite natural supports as identified by the client. Develop and maintain effective working relationships with other agency stakeholders. Make referrals to collaborating partners and other community services, as client needs indicate.
Deliver at least 40 hours of work per week, with a goal of 30 hours of direct client contact. Productivity standards established at a minimum of once a year and as needed due to contract changes. Complete all tasks agreed upon by timelines given.
In collaboration with each child and family, support services to families, youth, and adults, that meet the following 12 Arizona Principles: Collaboration with the family, youth, and adult; Emphasis on achieving functional outcomes; Collaboration with others, including participation in unified assessment, planning and service approaches when families, youth, and adults are involved with multiple systems; Access for families, youth, and adults to a comprehensive array of sufficient behavioral health services to meet their needs; Best practices; Most appropriate setting; Timeliness; Services tailored to the family, youth, and adult; Stability in placements; Respect for the family's, youth's, and adult's unique cultural heritage; Independence, and; Connection to natural supports. Develop and maintain effective working relationships with all related state, community mental health, and contracting organizations.
Maintain all client records in current status and in accordance with agency policies and procedures, as well as RBHA, state, and federal requirements. Produce complete, timely, thorough, quality clinical documentation of all services rendered, both billable and non-billable, as required. The documentation must include sufficient information about the activity being documented and the client's participation in, and response to, the activity.
If driving while on agency time or on agency business, a valid Arizona driver license and appropriate liability insurance and approval for liability coverage with EMPACT - SPC's liability insurance carrier is required. At time of hire, a 3-year state driving record with no major infractions or excessive driving violations/tickets is required.
Attend regular scheduled weekly supervision. Seek out additional supervision as needed.
What you'll provide:
Must have a bachelor's degree in behavioral health related field or any combination of education/experience that meets BHT requirements as stipulated by AZDHS/BHS.
Need to have flexible work hours and ability to meet with individuals in their home, community or office.
Must have Fingerprint clearance card or be eligible to obtain one.
Must have a valid AZ driver's license with no major infractions in the past three years.
Must have regular access to reliable transportation.
Bi-lingual (Spanish) ability preferred. Spanish language differential available.
