The team is multidisciplinary and is led by a LMHC, has 4 case managers, a peer specialist, a psychiatric provider, and a nurse -as well as counselors. The focus is removing barriers by accessing benefits, housing, medications medical services etc. and reduce hospital admissions. The hours are Monday - Friday, 8 AM - 5 PM. Overview: The purpose of case management is to assist clients in gaining access to needed medical, social, educational, and other services. The primary goal of case management is to optimize the functioning of individuals who have complex needs by coordinating the provision of quality treatment and support services in the most efficient and effective manner. The desired outcomes for persons using the service system are self-sufficiency, and satisfaction in the living, learning, work and social environments of their choice. Role Requirements: Bachelor's degree from an accredited university or college with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education, or a related human services field (a related human services field is one in which major course work includes the study of human behavior and development). Must have a minimum of one (1) year of full time or equivalent experience working with: adults experiencing serious mental illness. OR Bachelor's degree from an accredited university or college and three (3) years full time or equivalent experience working with: adults experiencing serious mental illness. Must be able to travel locally to clients' homes, training locations and other locations as necessary. Ability to complete approved mental health targeted case management training within three months of initially providing case management services. Must have knowledge of available resources in the service area for adults with serious mental illness. Must have knowledge of and comply with state and federal statutes, rules and policies that effect the target population. Must be able to provide individualized, integrated mental health, substance abuse and co- occurring services to meet the needs of individuals served and their families. PSYCHIATRIST -Inpatient Clinical -058 JOB SUMMARY: The Psychiatrist position delivers psychiatric healthcare services to individuals in the inpatient and outpatient facilities and to determine the most appropriate level of care required. JOB SUMMARY: The Program Services Manager establishes, administers, and directs the operations of the assigned program. The program Services Manager assumes program oversight responsibility for providing needed treatment, rehabilitation, and support services to identified persons with severe and persistent mental illnesses, co-occurring SA disorders and criminal justice involvement. Understand the fiscal and reporting requirements of grants funding the program. * Develop resources and billing opportunities for program sustainability outside of grant funding. * Supervise medical records management assuring maintenance of the medical record in compliance with agency policies, Medicaid, and other third-party payment requirements; train staff on medical record requirements; regularly review individual assessments, treatment plans, and progress notes written by the staff; and supervise individual staff for medical records mastery. * Ensure data collection for the grants as written and contracted with county partners. RECOVERY SPECIALIST - INPATIENT CLINICAL 058 JOB SUMMARY: The Recovery Specialist functions as a member of the interdisciplinary treatment team with the primary responsibility of monitoring consumer behavior while on the unit, developing treatment plans, discharge planning, and other responsibilities needed to assist consumers in transitioning back to the community following admission to the Crisis Stabilization Unit.
