Essential Job Functions Overview
Collaborates, coaches, and aids individuals with mental illnesses in a residential (community residence, apartment, SRO, enriched crisis, and transitional housing) setting. Primarily responsible for teaching/ supporting the skills necessary for optimal success living independently. Based upon individual needs
and preferences, utilizes recovery-focused, strengths based, person-centered practices to assist individuals in gaining (regaining) skills lost as a result of mental illness. Individual will strive to create a healing environment that respects the perspectives and experiences of the individuals, families, staff, and communities we serve by practicing safe, respectful communication as well as respecting individuals' boundaries and differences.
Assist residents in developing and maintaining a structured daily schedule according to individualized goals and preferences using trauma informed practices.
Utilize motivational interviewing (MI) techniques to provide individual support, training, and assistance in working towards achieving one's personal goals.
Encourage and participate in social, recreational, educational, and cultural activities.
Provide resident transportation via Agency vehicle as needed.
Assist in general program housekeeping to include provision of hands-on assistance to teach the skills needed to maintain personal living spaces.
Develop person-centered service plans for the individuals on your caseload.
Assist residents in monitoring progress on stated goals and provide updates regularly.
Document interactions with residents as it relates to their service plan goals.
Experience, Education, & Physical Qualifications
High School Diploma with two (2) years of relevant experience or an associate degree in human services or related field or bachelor's degree (preferred). Valid NYS Driver's License. May be required to lift (minimum of 35 lbs.), push, pull, or carry heavy objects.

Position Summary:
Health Coach primary responsibility is to engage patients/ participants in collaborative relationships, empowering them to manage their chronic diseases

Contact and performs initial interviews with individuals who are eligible for health coaching program.
Provide coaching to reduce or eliminate behaviors that are considered high risk in participants with chronic diseases, such as Diabetes, Hypertension, Asthma and Hyperlipidemia.
Uses registries to identify patients with newly diagnosed, undiagnosed or poorly controlled chronic conditions and schedules follow-up appointments.
Conducts outreach to patients with overdue screenings or upcoming appointments
Performs intake and assessment of patients with newly diagnosed or poorly controlled chronic conditions, screens for Health Home eligibility, and checks Health Home enrollment
Screens for behavioral health and substance use problems, including depression (PHQ), alcohol abuse (AUDIT), substance abuse (DAST) and smoking status.
Reinforces education provided by PCP or nurse on management of the chronic disease, provides self-management tools, and reviews how to use those tools.
Establishes goals and creates a care plan.the patient, Works with patient to mitigate impacts of social factors on health and functional status, e.g. by arranging transportation for patients.
Does post-visit review of next steps with patient.
Coordinates care, assists with referral management, and conducts between-visit monitoring and outreach.

Education/Experience Required:

Under the direction of the Lead BH Community Health Worker, this position is responsible for providing support and guidance to individuals seeking substance use disorder treatment. The peer recovery coach, an individual in sustained recovery, that can utilize their experiences to mentor or coach, provide and advocate effective recovery based services, Peer mentoring is inclusive of information and guidance relevant to a clients recovery needs. (social skills, Substance use recovery supports, training, mentoring, behavioral skills management, employment, and recreational activities). The peer recovery coach will assist team in: 1. supporting the clients goals for recovery 2. with interventions that will assist clients navigate their journey of recover. This position is responsible for providing supportive social services including bi-directional referrals and follow up as required in accordance with CIFC Health standard of care. And assist with financial insurance assistance to CIFC Health patients seeking Behavioral Health/Substance Use Disorder treatments and their families, In addition, this position is responsible for supporting the CIFC Health providers by providing assistance with substance abuse treatment support and referrals.
Essential Job Responsibilities:
Provide education on substance use disorder and skills training in illness and recovery management to clients and their families.
Ability to work collaboratively with clients to assess their hopes, strengths, accomplishments, and challenges in regards to their recovery goals.
In collaboration with other care team members, works clients to develop and assess needs and develop goals and treatment plans inclusive of the clients personal interests, goals, strengths and challenges regarding recovery.
Actively identify and support referral and linkages to community resources (communities of recovery, educational, vocational, social, cultural, spiritual resources, mutual self-help groups, professional services, etc.) that support the clients recovery goals and interests.

Communicates with other providers internally and externally regarding substance use disorder referral outcomes by creating Provider letters, Discharge Letters, and Summaries in cooperation with providers.
Participates in Clinical Team Meetings and communicates with providers involved in patients care to facilitate all communication and coordination with the team.
Attend provider team meetings to promote collaboration of service and treatment providers.
Attend and participate in individual and/or peer supervision as directed
Adheres to all HIPAA regulations, including those related to the heightened protection of health records, and maintains confidentiality at all times.
Fulfills all compliance and training responsibilities related to position, including compliance with and enforcement of CIFC and CIFC Health policies and procedures.

Requirements:
Successful candidates must possess a minimum High School Diploma or equivalent. (Associate/ Bachelors Degree preferred), Lived experience and/or in recovery from Substance Use for minimum of 2 years. Certified Addictions Recovery Coach preferred or willingness to obtain. Certified Addictions Recovery Coach status or willingness to obtain. Community health experience and/or community resource knowledge strongly preferred. Computer, Electronic Health records experience including using the internet. Bilingual abilities preferred.

Under the direction of the Executive/Wellness/Program Director and in accordance with the policies of the YMCA of Southwest Florida, the Wellness Coach will be responsible for the delivery of safe, effective instruction to members of exercise equipment and interaction with all members utilizing the wellness facility.
ESSENTIAL FUNCTIONS:
* Staff is expected to act professionally and dress according to the requirements: uniform shirt, neat shorts or warm-up pants (no spandex), sneakers and YMCA name tag.
* All Staff should arrive promptly for their shift. Opening personnel must arrive with adequate time prepare facility for opening.
* Welcome and greet all members as they arrive or shortly thereafter. Get to know the member's names, needs and goals. When possible, interact with members. Assist with machine usage, appropriate exercise advice - do not give advice that you are not absolutely SURE is safe and appropriate.
* Make sure that first time users are encouraged to go through the orientation process. ALL members must fill out the medical form. If you feel that there is a medical condition that requires further investigation or physician's approval, refer it to the Executive/Wellness/Program Director.
* Assist members with their workouts whenever possible. Make sure that they are using machines properly, both cardiovascular and weight training.
* Circulate and be visible to members, even when it is slow.
* Give facility tours as needed to assist Membership Services Staff
* Keep the sign-up sheets neat and orderly, assist members with transitions on and off equipment when it is busy.
* Follow cleaning schedule. Refill cleaning bottles as needed. If it is very slow and you are able to continue to circulate, feel free to clean more than your allotted responsibilities!
* Be aware of signs that equipment may be in need of maintenance. If the equipment is not functioning properly, place an out of order sign on it and inform the Executive/Wellness/Program Director. .
* The Wellness Center phones and computers are not for personal use. Personal Emergency calls are permitted.
* Ensure all participants wear appropriate exercise attire and footwear
* Monitor proper executions, form and technique when demonstrating exercises
* Make sure all equipment is put away correctly following your shift. This includes proper restacking of weights.
* Wash, dry and fold towels as needed
* Wellness Coaches are expected to find their own substitutes if they are not work their scheduled shift and communicate the substitute to the supervisor in a timely manner.
* Attend regularly scheduled, mandatory staff meetings.
* Utilize time clock and monitor timesheet. Bring any corrections to supervisor within a timely manner.
* Responsible for upholding all staff safety requirements including cleaning protocols throughout the day, as required by my department.
Opening Shifts:
* Possess facility keys. Insure Keys are secure and notify supervisor of loss.
* Insures facility is open on time as scheduled.
* Insure all electronic cardio equipment are on and ready for use at time of opening.
* All other duties as assigned

* Completion of YMCA's Blood Borne Pathogens training annually
