Duties: The Care Coordinator will act as an integral member of the ReVIDA Recovery multidisciplinary team by supporting patients in obtaining their individualized goals, by building and strengthening linkages with community resources and family members by deliberately organizing patient care activities and sharing information among all the participants concerned with a patient's care to achieve safer and more effective care. The Care Coordinator will assist individuals in opioid use disorder treatment in developing a service plan, referrals, and linkage, ensuring the patient has access/continuity of care throughout the mental health and primary care system, ensuring each patient has resources to acquire medication, has transportation for appointments, and attends appointments. The Care Coordinator will ensure individuals have access to psychosocial rehabilitation, support, employment, and housing options while encouraging the individual to utilize community/natural supports to assist in managing substance use disorders. ESSENTIAL FUNCTIONS: Perform a full range of phlebotomist duties to include proper venipuncture and finger-stick and heel-stick procedures using appropriate techniques, and perform set-up and completion of special test-related phlebotomy procedures utilizing appropriate techniques. Ensure that all aspects of patient accessioning, specimen collection, processing, and distribution are performed properly. Conduct patient in-processing prior to specimen collection, including review of paperwork accompanying patients and cordial guidance and assistance. Assemble equipment, such as tourniquet, needles, and disposable containers for needles, blood collection devices, gauze, cotton, and alcohol on work tray, according to requirements for specified tests or procedures. Verify or record identity of patient or donor and converses with patient or donor to allay fear of procedure and conduct interviews, take vital signs, and draw and test blood samples to screen donors at blood bank. If applicable, provide technical oversight and training to ensure that all invasive procedures (venipuncture, capillary puncture, clotting time, etc.) performed in the laboratory are done in the safest, most sanitary, and expeditious manner, with minimal discomfort for the patient. Qualifications * Four-year college degree preferred or equivalent education, training, and/or experience * Minimum 2 years' experience in QA role within the food production industry * PCQI training or HACCP certification preferred * Ability to pass Comprehension and Math Tests * Must have Computer skills and knowledge of Microsoft word, Excel, SAP, and AS400 * Good understanding of GMPs, analytical and microbiological testing Safety & GMP Objectives * Maintain a safe and clean work environment, maintaining compliance with established policies and procedures. * Comply with all company safety policies and procedures as well as complying with all OSHA, EPA and other regulatory rules and regulations (THINK "SAFETY FIRST"). * Ability to recognize unsafe acts and conditions in the facility and implement appropriate corrective actions. * Ability to fully complete appropriate paperwork related to specific job functions in addition to quality and safety/training. * Ability to recognize and report Skills and Specifications * Ability to interact effectively with various levels of professionals in the organization * Excellent verbal, interpersonal, and written communication skills * Ability to guide, teach and communicate with others * Willingness to learn new skills and teach others * Must be able to attend work as scheduled * Must be able to read and write in English * Must be able to perform basic math problems * Must be able to perform the jobs of his team members * Must be able to follow verbal and written instructions * Must be able to interact with others in a professional manner Reports to QA Supervisor
