About the Role The Home Study CaseWorker plays a critical role in evaluating a potential sponsor's ability to provide a safe, stable, and supportive environment for an unaccompanied child prior to reunification. This includes conducting in-depth, trauma-informed home visits; verifying information gathered by the care provider; and assessing all potential risks, protective factors, and support needs. The HS CaseWorker collaborates closely with the HS Supervisor and relevant stakeholders including the care provider's case manager and clinician, the sponsor, and, when necessary, family members of the sponsor or minor to ensure the child's safety and well-being. Home Study services are provided throughout the New York City tri-state area and nationally. This role requires frequent travel across states and may include overnight stays to complete in-person assessments as needed.
What You'll Do
* Initiate contact with the referring care provider (e.g., case manager, clinician) upon case activation to review the child's background and coordinate information-sharing
* Review the child's history to guide a trauma-responsive approach and ensure age-appropriate, culturally sensitive engagement throughout the assessment process.
* • Provide Home Study services by conducting in-home assessments and interviews with sponsors and household members to evaluate safety, caregiver capacity, and readiness for reunification.
* • Identify and flag any red Flags or safety concerns (e.g., abuse, neglect, trafficking, special needs, or risk of exploitation) and maintain high-level communication with the HS Supervisor to discuss concerns, timelines, or case support needs.
* • Educate sponsors on reunification responsibilities, including providing a safe and stable home, supporting access to education and healthcare, complying with immigration obligations, and fulfilling the Sponsor Care Agreement. Ensure sponsors are informed of the child's rights, including the right to attend school, be protected from abuse or neglect, and access essential services.
* Submit high-quality Home Study reports within 10 calendar days of referral acceptance, ensuring accuracy, objectivity, and alignment with ORR and agency standards.
* Routinely audit and maintain accurate Home Study case Diles (physical and electronic), ensuring all required documents are complete. Close out cases in accordance with ORR and program guidelines, including timely updates to the UC Portal and internal trackers.
* Report any suspected abuse, neglect, or trafficking in accordance with state-specific mandated reporting laws and ORR procedures.
* Be available and Flexible to accommodate evening, after-hours, and weekend work as needed to meet case deadlines and report requirements.
* Complete all required ORR and agency trainings and remain current on child welfare policies, best practices, and program protocols
* Participate in regular supervision, case staffing, and team meetings, both virtually and in person.
* Must have excellent verbal and written communication skills, including the ability to maintain clear, consistent, and timely communication with supervisors. Given the fast-paced nature of the program, strong communication is essential to ensure alignment of priorities, deadlines, and case needs
* Perform other duties as assigned by the supervisor or Program Director.
Your Qualifications
* Bachelor's degree in social work (BSW) or a related Field such as education, psychology, sociology, or other behavioral sciences.
* Valid driver's license
* Valid TSA-compliant identification, as travel, including air travel will be required as part of this role
* Minimum of two years of experience working with youth in the social service or child welfare Field.
* Skilled in writing professional, thorough, and timely assessment reports.
* Knowledge of child welfare systems and immigration-related policies
* Familiarity with local community resources and social service systems.
* Strong verbal and written communication skills, with the ability to manage high-volume report writing and meet strict deadlines.
* Proficiency in Microsoft Office and case management systems (e.g., Connections preferred).
* Bilingual in English and Spanish (required)

Work Environment:
This hybrid role requires three in-office days and two remote days per week, with Flexibility for after-hours, weekend, and holiday support as needed to meet report deadlines and expectations.
Position Type/Expected Hours of Work:
This is a full-time position. While the program primarily operates Monday through Friday from 9:00 AM to 5:00 PM, a 10:00 AM to 6:00 PM schedule is also available. Flexibility is required, as staff will frequently need to work evenings, weekends, or holidays for case-related matters, including accommodating sponsor availability, fulfilling travel requirements, or ensuring timely completion of Home Study reports in accordance with ORR deadlines.

Responsibilities
The Family Resource CaseWorker (FRCW) plays a critical role within the MST (Multisystemic Therapy) team by providing case management support to families. EFFC Program Description
Catholic Guardian Services' Enhanced Family Foster Care (EFFC) program provides family permanency services to children and families served within the program. Our program teams are interdisciplinary and are composed of Family Specialists/Case Planners, Social Work Supervisors, Directors of Socio Services, Parent Advocates, Case Aides and Socio-Therapists.
Our work includes the implementation of evidence-based models to address trauma, mental health and other challenges faced by children and families. We successfully provide wrap-around services and support during and after foster care placement by having direct access to Catholic Guardian Services' portfolio of social services and behavioral health programs. These programs include children's health home care management services, evidence-based preventive services programs, outpatient mental health services, career development and job readiness services for adolescent. Our Enhanced Family Foster Care Teams closely collaborate with these programs to ensure ongoing support to our children and families.
Major Duties & Responsibilities:
Participates in training and consultation sessions to develop skills in evidence-based and evidence-informed models and ensures that direct reports effectively utilize these models and do so with fidelity.
Attends internal and external meetings as necessary. Including interdisciplinary team meetings.
Maintains a professional and collaborative approach while interacting with the Socio-Therapist Team.

COMPLIANCE OVERSIGHT
Case Planners/Family Specialists are instrumental in making sure that our casework practice and service delivery are aligned with the agency's and ACS' performance expectations. As such the agency's expectation is that all foster care programs Case Planners/Family Specialists adhere to the following:
Comply with all due dates set by your supervisor for the completion and submission of all FASPs, PHRs, PYA and CANS.
Comply with all National Youth Transitional Database (NYTD) entries in Connections. This includes updating education tabs, making sure that ILS service goals are listed in all FASPs (or Plan Amendment) for all eligible youths, and documenting tribal affiliation progress notes.
Ensuring that Family Team Conferences (SPRs) are scheduled and held by identified due dates. If a conference does not take place on the first scheduled date a rescheduled date must be immediately requested.
To prevent higher rates of missed and rescheduled conference Case Planners are expected to reach out to birth families, foster families and children to identify good/bad times to hold the conference.
Monthly submission of your Case Planners' Self-Reporting Contact Compliance Report. Due to your supervisor the last workday of the month. The report tracks all required contacts and the timely documentation of these contacts.
Timely documentation of all progress notes. As per agency policy progress notes are to be documented within five (5) days of the event/activity.
Documenting child placement status change in Connections via a Notice/Approval Progress Note and making sure that a Child Tracking Form is submitted to your supervisor within 24 hours of such status change (i.e. AWOL, trial discharge, final discharge, hospitalization, detention facilities, etc.).
Monthly submission of your FEDERAL CONTACTS (Case Planner-Foster Child face-to-face contact) calendars to your supervisor by the 4th Monday of the current month for the upcoming month.
Making a successful face-to-face contact with EVERY child on your caseload by the 15th of each month and bringing missed contacts to the attention of your supervisor if not achieved by required date.
Ensuring the timely completion of-
PYA Discharge Checklists
Sex trafficking screening in the Child Trafficking Database (CTDB)
MHCU hospital admission and discharge notifications to ACS
Making sure that you are prepared for and attend ALL court hearings on time. This includes preparing the Court Update Report
before
the court date, discussing the case with your supervisor
before
court, reaching out to the FCLS Attorney
before
the court hearing.
Attending and being prepared for all scheduled supervision sessions as scheduled by your supervisor, attending all unit meetings and program/site meetings

To prevent higher rates of missed and rescheduled conference Case Planners are expected to reach out to birth families, foster families and children to identify good/bad times to hold the conference.
Monthly submission of your Case Planners' Self-Reporting Contact Compliance Report. Due to your supervisor the last workday of the month. The report tracks all required contacts and the timely documentation of these contacts.
Timely documentation of all progress notes. As per agency policy progress notes are to be documented within five (5) days of the event/activity.

Case Planner is responsible for clients in New York Foundling's NY 15/15 & ESSHI Vital Brookdale young adult housing program. Interventionist will provide direct home-based interventions to address risk factors, increase protective factors, and ensure the safety of families.
Responsibilities
Reports to assigned program supervisor or covering supervisor.
Provides home-based clinical treatment utilizing the Functional Family Therapy - Therapeutic Case Management (FFT-TCM) model to assigned caseload of up to 10 cases and provides coverage for peer's caseload as needed
Manages the extensive travel requirements involved in serving families in their homes.
Conducts assessments including intakes, review of referral information, identifying and engaging key participants, identifying relational patterns, and individualized treatment planning.
Engages families to sign for services and completes intakes with families within the required ACS timeframes.
Participates in all required agency, model and stakeholder trainings, individual and group coaching and staff meetings.
Adheres to model expectations and implement with fidelity including the timely completion of progress notes, session planning guides, session impact scales and appointment notes in the Care4 documentation system.
Actively participate in weekly consultation and supervision and site visits when they occur.
Consistently meets all program and model Key Performance Indicators (including session pacing, required casework contacts, successful client outcomes, timely completion of Family Assessment Service Plans, data entry in all systems, etc.).

* To request all school and medical reports; document in Connections utilizing appropriate progress note template and file in case record. Place printed copies of all important document in case files.
* To assist the families in accessing in-house and community-based services to address issues that place a child at risk of foster care placement and promote maximum safety, supervision and support for children and their families. This includes but is not limited to accompanying parents to programs, other service providers, domestic violence shelters, and any concrete services that may be needed. Complete referral applications and gather all required support documentation.
* To enter their progress notes in CONNX within 5 days of the occurrence utilizing progress note template.
