Responsibilities:
Provides patient care during all phases of the transport from initial contact until care is relinquished to appropriate team member at the accepting medical facility.
Documents patient assessment, evaluation, interventions, patient response to therapy, changes in condition, and procedures within the established timeframe. Participates in Pre and Post Transport briefings. Provides patient follow up to referral agencies.
Ensures equipment is in working order, noting any discrepancies and ensuring steps toward resolution. Completes all required vehicle and equipment checklists.
Collaborates across disciplines to drive success and improve outcomes. Provides safety and clinical education to referring facilities and EMS agencies as needed. Active participant in safety, education, quality, and outreach.
Within scope of job, requires critical thinking skills, decisive judgement and the ability to work with minimal supervision. Must be able to work in a fast-paced environment and take appropriate action.
Requirements:
Minimum Required Education: Bachelors of Science in Nursing
Required Licensure: Colorado OR Multi-state compact RN License at hire. Multi-state compact Nursing License within six (6) months of hire.
Flight Nurse Certification (CFRN)
Valid driver's license.
Basic Life Support (BLS) Healthcare Provider; Advanced Cardiac Life Support (ACLS), Pediatric Advanced Life Support (PALS), Neonatal Resuscitation (NRP); and any one of the following: Transport Professional Advanced Trauma Course (TPATC), or TNCC (Trauma Nurse Core Course) or Advanced Trauma Care for Nurses (ATCN).
Minimum Experience: 5 years ICU, Emergency department (or combination of both) experience
Preferred: Previous Flight, CCT or EMS experience. Instructor certifications (BLS, ACLS, PALS, NRP, TPATC or TNCC or ATCN)
BLS through the American Heart Association or the American Red Cross CPR for the Professional Rescuer with card in-hand before start date. BLS or CPR card must be good through sixty days of hire.

Paragon Behavioral Health Connections is seeking a Masters Level Clinician for Substance Use Disorder Treatment to provide Substance Use Disorder (SUD) treatment services within our Intensive Outpatient Program (IOP) and conduct clinical crisis assessments as needed. This role involves delivering treatment in accordance with ASAM Level 2.1 guidelines and providing both group and individual therapy sessions. The clinician will work in or near the designated locations and travel to homes and communities of the clients we serve.

Program Description:
The Child First model is an evidence-based, two-generation intervention that works with very vulnerable young children (prenatal through age 5 years) and their families, providing intensive, home-based services to decrease the incidence of serious mental health problems, developmental and learning disabilities, and abuse and neglect. Child First provides (1) a psychotherapeutic, dyadic intervention to strengthen the parent-child relationship, and (2) care coordination to connect the family to needed services and supports. Child First has been recognized as an evidence-based home visiting model by the U.S. Department of Health and Human Services (HHS) under the Maternal, Infant, and Early Childhood Home Visiting (MIECHV) Program and rated Effective by the National Registry for Effective Programs and Practice (NREPP) of the Substance Abuse and Mental Health Service Administration (SAMHSA).
Summary of Position
The Child First Mental Health and Developmental Clinician partners with a Family Support Partner to engage families who are referred to the Child First home-based intervention. Child Firsts primary goal is to strengthen the caregiver-child relationship so that it serves both as a protective buffer to unavoidable stress and directly facilitates the childs emotional, language, and cognitive growth. The Clinician uses trauma-informed CPP, a relationship-based, dyadic, parent-child treatment model, which focuses on the primary attachment relationships of the young child. The Clinician engages with both the caregiver and child in a supportive, reflective, and exploratory manner which fosters a protective, nurturing, and responsive parent-child relationship. The Clinicians therapeutic intervention focuses on: 1) helping caregivers understand typical developmental challenges and expectations; 2) increasing caregivers ability to reflect on the meaning and feelings motivating a childs behavior; 3) supporting caregivers problem-solving; and 4) helping caregivers understand the psychodynamic relationship between parental feelings, history, and the caregiver response to the child. The Clinician also provides consultation to teachers in early care and education settings, as needed.
Responsibilities
Engage with the Child First family and the Family Support Partner in the collaborative family assessment process (i.e., gather information from interviews, observations of interactions and play, reviewed records, collateral sources, and standardized measures).
Use all available information to develop a thoughtful, well-integrated clinical formulation and Child and Family Plan of Care, in partnership with the Family Support Partner and family.
Provide Child First home-based psychotherapeutic intervention with young children and their caregivers using relational, dyadic psychotherapy (CPP) and other modalities.
Help the caregiver gain insight regarding their personal history (including trauma history), feelings for the child, and current parenting practices.
Avert crisis situations by assisting the family in times of urgent need (e.g., risk of harm to child or caregivers, pending child removal), in consultation with the Family Support Partner and Clinical Supervisor.
Provide mental health and developmental assessment and consultation within early care and education settings and to other early childhood providers.
Embrace using videotaping to enhance therapeutic work with families and reflective supervision.
Engage in weekly individual, Team, and group reflective clinical supervision with Clinical Supervisor.
Engage actively in all aspects of the Child First Learning Collaborative, including in-person trainings, distance learning curriculum, and specialty trainings.
Keep all appropriate documentation for clinical accountability and reimbursement.
Participate in other clinical and administrative activities as appropriate.

The core goal of the Village Care Provider's (CNA)work is to provide assistance in a manner that promotes a high quality of life and a positive community. All responsibilities will follow the current federal, state and local standards, guidelines and regulations that govern long term care and assisted living facilities. You are delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties.

KEY RESPONSIBILITIES:
Working with your household team of Village Care Providers (CNA), recognize and respond to the needs and desires of the villagers
Work various houses or work flexible schedules as agency and supervisor deems necessary. Staff attendance and promptness to assigned shifts is essential. The responsibilities of this position may require staff to be able to work irregular, extended, and/or be available for on-call work hours, evenings, nights, weekends, holidays, etc.
Promote autonomy of villagers in decision-making regarding their daily lives and the life of the house
Work together with the other Village Care Provider (CNA) as a self-managed team, responsible for managing all aspects of the home, including food ordering, preparation and clean up, light housekeeping duties, all laundry needs of the villagers, and assisting villagers in engaging in the daily activities of their choosing
Cooperate with the nurses and other clinical team members to ensure the well-being of each villager
Receive and report resident status at the beginning and end of each shift and as needed to team members and to nurses and clinical support team members
Assist villagers with personal care functions, including: dental and mouth care, bathing, dressing and grooming, hair care, nail care, shaving, bowel and bladder care, other personal and hygiene care
Assist with and/or perform restorative and rehabilitative procedures as outlined and assigned by the care plan
Measure and record vital signs such as temperature, pulse, respirations, weight, height, and anatomical dimension/circumference
Assist moving, positioning and transporting villagers into and out of beds, chairs, wheelchairs, lifts, etc.; make room checks and verify resident location
Assist with the application of slings, elastic bandages, binders, etc
Provide range of motion exercises as outlined and assigned by the care plan
Work with the villagers and the Dietitian to create a balanced menu that meets the nutritional needs of the villagers as well as their preferences for foods they enjoy
Prepare and serve meals, between meal, bedtime and other nutritional snacks per the posted menu and stated preferences of the villagers
Keep all areas of the home clean, performing routine housekeeping duties (i.e. changing bed linens, emptying trash, dusting)
Launders clothing, linen and other articles
Assures villager calls for assistance are answered promptly
Cooperate with the Activity professional to develop and carry out support of the villager's daily activities, including opportunity for spontaneous activities to occur
Maintain confidentiality and rights of villager, family, employee, and house information in compliance with PHI HIPAA policies and applicable regulations
Meets with the Clinical Service Team as schedule or needed to discuss villagers' needs and progress in identifying and correcting problems
Observe, monitor, and report to the CST any change in condition
Document observed data per policy
Will serve in different coordinator roles on a rotating basis. These include Team, Care, Scheduling, Food & Housekeeping Coordinator
Attend and participate in Team meetings on a regular basis
Attends and participates in orientation programs, on-going training and educational classes
Maintains all equipment and appliances in the house by adhering to proper operating guidelines
Documents and maintains records necessary to comply with all state and federal regulations
Report any problems with equipment or appliances immediately to the maintenance director
Adheres to all safety guidelines and processes; and observe, monitor, and intervene in or report to the clinical support team any unsafe conditions; observes standard precautions
Reports accidents and incidents when they occur
Report any abuse or suspected abuse to the Abuse Coordinator immediately
Follows established smoking regulations and reports violations. (Non-smoking campus)

Able to work with mathematical concepts such as probability and statistical inference and provide necessary reports as needed.
REASONING ABILITY: Able to deal with problems involving a variety of concrete variables in standardized situations. Able to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Able to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. Able to define problems, collect data, establish facts, and draw valid conclusions.
* SUPERVISORY RESPONSIBILITIES:
The Chief Financial Officer provides the bulk of supervision required. In general, this position requires that the individual work independently approximately 80% of the time. Mentoring duties for clients and/or clerical support may be required.

Able to work with others, with system issues, and able to work within designated time frames. Patience in dealing with staff, individuals in service, and parents.
