Job Description
The Director of Quality and Safety reports to the Chief Nursing Officer and matrixed to the Chief Medical Officer. Under general supervision and within La Rabida's Children's Hospital established policies and procedures, develops, directs, coordinates, and evaluates the comprehensive care transformation activities leading to core measure performance, physician peer review, and professional practice evaluation processes as well as addressing and investigating patient safety events and working to implement corrective actions as necessary to enhance the culture of safety. This leader will lead the implementation and optimization of the quality and safety program. This leader serves as the safety champion for the organization and will define/lead the safety initiatives for the organization.
Lead a highly skilled team of quality and patient experience specialists focused on meeting customer needs and exceeding their expectations.
Oversee and review selected quality issues and provide guidance for staff to lead investigations of critical events.
Lead clinical reviews with evidenced based systems such as, Apparent Cause Analysis (ACA) Root Cause Analysis (RCA)/FMEA team projects using methodologies and tools utilized at La Rabida Children's Hospital and facilitates development of corrective action plans when indicated; and oversight and monitoring as appropriate.
Assist and supports the development of reports submitted to regulatory and accrediting agencies including the Department of Health, the Joint Commission, and others as directed by the Director of Regulatory, Risk and Compliance.
Oversee and review selected quality and service issues and provide guidance for staff to initiate improvement efforts for optimized performance
Participate in Medical Staff department meetings and serves as resource for the dissemination of information related to internal and external peer review requirements.
Coordinate and align all core quality functions for quality and operational excellence and support the quality committee structure for the hospital
Works with hospital leaders to develop, measure, analyze and implement process improvement initiatives utilizing quality strategies scaled for efficiency and purpose
Manage clinical and patient safety data needs and leads team(s) toward implementing successful improvement strategies
Develop customer service training for onboarding and ongoing training as well as just-in-time curriculum content for new initiatives.
Coordinates and supports the implementation of improvement tactics in alignment with organizational priorities. Identifies and presents best practices and other tactics needed to improve performance. Educates frontline staff and other key stakeholders on tactics and execution. Monitors and makes suggestions to improve best practices and their integration into existing systems.
Serve as a liaison to the Board Quality committee and to the Medical staff departments for coordination of quality improvement actions involving patient care processes and physician processes. Promotes meaningful communication and performance improvement information among and between La Rabida and the Medical Staff
Qualifications
A Bachelor's degree in any science discipline, including Nursing is Required.
Master's degree in related healthcare field Preferred
Licensed Registered Nurse Preferred
Six Sigma Black Belt certification or equivalent experience leading performance improvement projects Preferred.
CPHQ and CPPS Preferred
A minimum of five (5) years of related experience is a must particularly in quality measurement and reporting systems, oversight over physician peer review programs, and process evaluation and improvement. Extensive knowledge of performance improvement systems and/or process improvement methods and standards. Effective project management skills are required. Must have experience with electronic health record applications and modules Microsoft Office Suite (e.g. Word Excel Access PowerPoint) other computer applications database development and outcomes management. Proficiency Windows-based PC environments and statistical software packages used for analysis and graphing.
Certification by the National Association for Healthcare Quality desirable
CPR Required (Only certifications issued by the American Heart Association or the American Red Cross will be accepted)
Strong group facilitation and project management skills required. Ability to collaborate and lead various project teams. Solid written/verbal presentation critical thinking and analytical skills required
Must have experience in safety programs - developing and implementing patient safety programs, promoting a culture of safety, utilizing patient safety tools and techniques as this role will launch and lead our patient safety strategy.

Job Description
The Security Officer/Driver patrols and maintains surveillance of the Hospital, both internally and externally, to ensure the physical safety of our employees and the patients we serve. The Security Officer/Driver additionally services patients, visitors, and staff through verbal interaction and vehicular transportation. The percentage of time spent in each capacity varies according to days and shifts worked, as well as shuttle and transportation needs.
Security Officer (35 - 85%)

Diagnostic Medical Sonographer/Ultrasound
General Summary/Basic Purpose of Job:
Diagnostic Medical Sonographers produce sonographic images of high-risk OB/Gyne, general and vascular structures and internal organs and cavities for use in diagnosing disease. During this process, they must be able to operate the Diagnostic Ultrasound equipment to obtain the most diagnostic images possible using the appropriate transducers and operational controls.
Essential Functions and Duties:
Has a thorough understanding of ultrasound equipment and physics in order to produce high quality images for review by the imaging physician or radiologist.
The ability to discern the difference between real anatomic reflections from tissue versus artifact and adjust controls to reduce the effect of the artifacts.
Ability to integrate diagnostic sonograms, laboratory results, patient history and medical records, and adapt the Sonographic exam as necessary.
Perform ultrasonic diagnostic procedures in accordance with accepted ACR/SDMS/AIUM and department examination protocols as directed by the reading physicians.
Maintain ultrasound equipment in a clean and sanitized condition at all times and report any damage or malfunction immediately to your supervisor or biomedical engineering. Clean every transducer and transducer cable after each use.
Use AIDET principles with all patients by: acknowledging them, introducing yourself, letting them know how long the exam will take, explaining the procedure and thanking them for visiting our facility.
The sonographer shall verify the identity of all patients by checking their registration ID band, and by asking the patient the following: ask for their full name, and date of birth and verify that the answers match the arm band and the referral paperwork.
Understands the use of all Doppler controls and principles in order to produce the proper images for evaluation and diagnosis by the reading physician.
Must understand the difference between normal appearing anatomy, abnormally appearing anatomy and alterations caused by pathology, and fluid collections, and create an imaging file that best represents this.
Demonstrates the ability to assist in performing biopsies, fluid aspiration, tissue aspiration and other invasive procedures while documenting these procedures using ultrasound imaging.
If assigned and is fully trained in breast, pediatric, vascular, OB& GYN or general ultrasound imaging, be able to produce accurate imaging of all anatomy or pathology using the appropriate scanning techniques and protocols.
Consistently places studies into Taken status in Meditech, verified status into PACS, and properly attaches all relevant ultrasound orders in AS software.
Assigns ultrasound studies properly to the designated reading physician based on the exam type; i.e. MFM OB read versus the radiologist read. And ensures that OB studies are not in preliminary status in PACS.
Equally, shares the responsibility of performing exams on inpatients, ED patients, portable exams, and Schwab patients; and is an effective communicator with other ultrasound caregivers and office staff.
Performs quality assurance tests frequently on the ultrasound machines and reports any defects or damages to the biomedical engineering department via the SinaiNet Renovo portal or by calling the biomedical engineering department directly.
Ensures proper protocol and documentation of the high level disinfection process for the ultrasound probes (i.e. Trophon, Cidex, ect.) and reviews the log book for inconsistencies.
Ability to rotate across the system including: MSH, HCH, La Villita, Hawthorne and Lawndale. This includes shift flexibility as well as willing to cover PTO, call-in's and emergencies.

Establishes change protocol procedures for the system. This includes prioritizing and implementing changes requested for the system.
Coordinates activities of assigned projects or particular phases of a given project. Develop detailed work plans, timelines, and identifies appropriate resources needed and provides timely status reports.
Responsible for projects within the specification of the project management schedule. Builds and maintains a catalog of decision points as it relates to the build of the system. Evaluates system utilization and performance through collecting information regarding potential system enhancement needs.

Minimum Qualifications:

The Research Project Manager will work with Principal Investigator(s) and Research Manager to manage, oversee, and guide ongoing complex research projects to ensure project deliverables and timelines meet expectations. May assist in daily supervisory responsibilities of study/project team members.

Job Description
The Department of Psychiatry and Behavioral Health at Ann & Robert H. Lurie Children's Hospital of Chicago is seeking a pediatric neuropsychologist who will join a multidisciplinary child and adolescent psychiatry department to work with children and adolescents followed in the Pediatric Neurosurgery subspecialty clinics for pediatric neurovascular disease, with potential opportunities to work with patients with epilepsy and brain tumors. This is full-time non-tenure-eligible Clinician-Educator faculty level position.
The successful applicant will have Duties include:
1) conducting screenings, consultations, and evidence-based neuropsychological evaluations of children and adolescents with medical, psychiatric, and neurodevelopmental disorders
2) teaching in multidisciplinary child health and mental health training programs
3) engagement in research or other scholarly activities
Qualified candidates will have obtained a PhD from a clinical psychology training program and completed an APA-approved internship specializing in clinical child or pediatric psychology. The individual will also have completed a fellowship in pediatric neuropsychology and be board certified or eligible for board certification in clinical neuropsychology. Experience with children/adolescents with endocrine and genetic disorders is preferred; experience with other medical populations is highly desirable. Spanish fluency is also highly desirable.
K.S.A.'s:
Minimum education required to perform job: a doctoral degree in clinical psychology with appropriate internship and a two-year post-doctoral fellowship in clinical child neuropsychology. If degree is in a related area (e.g., human development) must have completed APA approved internship.
Required licensure and/or certification: Must be licensed in the State of Illinois prior to start date. If employee is license-eligible, employee must obtain license and be granted clinical privileges before start date.
Experience conducting neuropsychological examinations in the context of pediatric epilepsy care, and collaborating with a pediatric epilepsy surgical planning team.
Experienced in creating appropriate paradigms for fMRI exams and well versed in their administration.
Experience in the design, administration, and analysis of functional imaging examinations such as fMRI, MEG, and EEG-fMRI.
Experience in design, administration, and analysis of other functional brain mapping examinations using techniques such as Wada, subdural grid electrical stimulation, awake intra-operative language mapping, and Transcranial Magnetic Stimulation.
Understanding of the MRI environment and able to collaborate with the MR technologist, MR physicist, and neuroradiologist.
Knowledge of the basic applications of advanced neuroimaging post-processing techniques.
Trained in aspects of clinical and/or behavioral translational research.
Ability to think proactively, act creatively, and take initiative appropriately. Excellent verbal and written communication, critical-thinking, and interpersonal skills.
Documented track record of academic productivity/potential, including peer-reviewed publications and grant applications.
Demonstrated ability to work as a part of a multidisciplinary team.
Special physical requirements - sits and stands throughout the day. Physical management of patients in crisis situation may be necessary.
Basic Life Support Certification required.

Conducts DSM diagnostic evaluations of children and adolescents.
Uses neuropsychological test results to inform planning for pediatric epilepsy surgery.

Collaborates and consults with medical providers, schools, and social service agencies as well as city, county, and state funding and placement agencies through oral and written presentation of clinical findings and recommendations. Completes all case management duties of assigned cases including discharge planning, completion of the diagnostic evaluation and referrals to appropriate support services as needed.
Participates in rounds, patient care conferences, diagnostic and treatment staffing team meetings, multidisciplinary case conferences, and discipline-specific team meetings.
Engages in research related to pediatric neuropsychology and, when appropriate, in program evaluation. Scholarly activities and research related to the disorders encountered in their work or treatment approaches used in their clinical activities is strongly encouraged
Provides supervision for varying numbers of participants in the APA approved internship training program.

GENERAL SUMMARY:
Operates radiological equipment to produce quality radiographs (x-rays) of the body for diagnostic purposes under the direction of a physician.
ESSENTIAL FUNCTIONS AND DUTIES:
Position patients and adjust immobilization devices to obtain optimum views of a specified area and/or body part by using ionizing radiation. Also assists physician during fluoroscopy exams.

Consistently and correctly place patient's identification on images using at least two patient identifiers in accordance with the National Patient Safety Standards; properly processing imaging and labeling them with the correct study descriptions. Also labeling the images with the correct lead makers on a consistent basis.
Prepares and maintains appropriate records as required. Verify cases in PACS (Picture Archiving Communication System) and complete cases in the RIS (Radiology information system) on a timely basis. Participate in any and all projects pertaining to the maintenance and/or correction of the radiology data bases.
Explains procedures to patient to reduce anxieties and obtain patient cooperation. Display the ability to recognize patients pain and provide comfort when possible. Display superior customer service skills and participate in the distribution and collection of customer service surveys.
Able to moves x-ray equipment into specified positions and locations. Knowledgeable in the adjustment of equipment settings in relations to KVP, MA, time and distance.
Practices radiation safety and protection by utilizing the proper techniques in collimation, shielding and ensuring patients, co-workers and family are at the desired distance prior to emitting ionizing radiation.
Competently operates fixed and mobile x-ray equipment across many settings- operating room, emergency room, or at patient's bedside. Participate in all areas of General Imaging not limited to Surgery, Out Patient Clinic, Emergency Room, Inpatient area, Portables, Schwab and Urodynamics.
Demonstrates radiologic techniques for age-specific patients needs including procedures for port chest, skull, spine, abdomen, extremity, pelvis, and O.R. procedures with C-arm.
Consistently and competently demonstrates knowledge and ability in use of departmental equipment not limited to the CR/DR general diagnostic rooms, digital/analog portable x-ray machines, C-Arms, fluoroscopy, patient moving equipment, CR Readers and, Disc Makers.
Demonstrates ability to address proper safety procedures including shielding from radiation, equipment safety, and patient safety. Knowledgeable of all Emergency Code and perform well in all Mock Training scenarios. Certified in CPR and obtained certification in a timely manner.
Demonstrates knowledge of criteria to identify victims of abuse and neglect.
Performs other duties as assigned. Work overtime when needed. Display leadership and teamwork throughout the department and hospital. Provide support in all quality improvement initiatives by submitting ideas and suggestions. MINIMUM EDUCATION:

Knowledge, Skills and Abilities:
Licensed physician (DDS or DMD) in the State of Illinois with board certification in Pediatric Dentistry.

Job Description
The Program Supervisor is responsible for promoting a child-focused, trauma-informed, supportive environment at the Children's Advocacy Center by providing program oversight, conducting supervision with staff and interns, and providing direct services to children and families.
Supervise and direct assigned staff in the performance of their duties and conducting regular performance evaluations.
Oversee the coordination, advocacy, and forensic interviewing services at the center, ensuring the program complies with NCA accreditation and grant contractual requirements.
Assist in screening, interviewing, and selecting new hire candidates.
Responsible for training, oversight, and onboarding for assigned staff members.
Provide twice weekly supervision, at minimum, to assigned staff members. Supervision will include review of progress on assigned tasks, reflecting on the impact of case and team dynamics, progress toward performance goals, and self-care.
Perform continuous quality improvement by observing each supervisee in the performance of their duties for one case twice a year, highlighting strengths and opportunities for growth.
Coordinate quality assurance efforts through chart audits.

Job Description
Functions in an independent and professional manner to provide high-quality, trauma informed behavioral health services to outpatient clients and their families
Clinical Responsibilities:
Provides short-term and long-term evidence-based behavioral health services to patients and their families, including individual, family, and group therapy
Provides behavioral health consultation services in clinics within an integrated health model for a minimum of 8 hours per week
Provides care for patients with presenting symptoms such as behavioral problems, trauma exposure and symptomology, school and learning problems, attention problems, anxiety, depression, as well as others

Job Description
Serves as Integrated Assessment Clinical Screener. Utilizes assessment instruments designed for this program to establish measures of child/client functioning and to determine types of community and social supports required; administers clinical interviews for the IA program to clients (children), parents and other adults in the child's life; creates IA template reports and provides other services necessary to assess the child and implement a care plan.
Clinical Responsibilities: (90%)
Reviews all prior documentation/records related to the case (e.g. Juvenile Court, DCFS, medical, school, mental health, etc.). Integrates this information into IA report(s). Administers the Integrated Assessment screening/interviews to children who are DCFS youth and caregivers/foster parents, parents and other adults who are part of the child's life (e.g. stepparents, aunts/uncles, etc.), to determine the types of services, treatment, and community and social supports needed to provide the required help, care, and treatment to the child and to optimize permanency outcomes. IA is conducted in the home of the child.
Administers all required tools based on the appropriate age/role of each client. Completes the required referral and submits the tools to DCFS by the federal/contract mandate on all cases. Makes appropriate referrals based on the results of the tools and clinical observation. Ensures the referral matches the final IA report
Writes the IA template report on all cases summarizing all historical information, integrating all interview data, medical data including health works, juvenile court information, DCFS investigative information, data from other child welfare agencies, and other data and information
Upon completion, presents the written report to the IA Clinical Lead or IA Program Director for review and supervision. Meets with Clinical Lead once per week in person utilizing the expectations of the program model of supervision
Meets with Permanency Worker/Supervisor to discuss findings, the development of the service plan, and to prepare for the family meeting. Makes expected attempts to staff the IA report in person. Makes weekly attempts to attend the family meeting for 30 days after the staffing
Completes and submits Child and Adolescent Needs and Strengths (CANS) as outlined in the IA policies as well as completes and submits each closed case file to the Clinical Lead to ensure completion within 14 days of SACWIS entry. Ensures each case's closed file includes all vital information. Submits closed file to supervisor and IA Administrative Assistant on time
Maintains ongoing communication with the Permanency Worker, supervisor, and family members throughout the interview/screening and writing process. Is available to staff for clinical consultation related to the IA report, client functioning and identification of further assessment and/or service needs
Attends and participates in the LRCH IA case consultations by presenting cases for consultation as well as providing feedback to others presenting. Prepares for and participates in monthly training discussions
Administrative and Case Management Responsibilities: (10%)
Participates in quality assurance activities by submitting the weekly contact sheet
Participates in research activities by ensuring all information is included in the IA report
