    Job Description Participates in the on-going treatment of clients including medication supervision, health education, assessment, case management, and provision of therapeutic services; provides staff orientation and training; assists in team supervision and participates in the planning, development, and implementation of services. DUTIES Administers medications as directed by medical staff and monitors medication effects. Provides consultation and education to clients, staff and community on medication actions, side effects, and proper health and nutrition habits. Maintains client charts and records according to established guidelines and procedures. Provides case management services for clients, including development of treatment and discharge plans, follow-up, and closure. Provides clinical outpatient treatment, including crisis intervention, to individuals, families, and groups Performs physical assessments, screens walk-in clients, and assists physicians in physical examination of clients. Draws laboratory samples, screens laboratory results, and facilitates follow-up medical treatment. Assists in client outreach and advocacy activities. Attends and participates in staff meetings, team staffings, and in-service training. Facilitates service training sessions. Develops and maintains liaison relationships with other community medical agencies. Provides orientation and training to other staff members. Participates in the planning, development, and implementation of new services to underserved populations. May act as unit supervisor in his/her absence or provide on-site supervision of clinical and clerical staff. Assists behavioral health providers as needed. Coordinate all care and services required to enhance and facilitate the optimal physical and behavioral health wellness of persons served within a trauma-informed care framework. This includes: 1) facilitating communication between medical and behavioral health practitioners/systems; 2) assisting with scheduling of and compliance with preventative and specialty physical health interventions; 3) integrating treatment interventions to address both the physical and behavioral health needs of the person/family concurrently; 4) assist persons/families served with education and resources outlining the interplay of physical and behavioral health issues on overall wellness and quality of life.
