The part-time Care Manager (CM) Float acts as a temporary member of an interdisciplinary team (IDT), serving frail elders, adults with physical disabilities and adults with intellectual/developmental disabilities who are members of Lakeland Care (LCI). The team's goal is to support members of LCI in navigating health systems and utilizing resources to promote optimal health and wellness by providing high quality, person-centered, outcome-based care.
The part-time CM Float provides care management in partnership with a part-time Registered Nurse Care Manager (RN CM) Float to LCI members. The part-time CM Float coordinates and designs provision of services and supports based on a comprehensive assessment of the member's identified outcomes and needs.
This part-time CM Float is a job-sharing position. It will provide coverage for Care Managers during periods of extended leaves of absence, filling a time between the departure and hiring/training of a new employee, or any other reason deemed appropriate by the CM Supervisor/Program Manager. The part-time CM Float will be responsible for coordinating shadowing opportunities and warm hand offs as caseload transitions occur. This position will ideally carry a half caseload at a given time but may vary based on business need.
The part-time CM Float position may be required to cover multiple service regions and be flexible with travel time.
Responsibilities & Competencies:
Conduct a comprehensive assessment of the member's outcomes, needs and risks; and conduct a reassessment as the member's outcomes and/or condition changes.
Coordinate care and benefits to ensure a comprehensive support approach, as well as on-going access to federal and state programs.
Monitor and evaluate the members' outcome-based member-centered plans, considering cost and effectiveness in authorizing services and choosing providers.
Coordinate and participate in home visits and care conferences involving the member, their supports and providers to assess and reassess long-term care needs and coordinate appropriate interventions.
Implement risk mitigation strategies to promote the member's health, safety and independence while respecting the member's rights.
Create and maintain member records as required by the Department of Health Services (DHS) contract and LCI policy.
Participate in on-going training as required; maintain current knowledge to ensure compliance with Federal and State regulations, LCI policy and procedure and accepted professional standards.
Strong time management skills to manage workload and caseload logistics.
Ability to establish relationships across LCI with the assigned CM Supervisor, team and their support divisions during times of coverage.
Maintain the confidentiality of member information and protected health information (PHI) as required by State and Federal regulations, including the Health Insurance Portability and Accountability Act (HIPAA) of 1996.
Requirements
Certified Social Worker in the State of Wisconsin with a minimum of one (1) year experience working with at least one of the Family Care target populations, or
A four-year bachelor's degree or more advanced degree in Human Services or related field with one (1) year experience working with at least one of the family care populations, or
A four-year bachelor's degree or more advanced degree in any other area than Human Services with a minimum of three (3) years' experience working with at least one of the Family Care target populations.
Ability to be adaptable, prioritize and work in a fast-paced environment.
Working knowledge of computers, computer programs, typing, and data entry.
Ability to access members' homes which are not required to comply with the ADA regulations.
Ability to lift up to 25lbs.
Current driver's license, acceptable driving record and proof of adequate insurance.

Home Health Occupational Therapist Case Manager to support the Marinette patient service market. Normal office operating hours are 8:00am to 4:30pm. Part time position allows for some flexibility; final usual schedule to be determined upon hire. Position will require a weekend rotation, usually once every 5th to 6th weekend.
Major Responsibilities:
Evaluates the patient's physical, perceptual, cognitive, social, and vocational abilities and their impact on the patient's activities of daily living. Conducts an occupational therapy evaluation of each patient and establishes a plan of care and treatment. Maintains and updates accurate clinical and patient records according to state and federal guidelines.
Plans and implements individual treatment to re-train and teach compensatory techniques for residual disability in activities of daily living, adapted homemaking techniques, bathroom transfers, functional application of mobility skills and, when appropriate, training in adapted work methods.
Discusses caseload issues and changes in case census with management. Communicates problems to physicians and management and clearly states plans, actions and goals so changes are reflected as they occur.
Communicates with physicians and other team members to confirm and update patient plan of treatment and provides physician with status report on a regular basis.
Manages assigned caseload, including scheduling and informing site leadership of coverage needs and ability to provide coverage. Manages the patient's medical condition including medications.
Teaches appropriate treatment procedures and supportive activities to patients and caregivers, and develops plan for obtaining adaptive devices as needed. Recommends adaptive equipment and provides usage training as needed. Acts as a patient advocate to insure optimal outcomes.
May manage the caseload of Occupational Therapy Assistants (OTA), including evaluating their patients, insuring compliance with the established Care Plan and adhering to all State of Wisconsin regulations regarding OTA supervision.
May assist with evaluating patients, turning the patient over to the OTA and informing them of the Plan of Care (POC) per State of Wisconsin regulations.
Provides on-going assessment of patient response to treatments, and teaches and applies interventions as appropriate. Updates and revises plan of care as indicated and provides physician with a status report on a regular basis. Participates in chart audits and continuous quality improvement activities.
Performs various patient positioning and transporting duties, which require lifting and pushing/pulling, while utilizing proper technique.
Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Licensure, Registration, and/or Certification Required:
Occupational Therapist license issued by the state of Wisconsin, and
Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained within 6 months unless department leader has determined it is not required, and

Bachelor's Degree in Occupational Therapy.
Experience Required:
Typically requires 3 years of experience in occupational therapy within an acute hospital, home health care, or other relevant environment.

Essential Functions & Responsibilities
The physician serves as medical director for skilled nursing facilities (SNF) and participates in administrative and quality initiatives for the facilities he or she is responsible for;
Medical directorship functions include attending and participating in monthly quality assurance and process improvement meetings; participating in quality improvement initiatives; providing guidance to facility staff; reviewing and revising (if necessary) facility's clinical guidelines; insuring compliance with state and federal regulations, training facility staff; and supervising facility clinical staff.
In addition to the medical director role, the physician also serves as the attending physician for the residents residing at each facility;
The physician collaborates with Nurse Practitioners and Physician Assistants to manage day to day resident care at each facility. Typically, each facility will have a full-time PA or NP assigned to the facility depending on the acuity and volume of the patient population;
Physician will provide high quality, patient-centered, comprehensive primary care services by rounding on patients residing in SNFs;
Physician is responsible for diagnosing and treating patients with acute illness, such as bacterial and viral infections, as well as chronic diseases and conditions, such as hypertension, heart disease, or diabetes

We're currently recruiting a part time Primary Care (Fam Med or Med/Peds) Physician for Froedtert Drexel Town Square Health Center located in Oak Creek, WI.
The Primary Care practice is very collegial, collaborative, and consultative with exceptional support staff. The Froedtert & the Medical College of Wisconsin regional health network is a partnership between Froedtert Health and the Medical College of Wisconsin supporting a shared mission of patient care, innovation, medical research, and education.

Prepare and administer meals according to client's diet
Take and record vital signs, as well as height and weight, if necessary
Assist client in active and passive range of motion defined by the plan of care, document observed changes and report any changes to the Director of Nursing
Assist client in walking and transportation to doctor appointments, outdoor activities and shopping
Develop and promote a safe environment for quality client care through adherence to established policies, procedures and standards
Ensure client's rights are adhered to
Required Skills
Licensed driver with reliable transportation that is insured in accordance with the organization's requirements
Ability to adhere to HIPAA and maintain client confidentiality
Ability to read, write, speak and understand English and communicate effectively
Ability to travel up to 30 minutes as needed for client care service needs as well as staff and business needs including job required training and conferences.

About You
Qualifications - What You'll Bring:
