About the Role
Case Aide performs various supportive activities for casework services in support of the needs of the Family Foster Care Program. Duties may include "support activities" such as collecting information or investigating situations to obtain information about family situations; directly assisting children and family members in various ways, e.g., providing transportation, arranging home visits, accompanying children to service providers, participating in and reporting observations and findings at case conferences. Work is performed under the supervision of a Case Work Supervisor.
Responsibilities
* Assists with locating information regarding parents/families and resources in the community and communicates information to case planners.
* Follows through on case aide related assignments, i.e., make home visits when approved by the Case Work Supervisor, help supervise child-family contact visits. Accompany children to service providers.
* Assist with office clerical duties, including photocopying, filing documents in case records, processing letters and other mailings, serving as a receptionist - answering telephones and taking messages.
* Assists in maintaining an orderly, safe, and informative on-site environment for children and parents.
* Promptly reports hazards and all observations suggesting suspected abuse, neglect, or exploitation of children to the appropriate individual.
* Willing to travel.
* Performs related and similar duties as required or assigned.
Qualifications
* Associate degree is strongly preferred plus a minimum of two years of prior experience as an Office Assistant.
* High School diploma or GED equivalent also considered with prior experience in child welfare environment.
* Valid Driver's License is needed.
* Ability to carry out assignments with attention to accurateness, thoroughness, and timeliness and with minimal supervision.
* Demonstrated ability to rotate positively to people from diverse backgrounds and lifestyles. Must be LGBTQ affirming.
* Excellent interpersonal and customer services skills.

EFFC & SFFC Program Description Catholic Guardian Services' Enhanced Family Foster Care (EFFC) and Specialized Family Foster Care Programs (SFFC) provide family permanency services to children and families served within the program. Our program teams are interdisciplinary and are composed of Case Planners, Social Work
Supervisors, Directors of Socio Services, Parent Advocates, Case Aides and Socio-Therapists.
Additionally, our middle school, high school and college students receive academic, employment
readiness and housing support from our Journey Program (Fair Futures).
Our work includes the implementation of evidence-based models to address trauma, mental health and
other challenges faced by children and families. We successfully provide wrap-around services and
support during and after foster care placement by having direct access to Catholic Guardian
Services' portfolio of social services and behavioral health programs. These programs include
children's health home care management services, evidence-based preventive services programs,
outpatient mental health services, career development and job readiness services for adolescent.
Our Enhanced and Specialized Family Foster Care Teams closely collaborate with these programs to
ensure ongoing support to our children and families.
POSITION DESCRIPTION:
The Adoption/KinGap Case Aide is responsible for ensuring the permanency of assigned Adoption and
KinGap related cases. The Case Aide works directly with pre-adoptive foster families, kinship
foster families, case planning staff, ACS and the courts to coordinate and expedite permanency via
adoption and KinGap finalizations.
The Adoption/KinGap Case Aide is responsible for representing Catholic Guardian
Services in a professional and responsible manner at all times
MAJOR DUTIES & RESPONSIBILITIES:
? Provides exceptional customer service to pre-adoptive and kinship foster parents by assisting
them to gather and complete needed adoption and KinGap documentation leading.
? Responsible for the facilitation and completion of the KinGap packets including, the
application, expense agreement, affidavit of relations, and supporting documentation.
? Collaborates with the agency's adoption subsidy point person and the Special/Exceptional Rate
Liaison.
? Ensures that necessary paperwork required to finalize the Adoption and/or KinGap is obtained.
? Works directly with Case Planning staff and liaises directly with ACS KinGap Unit to
troubleshoot any existing barriers to finalization.
? Educates foster families and staff (through trainings/ in-services, meetings, etc.) on the
nuances of Adoption and KinGap forms.
? Secures Remand and Permanency Hearing Orders for all children placed in kinship foster homes.
Electronically stores the orders for future use in the processing of KinGap applications.
? Prepares reports for assigned case planning team updating them on the status of adoption and
KinGap documents.
? Documents all case activities in CONNECTIONS.
? As needed, participates in Interdisciplinary Team Meetings (ITM), Family Team Conferences
(FTCs), court hearings as required by family court, and other meetings as needed/required.
? Attend weekly Freed Child/KinGap telephone conferences.
? Attends and completes in-person and virtual trainings facilitated by the agency, ACS, and other
outside organizations.

CGSs Unaccompanied Children Services Program serves children/youth presenting a range of challenging behaviors, especially those related to symptoms of trauma. The Programs services are provided by bilingual (Spanish/English) staff who receives specialized training in the special needs of refugee children. The program provides a safe, therapeutic environment in a family-like setting that addresses the requirements of each child/youth in a manner that is sensitive to his or her age; preserves cultural, ethnic and religious heritage; and meets dietary needs, native language, sexual orientation, gender identity, and other critical individual needs.
Responsibilities
The Case Aides responsibilities include but are not limited to the following:
Will be transferred a case at the point the case manager received approval to release a child.
Will be responsible to send the travel request and will contact sponsor, child and foster parent if applicable of the travel date and time
Will be responsible to notify the sponsor of the travel cost and to make arrangements to collect the funds
Conduct case management tasks as assigned
Conduct the 30 day follow up call to sponsor and child and complete the ORR 30 day follow up form
Review the entire file and prepare for closing
Responsible to assure the children and sponsor are aware of any legal proceedings after release.

COMPLIANCE OVERSIGHT
Case Planners/Family Specialists are instrumental in making sure that our casework practice and service delivery are aligned with the agency's and ACS' performance expectations. As such the agency's expectation is that all foster care programs Case Planners/Family Specialists adhere to the following:
Comply with all due dates set by your supervisor for the completion and submission of all FASPs, PHRs, PYA and CANS.
Comply with all National Youth Transitional Database (NYTD) entries in Connections. This includes updating education tabs, making sure that ILS service goals are listed in all FASPs (or Plan Amendment) for all eligible youths, and documenting tribal affiliation progress notes.
Ensuring that Family Team Conferences (SPRs) are scheduled and held by identified due dates. If a conference does not take place on the first scheduled date a rescheduled date must be immediately requested.
To prevent higher rates of missed and rescheduled conference Case Planners are expected to reach out to birth families, foster families and children to identify good/bad times to hold the conference.
Monthly submission of your Case Planners' Self-Reporting Contact Compliance Report. Due to your supervisor the last workday of the month. The report tracks all required contacts and the timely documentation of these contacts.
Timely documentation of all progress notes. As per agency policy progress notes are to be documented within five (5) days of the event/activity.

Work closely with the other Lead Youth Care Specialists and Residence Supervisor to ensure consistency in staffing across shifts and adequate communication during shift changes;
Ensure Primaries uphold expectations of their role and implement trauma informed responses with youth.
Provide leadership, supervision and guidance to Youth Care Specialists to carry out their shift responsibilities seven days a week; Supervise the Youth Care Specialists in assisting the pregnant and parenting residents in developing effective parenting skills and positive behavior while promoting independence;
Share on-call alternating weekend coverage with fellow Lead Youth Care Specialists and Residence Manager.
Counsel the Youth Care Specialists to ensure compliance with Agency rules, policies and procedure, as well as interpreting the Program rules to the Youth Care Specialists and residents;
Complete the bi-weekly schedule of the Youth Care Specialists to ensure adequate staff coverage for the shift, including the review and approval of vacation requests;
Review with Resident Manager bi-weekly Time Saver time cards for the Youth Care Specialists on designated shift; and serve as a backup for timecard approve in absence of Resident Manager.
Ensure that proper documentation is reviewed and submitted in a timely manner, including but not limited to daily shift logs, program notes on residents, medication reports, and incident reports;
Report all critical, serious and unusual incidents to the Director, Social Work Supervisor and Case Planners.
Monitor and review Medication Administration Records (MAR) and submit report to Residence Manager;
Provide weekly supervision to Youth Care Specialists on designated shift;
Ensure that Youth Care Specialists receive all the required trainings (i.e., CPR and First Aid, TCI, Medication Administration, Infection Control, Child Abuse & Prevention), as well as other important trainings geared to the care of pregnant and parenting teens as well as infant & toddler care;
Collaborate with Program Liaison to ensure accurate and effective implementation of recreation and structured programming by Youth Care Specialists and co-facilitate groups when necessary; as well as, ensure that all Youth Care Specialists participate in the training and implementation of Trauma Systems Therapy and the successful dissemination of a strengths based behavior management system among residents, maintaining a thriving therapeutic milieu;
Work collaboratively with case planning team to ensure youths goals are identified and consistently monitored.
Promote and maintain a safe, secure and nurturing environment for residents;
Escort residents to ER visits, medical appointments and school, as directed;
Participate in orientation for new client admissions, treatment update and case planning meetings;
Ensure the posting of all necessary notices on staff and resident bulletin boards, such as safety regulations; residence rules and curfew times, staff schedules, etc.;

About the Role
Case Aide performs various supportive activities for casework services in support of the needs of the Family Foster Care Program. Duties may include “support activities” such as collecting information or investigating situations to obtain information about family situations; directly assisting children and family members in various ways, e.g., providing transportation, arranging home visits, accompanying children to service providers, participating in and reporting observations and findings at case conferences. Work is performed under the supervision of a Case Work Supervisor.
Responsibilities
Assists with locating information regarding parents/families and resources in the community and communicates information to case planners.
Follows through on case aide related assignments, i.e., make home visits when approved by the Case Work Supervisor, help supervise child-family contact visits. Accompany children to service providers.
Assist with office clerical duties, including photocopying, filing documents in case records, processing letters and other mailings, serving as a receptionist - answering telephones and taking messages.
Assists in maintaining an orderly, safe, and informative on-site environment for children and parents.
Promptly reports hazards and all observations suggesting suspected abuse, neglect, or exploitation of children to the appropriate individual.

Qualifications
Associate degree is strongly preferred plus a minimum of two years of prior experience as an Office Assistant.
