I. JOB SUMMARY
The Registered Nurse (RN) provides direct nursing care in accordance with established policies, procedures and protocols of the healthcare organization, while complying with laws and regulations in maintaining patient information. The Opportunity
The RN Case Manager serves as a core member of the care team by coordinating and supporting healthcare within the facility and coordinating referrals for services outside the clinic. Serves as a liaison between the patient, provider and community referral agencies. Performs needs-based assessment of clients. Collects specific data, assesses the client's condition, provides accurate consistent information, formulates and recommends a plan, negotiates appropriate options with the client, and validates the plan with the client for completeness and accuracy. Provides disease management and health coaching; makes appropriate referrals for community-based and LMH resources; coordinates healthcare services and assists with scheduling when needed; advocates for patients within the health system or with community agencies; and serves as a liaison between the patient, provider and other agencies.
Employs care management skills and proven patient facilitation techniques (motivational interviewing, teach-back method, etc) throughout.
Maintains longitudinal care plan and record of services and outcomes.
Ensures CMS or other payer requirements for billing are met and documented.

The Employee Health Nurse helps to ensure the health of LMH Health associates through management of employee injury and illness follow-up and reporting, staff education, and personal protective equipment and immunization programs. Incumbent serves as a resource to staff with questions regarding communicable illnesses.
II. ESSENTIAL JOB RESPONSIBILITIES
Advise and research current best practices for employee health policy and procedures. Manage employee injury internal investigation and reporting process; regularly communicate with workers' compensation carrier and evaluate accommodations and return to work options for associates with restricted duties. Collaborate with Human Resources regarding these issues and other non-work related medical restrictions and employee health related concerns. Develop therapeutic relationships with associates promoting decision making based on the assessment of their medical histories, symptoms, examinations, diagnosis, conditions and treatments. Communicate and collaborate concisely and continuously among clinical team, including physicians, both verbally and in documentation focusing on the associates identified problems and needs to optimize their health outcomes. Investigate, follow-up, consult and problem solve with associates regarding infection control questions or concerns. Track, trend and report data regarding Infection Prevention and Environment of Care Committee requirements. Trend employee illnesses/injuries to identify safety and health issues. Collect and report data for employee health topics, such as lifting practices. Collaborate with Human Resources and other leaders as situations require. Exemplifies professionalism: applies lifelong learning; contributes to the science of nursing; participates in governance; shares knowledge to expand the scale and scope of best practices with others; builds the ethical, moral and regulatory base for best practices and processes of patient and unit care. Performs other duties as assigned in line with current basic nursing scope.
