The position is responsible for the intake, placement and continuous support for students who require additional accommodations and/or support to be successful within the Extended School Services programs. The Inclusion Specialist collaborates with students, families, program staff and district staff to ensure the needs of each student are met. This position is within the Community Education Department and is year round.

WHAT YOU'LL BE WORKING ON:
* Ensure Extended School Services programs maintain compliance with the Americans with Disabilities Act (ADA), Individuals with Disabilities Education Act (IDEA) and Section 504 Rehabilitation Act regarding the accommodation of the individuals with identified inclusion need.
* Review and gather registration information to identify situations that need further review of IEP's, 504 Plans and other clarifying information.
* Create and update individual assessments for each student with identified inclusion needs using data gathered from a variety of resources including students, parents, staff, district staff and others.
* Consult with program and district staff about accommodations, support and behavior management of students.
* Assist Extended School Services Manager with Local Special Needs Levy budget and qualifying logistics as well as tracking of students for state/annual reports.
* Schedule and participate in parent/student meetings in conjunction with appropriate district and program staff to ensure student's success in program.
* Identify and assist with implementation of action plan to ensure student success.
* Work with program staff, Coordinators and Manager when necessary in recruiting, identifying, hiring and supervising additional adult support needed for students.
* Participate in program as additional support for student if needed or fill in when subs are not available.
* Identify, train, support and mentor program staff in inclusion needs, matters and positive behavior support.
* Maintain proper documentation of support needed in Extended School Services for each student and ensure appropriate information and individual plans are shared with program staff.
* Visit program sites and observe students with inclusion needs or behavioral issues, as needed.
* Lead trainings with program staff as needed to ensure student success.
* Evaluate each student's accommodation plan and support as needed.
* Identify practices and procedures and assist in the development of improvement plans that are consistent with district and department guidelines, program goals and health recommendations.
* Meet regularly with and serve as liaison with Lakeville Area Schools Special Education Department as well as Intermediate School District #917.
* Establish relationships with other resources and organizations to maintain cutting edge knowledge of the field.
* Identify needed supplies and /or equipment to enhance program delivery for students with special needs.
* Attend department meetings, district meetings and PLCs.
* Attend professional development opportunities and stay up to date with current trends.
* Ability to communicate clearly and sensitively to parents, staff and students concerning policy, mission and procedure, both written and orally.
* Ability to exercise good judgment and discretion, especially with high regards to sensitive or confidential information.

* Knowledge of developmentally appropriate activities of children.

POSITION SUMMARY:
The primary goal of the Health Care Specialist is to care for student health, injuries, and/or illnesses in an expedient manner. The Health Care Specialist must also demonstrate the ability and desire to work with medically fragile and/or cognitively impaired students and have the ability to perform technical nursing procedures under the direction of a Licensed School Nurse.
PRIMARY JOB FUNCTIONS:
1. First Aid:
a. Administer first aid, emergency, and episodic care accordingly to their nursing licensure and knowledge.
b. Monitor students and refer to Licensed School Nurse when assessments, evaluations, or nursing interventions may be necessary.
2. Cooperation:
* Assist in planning with the Licensed School Nurse as the LSN plans services for students with special needs (i.e., technical nursing procedures, homebound services, transportation, or excessive absence follow-up).
* Assist with Special Ed evaluations.
* Develop Individual Health Plans (IHP) under the supervision of the LSN.
* Function as a team member of Health Services so student health is maintained or improved and learning is facilitated. Attend IEP meetings as Health Services, TAT or SST representative.
3. Record Keeping:
* Responsible for health records including the emergency card, pupil health records, physicals, and medical documentation.
* Maintain data logs according to Lead Nurses request.
* Compile logs into a year-end report (May of each year).
* Accurately document all duties performed and any other necessary reasons.
* Maintain confidential health files for students; restrict access to staff with "need to know."
* Maintain third party billing documentation.
4. Student Wellness:
* Assist with school screenings such as vision, hearing, and early childhood under supervision of a Licensed School Nurse.
* Comply with procedures for communicable diseases, under direction of Licensed School Nurse.
* Reduce the spread of communicable diseases by collecting student immunization data according to State mandates.
5. Delegated Functions:
* Administer medications, simple & complex, to students under the direction of a licensed school nurse and school policy (ex: G-tube, respiratory suctioning, complex wound dressing, catheterization and port care).
* Support the technical health needs of students as directed and delegated by a Licensed School Nurse.
6. Other duties as assigned:

POSITION SUMMARY:
Provide assistance to students and staff to help students function effectively in the school environment. Provide guidance and support in programming and development for students with self-regulation needs.
PRIMARY JOB FUNCTIONS:

Job Summary:
The Inclusion Site Specialist works directly with all children in out-of-school time programs offering direct support in addressing social emotional, health and safety, and behavior needs. In this role, they work alongside the Site Supervisors, Out-of-School Time Site Support Specialists and the Inclusion Coordinator directly implementing inclusion modifications and accommodations and supporting the needs of individual students. They participate in regular trainings and meetings to develop new inclusion skills. They plan and offer activities for all kids, are in rotation and engage in all aspects of the Kids Club program including opening and closing sites, supporting emergency responses and administering medications. This position will float to sites to support inclusion or other needs as required by the program. This position backs up the Site Supervisor when not available.
Position Summary:
* Work with all kids as a regular member of the Kids Club program. Plan and lead activities, engage in activity rotation on site. Support out-of-school (OST) time programs as needed under the guidance of program coordinators.
* Collaborate with the Site Supervisor and other site staff to support invisible inclusion, offering support when behaviors or emotions are dysregulated. Promptly address student needs, intervening with positive behavior intervention strategies, accommodations and modifications.
* Inform Site Supervisor, Out-of-School Time Site Support Specialist (OST) or Inclusion and Program Coordinators of any observations regarding difficulties in student adjustment, learning, social interaction, behavior, and/or the physical functioning of individual children.
* Know and understand the health, safety and behavioral needs of each child at your site/s. Actively recommend and implement reasonable accommodations and modifications as established by the program. Support the development and adaptation of site support and inclusion plans in collaboration with Site Supervisors, OST and Inclusion Coordinator.
* Float as necessary to sites where student needs require reduced ratio, primary care support and/or to support overall ratios.
* Along with Site Supervisors, support provision of special services to students with disabilities including but not limited to toileting, feeding, and administration of medications in compliance with all district and OST policies and procedures.
* Support streamlined, confidential and consistent communication of needs across the program. Assist Supervisors and OSTs in maintaining and updating the Health, Safety and Inclusion binder, Site Support Plans, Inclusion Plans, Special Needs Lists and all documents related to inclusion. Complete documentation as necessary including all Department of Human Services Certification reporting.
* Attend regular team meetings with OSTs and Inclusion Coordinator to review student needs, discuss special needs topics, and share resources. Attend site-based and other program meetings, events and trainings as required.
* Attend student support meetings as needed, and regularly communicate with families and district staff regarding student needs following district and program policies and practices.
* Along with Site Supervisors understand and implement all facets of behavior management systems and program strategies, modeling appropriate behavior to guide children in developing positive social skills.

Notify Site Supervisor about equipment that needs repair or supplies that need purchasing.

POSITION SUMMARY
The State Reporting Coordinator oversees compliance with federal and state reporting requirements. This role includes supervising information systems professionals and interpreting data accountability statutes, regulations, and requirements to ensure accurate and timely submission of data to federal and state agencies. As a system administrator for Infinite Campus Online Registration, the Coordinator manages the online enrollment system, troubleshoots issues, and works with the team to support end users. Additionally, the role involves advising district leadership on compliance, training staff, and continuously improving data management processes to ensure accuracy, privacy, and security of student data, directly impacting district funding and accountability.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
The essential functions of this position may include, but are not limited to, the following fundamental duties:
* Oversee Federal and State reporting requirements.
* Supervise and lead information systems professionals, providing guidance, support, and mentoring to ensure efficient and effective performance.
* Interpret and apply federal and state data accountability and reporting statutes, regulations and requirements to prepare and submit data to federal and state agencies, encompassing MARSS, Ed-Fi, CRDC, DIRS, MCCC, Carl Perkins, OLL, etc.
* Apply all applicable local, state, and federal statutes and rules to the district's reporting and data keeping practices, including the recording of specialized student data in a private and confidential manner.
* Lead the development and implementation of practices and procedures to update and validate data within the district's student information system to minimize errors that may impact student data reporting and funding.
* In coordination with the Information Systems team, train site and district staff in the appropriate local, state, and federal reporting practices, as well as district procedures and practices.
* Serve as a system administrator for Infinite Campus Online Registration, including developing and monitoring the online enrollment system, as well troubleshooting system issues and supporting end users.
* Collaborate with other system administrators to integrate the district's ticketing system seamlessly with both our online enrollment system and our departing student processes.
* Support end users by providing access to student data through the use of the district's student information system's reporting tools when appropriate while adhering to the district's procedures and policies relating to student data privacy and security.
* Advise district leadership on the local, state, and federal requirements as it relates to student data reporting.
* Serve as a system administrator within the district's student information system.
* Work with department members and district staff to continuously improve and implement procedures for the effective collection, storage, retrieval, protection, and dissemination of student information in accord with the needs and policies of the school district and applicable local, state, and federal laws.

Position Description/Qualifications:

* Interpersonal skills, that lend themselves to a compatible working relationship with students and other adults in the work place as determined by the principal.
* Must have the ability to lift and toilet student.
