This position performs resident/patient care activities and related services necessary in caring for the personal needs, safety, and comfort of residents/patients as assigned. Assists in providing a positive physical, social, and psychological environment. The CNA II relates positively, effectively, and appropriately with residents/patients, families, community members, volunteers and other facility staff. Successful incumbents have fast and adaptive problem-solving skills who value collaboration and teamwork. This position has an in-depth understanding of patient care policies and regulations and is comfortable working with specific populations of patients, including elderly patients and those with special needs.
Essential Functions and Responsibilities:
1. Assists with resident/patient care needs and comforts including but not limited to:
Assists or gives oral hygiene and mouth care.
Assists with bathing functions and dressing/undressing as necessary.
Assists with nail care, shaving and hair care.
Assists with lifting, turning, positioning and transporting residents.
Makes both occupied and unoccupied bed (as assigned).
Keeps residents/patients dry (i.e. change gown, clothing, linen, etc.) when they become soiled or wet.
Assists resident/patient with bowel or bladder functions.
Checks resident/patients frequently to assure that their personal care needs are being met.
2. Assists with nursing functions as follows:

Qualifications
Required:
Current CertifiedNursingAssistant (CNA) certification.
Current Basic Life Support (BLS) certification.

Essential Duties and Responsibilities
Works effectively as a team member in the delivery of care and assists in the orientation of new employees. Maintains good working relations with nursing unit staff and other facility employees through appropriate communication.
Performs or assists the residents in performing specific activities of daily living while allowing for independence (as appropriate) according to established procedures in the resident's care plan and accurately reports results and provides input to the nurse.
Instills confidence in residents by maintaining a neat appearance and performing duties in a competent and caring manner. Reports observations of residents to the nurse caring for the resident to ensure professional assessment.
Maintains a safe and clean environment for residents and co-workers by performing infection control procedures in accordance with facility policies and procedures.
Performs various tasks such as: assures resident clothing is clean and properly marked, serves and collects meals and snacks, encourages and/or assists with feeding, answers call lights, transports residents within the facility, and assists with admission orientation of residents to the facility environment.
Licensure Certification:
CertifiedNursingAssistant (CNA) certificate and CPR certification required.
Physical/Mental:
Physical effort required by the job includes standing, walking stooping, pulling, pushing, and lifting up to 50 pounds at times.

Responsibilities
Assists physicians and nurses in the performance of diagnostic examinations by performing duties such as monitoring vital signs, preparing and positioning the patient, or by performing a variety of routine diagnostic tests such as glucometer test for blood glucose level, collection of urine sample, etc.
Assists physicians and nurses in the care and treatment of veteran patients receiving medical, surgical or skilled long term treatment
Provides a full range of patient care to complex patients with a variety of physical and/or behavioral problems including personal nursing care, support duties, and treatments
Uses good communication skills in dealing with patients, some of whom are unusually difficult to care for or communicate with
Complies with universal precaution and with all infection control policies and procedures
Makes rounds to ensure visual contact, safety, and comfort of each patient
Maintains a safe/clean work environment and completes assigned environmental duties
Enters specific information onto the patients' records using the Computerized Patient Record System (CPRS)
Performs direct patient support work including personal patient care and/or basic patient education
