Reports to: Physical Medicine and Rehabilitation Supervisor/Director/Manager Essential Functions Performs and documents initial and ongoing assessments of patient's condition. Designs and implements a plan of care which is appropriate to problems identified and involves the patient and family. Educates and counsels patients and families regarding treatment plans and progress. Attends and participates in patient care coordination meetings, team and family conferences, as needed. Plans and/or assists with patient discharge from therapy services. Supervises activities of clinical fellows, students, and other support personnel. Evaluation of inpatient performed within twenty-four (24) hours of received orders. Completion of electronic outpatient evaluation within 72 working hours of visit Minimum Education Bachelor's degree □ Preferred X Required Master's degree X Preferred □ Required Required Skills Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.
